Juvenile nasopharyngeal angiofibroma: an update of therapeutic management.

BACKGROUND: Juvenile nasopharyngeal angiofibroma (JNA) is a benign tumor of the nasopharynx composed of fibrous connective tissue and an abundance of endothelium-lined vascular spaces. The name of the disease is derived from the fact that it occurs during adolescence, almost exclusively in boys. METHODS: This study examines the natural characteristics of JNA, the imaging techniques used to diagnose and stage the neoplasm, and the treatment approaches used to manage the disease. RESULTS: Forty-three cases of JNA diagnosed and treated at the University of Texas M. D. Anderson Cancer Center ove

Primary localized nasopharyngeal amyloidosis. A case report.

Primary localized amyloidosis of the nasopharynx is a rare disease. We present the case of a 13-year-old girl, to our knowledge only the second pediatric case reported in the literature. Symptoms were nasal obstruction and bleeding from the oral cavity. Physical examination revealed a mass in the nasopharynx and left side of the soft palate. The diagnosis was made by biopsy and histopathologic study of the surgical specimen. Immunohistochemical study revealed amyloid light chains (AL). The main treatment was surgical. At follow-up 9 months later, no recurrence had developed. While rare, locali

HLA-A2 allelic microvariants in nasopharyngeal carcinoma.

HLA-A2-positive nasopharyngeal carcinoma (NPC; n = 38) and normal control (n = 51) populations were analysed by high-resolution oligotyping to identify A2 allelic microvariants. Within the control group, A*0207 was found to be the most common allele, accounting for 50% of the A2 frequency in normals. In contrast, this allele was found to be present in only 23% of NPC cases, suggesting a protective effect. Of these 9 NPC patients with A*0207, all were associated with B46, unlike in the control group, where it can be found associated with non-B46 antigens. Another allele, A*0201, which was thoug

The nasopharyngeal airway. Assessment of position by fibreoptic laryngoscopy.

Artificial nasopharyngeal airway position and performance were assessed in 120 anaesthetised adult patients. Using a fibreoptic laryngoscope mean distances from nares to larynx were measured at 209 mm (SD 11) in males and 180 mm (SD 11) in females; those from nares to epiglottis were 159 mm (SD 12) in males and 140 mm (SD 11) in females. Nasopharyngeal airways were frequently misplaced, 60% lying beyond the tip of the epiglottis and 13% lodged in the vallecula. Forty-two percent of subjects had clinical evidence of respiratory obstruction. Nasopharyngeal airway compression in the nasopharynx a

The assessment of hypertrophy of nasopharyngeal tonsil by acoustical methods.

The possibilities for using acoustical analysis as a noninvasive measure of the degree of obstruction created by an enlarged pharyngeal tonsil (adenoid) are discussed. The method capitalizes on the relationship which exists between a specific obstruction of the respiratory tract and the resulting respiratory sound. Results indicate that as the adenoid increases in size relative to the size of the nasopharynx, the dominant components of the respiratory sound spectrum shift towards a higher frequency range. Furthermore, the correlation found between the assessments of the obstruction due to vari

Diabetes insipidus and hypercalcaemia secondary to nasopharyngeal carcinoma.

We report a case of nasopharyngeal squamous carcinoma complicated by diabetes insipidus and hypercalcaemia. As there was no evidence of bony metastases we conclude that this latter finding was due to a humoral factor produced by the tumour. The management of these problems is discussed.

[The tensor tympani muscle reflex in nasopharyngeal carcinoma].

The initial stages of carcinoma of the nasopharynx may be manifested by different types of impaired hearing. The Eustachian tube may be obstructed by a tumour or its orifice compressed, sometimes the tumour penetrates into the middle ear. The authors discuss the position which may develop in infiltration of the semicanalis m. tensoris tympani and how functional disorders of this muscle may be manifested. They describe the provoked reflex of this muscle by trimolar acetic asid and investigate tympanometric reactions of this artificially provoked contraction of the m. tensor tympani in patients

[Erythema nodosum in patients with hypertrophy of the nasopharyngeal tonsil].

The author analyzes the clinical observations of erythema nodosum in patients with hypertrophic nasopharyngeal tonsils. Basing on the literature data and her own observations, she suggests a relationship between adenoid growth and nodular eruption.

Pre-operative embolization of juvenile nasopharyngeal angiofibromas.

Pre-operative embolization of juvenile nasopharyngeal angiofibroma was performed in 15 patients. The lesion was supplied by the internal carotid arteries (8 cases), by the internal maxillary artery (15 cases), the accessory meningeal artery (10 cases) and the ascending pharyngeal artery (10 cases). Superselective embolization of the external carotid artery feeders was performed with Ivalon particles, without neurological complications. Good control of per-operative blood loss was noted in 13 out of 15 cases, 2 patients presenting severe per-operative venous bleeding. Recurrence was noted in on

Nasopharyngeal carcinoma: clinical and radiographic findings in children.

Nasopharyngeal carcinoma (NPC) in childhood occurs so infrequently that it is not suspected in affected children until the disease has been present for a long time and local spreading has occurred. The survival rates are therefore quite poor. Six children with NPC are described. A massive local lymph node spread simulating lymphoma was present in half of the patients; in the other half the disease was more subtle, presenting with epistaxis and CNS involvement. If an evaluation of the nasopharynx were part of the initial physical examination in children, the diagnosis of NPC would be made earli

Lymphoepitheliomas of the larynx (undifferentiated carcinomas of nasopharyngeal type).

Three cases of lymphoepitheliomas of the larynx are reported. This type of carcinoma--which occurs almost exclusively in nasopharynx--is very unfrequent in the larynx (0.2%). In the 3 cases, the tumors arose in a ventricular diverticulum which presented the same lymphoepithelial structure as the tonsils. Under such conditions, these lymphoepitheliomas, which have to be called 'undifferentiated carcinomas of nasopharyngeal type' are believed to develop from the lymphoepithelial tissue of the laryngocele.

Magnetic resonance imaging based on radiomics for differentiating T1-category nasopharyngeal carcinoma from nasopharyngeal lymphoid hyperplasia: a multicenter study.

PURPOSE: To investigate the role of magnetic resonance imaging (MRI) based on radiomics using T2-weighted imaging fat suppression (T2WI-FS) and contrast enhanced T1-weighted imaging (CE-T1WI) sequences in differentiating T1-category nasopharyngeal carcinoma (NPC) from nasopharyngeal lymphoid hyperplasia (NPH). MATERIALS AND METHODS: This study enrolled 614 patients (training dataset: n = 390, internal validation dataset: n = 98, and external validation dataset: n = 126) of T1-category NPC and NPH. Three feature selection methods were used, including analysis of variance, recursive feature eli
