We report the case of a 7-year-old boy who presented with a 1-month history of neck pain, left-sided torticollis, and no neurological deficit. Computed tomography and magnetic resonance imaging revealed an expansile lesion in the axis, with epidural and prevertebral soft tissue components. Histopathologic examination of the biopsy specimen revealed primary vertebral Ewing sarcoma. This is the first case of primary vertebral Ewing sarcoma that has presented with torticollis. It is essential for physicians to be familiar with this condition and the associated imaging findings because early diagn…

BACKGROUND: Chronic neck pain represents a significant public health problem. Despite high prevalence rates, there is a lack of consensus regarding the causes or treatments for this condition. Based on controlled evaluations, the cervical intervertebral discs, facet joints, and atlantoaxial joints have all been implicated as pain generators. Cervical provocation discography, which includes disc stimulation and morphological evaluation, is occasionally used to distinguish a painful disc from other potential sources of pain. Yet in the absence of validation and controlled outcome studies, the pr…

Studies using surface electromyography have demonstrated a reorganization of muscle activation patterns of the neck and shoulder muscles in patients with posttraumatic neck pain disability. The neurophysiologically oriented "pain adaptation" model explains this reorganization as a useful adaptation to prevent further pain and injury. The cognitive-behavioral-oriented "fear avoidance" model suggests that fear of movement, in addition to the effects of pain, modulates the muscle activation level. We analyzed the extent to which pain and fear of movement influenced the activation patterns of the …

Intracranial vertebral artery dissection is an uncommon but important cause of subarachnoid hemorrhage. These patients have a predilection for early rebleeding that makes early diagnosis and intervention imperative. The arterial dissection and associated dissecting aneurysm are often located close to the origin of the posterior-inferior cerebellar artery. Endovascular treatment techniques have been increasingly employed. An illustrative case that was treated successfully with coils but complicated by a posterior-inferior cerebellar artery territory infarction is presented. The objectives are t

BACKGROUND: The role of pain in head and neck cancer is seldom addressed. This retrospective study examined in a group of 190 curatively treated patients to what extent pain complaints should be considered to be the first sign of recurrent disease.
METHODS: The research population exists of 95 patients with a recurrent head and neck carcinoma and control group matched for age, gender, primary tumor, and duration of follow-up.
RESULTS: Of the patients with proven recurrent disease, 70% reported pain complaints as the first symptom. In 35% of these cases, the complaints concerned referred pain

Neck pain in the suboccipital and paracervical region ('coathanger' configuration) is often reported by patients with autonomic failure and orthostatic hypotension. The frequency of this pain, along with pains in the buttock and calf regions, was determined by questionnaire in two major groups with primary chronic autonomic failure--pure autonomic failure (PAF) and multiple system atrophy (MSA). Comparisons were made with Parkinson's disease, cerebellar degeneration and other disorders in which neurological symptoms overlap but in which there was neither autonomic failure nor orthostatic hypot…

Chronic intractable benign pain (CIBP) is defined as non-neoplastic pain of greater than 6 months duration without objective physical findings and known nociceptive peripheral input. To test the CIBP concept, 283 consecutive chronic pain patients were examined independently by a neurosurgeon and physiatrist and only congruent physical findings were coded. Because they did not fit the CIBP definition, patients with the following primary treatment diagnoses were eliminated: degenerative disease of the spine and spinal stenosis; degenerative disease of the hips; radiculopathy; malignancy; deaffer

BACKGROUND: Cervical traction is a common intervention in physical therapy practices to manage cervical pain and reduce neck muscle stiffness (e.g., resistance to passive stretching). However, more definitive research is needed to understand how this intervention influences muscle stiffness in individuals with idiopathic chronic neck pain.
OBJECTIVES: To determine the effect of a single session of supine cervical traction on stiffness of the upper trapezius (UT) and middle scalene (MS) in participants with idiopathic chronic neck pain using ultrasound shear wave elastography.
METHODS: We rec…

BACKGROUND: Chronic pain can affect body perception at the central level by causing the somatosensory cortex to rearrange. Additionally, cervical afferent abnormalities in individuals with neck pain can impair proprioceptive sensitivity, potentially leading to alterations in body alignment and biomechanics. Nevertheless, there are insufficient studies exploring these notions.
OBJECTIVE: The main objective of this study was to compare the head posture and neck proprioceptive sense of individuals with chronic neck pain and healthy controls.
METHODS: Utilizing a cross-sectional study, a total o…

BACKGROUND: To date, there are no studies in the literature that define the internal structure of the Tampa Scale for Kinesiophobia (TSK) in patients with chronic neck pain based on factorial analysis. As such, we aimed to verify and identify the best structure of the Brazilian version of the TSK in patients with chronic neck pain.
METHODS: We included Brazilian participants aged ≥18 years, both sexes, with self-reported neck pain for more than 3 months and pain intensity ≥3 on the Numerical Pain Rating Scale (NPRS). Dimensionality and number of TSK items were assessed using confirmatory fact…

BACKGROUND: Clinical guidelines recommend that people with chronic non-specific neck pain self-manage symptoms with physiotherapy-led exercise. However, current exercise strategies have modest short-term effects, engagement is poor, and 48% of people continue to experience long-term and recurrent pain. Updated exercise strategies co-produced using complex intervention development frameworks are required that consider the behavioural, psychological, environmental, and technical aspects of exercise prescription and patient adherence to optimise symptom outcomes, long-term engagement, and self-ma…

Background and Objectives: Thoracic dysfunction may contribute to neck pain, restricted mobility, and altered spinal alignment in individuals with forward head posture (FHP). This study compared thoracic flexibility exercise (TFE) with cervical stabilization exercise (CSE) in terms of their effects on pain, range of motion (ROM), spinal alignment, proprioception, and neck-related disability in patients with chronic neck pain and FHP. Materials and Methods: Twenty-four participants were randomly assigned to either the TFE group (n = 12) or the CSE group (n = 12). Both groups performed their ass…

Office workers frequently report low back pain (LBP) and neck pain (NP) related to sedentary work. While several prognostic risk factors of muscle performances have been identified, accurate predictive models are essential. This study aimed to develop and validate prediction models of LBP and NP among office workers. A cross-sectional study was conducted with 170 office workers aged 20-59 years, all with over one year of experience and daily computer use exceeding six hours. Participants underwent muscles performance tests for LBP and NP, including assessments of trunk muscles stability and en…
