The nail in systemic diseases.

Although abnormalities of the nails have been reported in different systemic disorders, most of these abnormalities are nonspecific. This article reports and discusses only those nail signs that provide the clinician with clues for the diagnosis of systemic disorders.

Pediatric nail disorders.

This article lists common pediatric nail disorders seen in infancy and adolescence. The diagnosis of pediatric nail disorders can be a sign of systemic disorders and diseases. The surgical treatment of young children and the treatment of pediatric patients are presented for those who do not frequently treat pediatric patients. The analysis of pediatric onychomycosis is presented, and the incidence and prevalence in the population is discussed.

The yellow nail syndrome.

The yellow nail syndrome is a rare clinical entity which combines three main features: yellow discolouration of the nails, chronic lymphedema, and pleural effusion. Since 1994, chronic rhinosinusitis is associated in this syndrome. A lot of other pathologic conditions are often present. The most frequent are: bronchiectasis, recurrent pneumonia, pericardic effusion, humoral immunity deficiencies. Among the etiopathogeneses advocated, the more credible ones deal with changes of the lymphatic network with hypoplastic vessels leading to inadequate adaptation, after local damage of the tissues. Wh…

Bowen's disease of the nail.

We present a case of Bowen's disease of the nail bed in a 60-year-old male. This case should remind us to biopsy any chronic lesion of the nail in order to reduce the delay in diagnosis. Indeed Bowen's disease of the nail unit often presents as a clinically misleading 'benign' lesion. Early diagnosis is important as this precancerous lesion appears to have a more aggressive behavior when it affects the nail unit. Bowen's disease of the nail is therefore considered as a squamous cell carcinoma, and both entities are grouped under the term 'epidermoid carcinoma'. Surgical excision is the first t

Matricectomy and nail ablation.

Matricectomy refers to the complete extirpation of the nail matrix, resulting in permanent nail loss. Usually however, matricectomy is only partial, restricted to one or both lateral horns of the matrix. Nail ablation is the definitive removal of the entire nail organ. The most important common denominator in the successful matricectomy is the total removal or destruction of the matrix tissue. Matricectomy may be indicated for the management of onychauxis, onychogryphosis, congenital nail dystrophies, and chronic painful nail, such as recalcitrant ingrown toenail or split within the medial or

Understanding nail disorders.

Diagnosing nail dystrophies and understanding their pathogenesis is best accomplished if the origin of the disorder is first established. Exogenous. In this group, the nail plate is altered by exogenous factors and is the site of primary involvement: periungual tissues may be affected secondarily. Endogenous. In this group, the nail plate is altered secondarily by changes occurring in the matrix, proximal nail fold, nail bed, hyponychium or underlying bony phalanx.

Unreamed femoral nailing.

Sixty one femoral fractures treated with ACE unreamed titanium nail (AIM femoral nail, ACE Medical, Los Angeles, CA) were studied. Ten patients died before bony union and three were lost to follow up. Forty eight fractures were followed up for an average of 11.2 months (4-31 months). All fractures united except one in which plating and bone grafting was performed at 6 months due to failure of progression of union. The mean time to bony union was 6.2 months. There was no implant failure but one distal interlocking bolt broke at 6 weeks. No incidence of adult respiratory distress syndrome (ARDS)

Management of nail psoriasis.

Nail psoriasis is often difficult to cure, but may respond to a range of different approaches used alone or together. As with treatment of psoriasis affecting skin, relapse is common and therapies may need to be maintained or repeated. The exact benefits of many of these treatments are not well documented in the literature. Basic nail care is important and topical therapies represent the main modality of treatment for the majority of cases. In severe nail disease, where there is a hypertrophic element, injection therapy with triamcinolone may be helpful. PUVA and other forms of radiation may b

[Nail-patella syndrome].

The nail-patella syndrome is an autosomal dominant trait characterized by abnormalities of the nails, patella and radial head, iliac crest and, in some cases, nephropathy. The genetic defect is localized on chromosome 9q34.1. The clinical features in affected individuals vary greatly. The nephropathy may progress to end-stage renal failure, leading to decreased life expectancy. We report a woman with bilateral hypoplastic thumbnails, who also had aplasia of both patellae and subluxation of the left elbow joint with hypoplasia of the radial head. The mother and the sister of the patient had sim…

Miscellaneous nail presentations.

A variety of individual nail dystrophies that are not categorized easily in other articles are reviewed. Onychoatrophia, anonychia, onychorrhexis, leukonychia, Beau's lines, onycholysis, onychomadesis, onychoschizia, haplonychia, longitudinal melanonychia, and ventral pterygium are included and pictured clinically. Their clinical description, etiology, associated conditions, differential diagnoses, and treatments are discussed and tabulated.

[The yellow nail syndrome].

The yellow nail syndrome is the combination of slow growing yellow discoloured nails, lymphoedema and pleural effusion. The symptoms are supposed to be due to impaired lymphatic drainage. It is a rare clinical condition: through the last 30 years only about a hundred cases have been reported. The treatment is symptomatic (diuretics and pleuradesis). We review the literature and report four cases.

Nail matrix hypergranulosis.

Nail pathology shares some common features with skin pathology, but it also has its own peculiar aspects. The anatomical and physiological characteristics of the nail unit probably play a major role in determining these pathological differences. Although the presence of keratohyaline granules is a normal feature of the skin, there is no granular layer in the normal nail matrix. As a consequence, nail matrix hypergranulosis should be considered a separate entity from skin hypergranulosis. In our review of 150 longitudinal nail biopsy specimens, keratohyaline granules were seen in the nail matri

[Yellow nail syndrome].

The yellow nail syndrome, first described in 1964 by Samman and White, consists of yellowish, dystrophic nails, lymphedema and pleural effusion. We present the clinical findings of a 53 years old woman who suffered of this very rare syndrome. The literature review reveals the different clinical features which were reported to be associated with the yellow nail syndrome. The pathogenesis of this autosomal-dominantly inherited syndrome consists of a dysfunction of the lymphatic drainage system, but details remain unknown. Up to now a causative treatment does not exist but topic tocopherol may im…
