An unusual cause of mononeuritis multiplex.

A middle-aged man of South Asian decent presented with a 4-month history of bilateral sensory disturbance affecting the median nerve distribution and dorsum of both feet. Neurological examination was otherwise normal. A patchy absence of sensory responses was noted on nerve conduction studies and electromyogram (NCS/EMG). Over the next 3 months sensory symptoms progressed to involve median, radial, ulnar, sural and peroneal nerves bilaterally. Repeat NCS/EMG confirmed a mononeuritis multiplex predominantly involving the sensory fascicles. Areas of hypopigmentation, a right-lower motor facial w

Sheet fitting palsy.

A 73-year-old woman developed an acute thrombosis of a persistent median artery following a Sisyphean struggle with a poorly fitted bedsheet. Ultrasound was able to diagnose the problem in a case where precise localization by electrodiagnostic testing was difficult.

Early onset of carpal tunnel syndrome during pregnancy: case report.

Carpal tunnel syndrome is common during pregnancy and is considered to have a short and benign course. Carpal tunnel syndrome occurs most frequently in third trimester. Patients are usually managed conservatively; symptoms improve after delivery and do not require surgical intervention. We report an interesting case of pregnancy-associated carpal tunnel syndrome in a 23-year-old woman, with detailed electrophysiological studies before and after wrist splinting.

Saphenous mononeuropathy after popliteal vein aneurysm repair.

INTRODUCTION: Saphenous mononeuropathy has been a well recognized consequence of lower extremity surgery. However, this complication has not been previously described with popliteal vein aneurysm repair. CASE PRESENTATION: We report the case of a 42-year-old woman with a saphenous mononeuropathy after popliteal vein aneurysm repair. Her saphenous neuropathy was confirmed by nerve conduction studies. Her case gives us an opportunity to review saphenous mononeuropathy and its many different etiologies. We also review the role of electrodiagnostic studies in the diagnosis of saphenous mononeurop

Cannieu-Riche anastomosis of the ulnar to median nerve in the hand: case report.

We observed in a male cadaver the presence of a new type of very long Cannieu-Riche anastomosis between the proximal portion of the deep branch of the ulnar nerve for the adductor pollicis and ramus of the recurrent branch of the median nerve to the superficial head of the flexor pollicis brevis. The clinical relevance of such a communication is the possible preservation of the function of all or part of thenar muscles from the ulnar nerve in case of median nerve lesion. The ignorance of that anomaly can induce obscure clinical, surgical and electroneuromyographical findings. We report on the

Guillain-Barré syndrome and postbariatric surgery polyneuropathies.

BACKGROUND: Postbariatric surgery poly-neuropathies (BSP) are infrequent events. However, with the number of gastric bypasses performed each year increasing, the incidence of BSP is expected to increase as well. The long-term sequelae of BSP worsen with delays in diagnoses. Therefore, early evaluation, recognition, and treatment are important in minimizing morbidity and mortality. METHODS: We present the case report of a patient who developed a poly-neuropathy after a laparoscopic Roux-en-Y gastric bypass. RESULTS: The patient's symptoms were ultimately determined to result from Guillain-Bar

Truncal polyradiculopathy due to sarcoidosis.

We report the case of a 43-year-old woman who developed multiple cranial nerve palsy, the symptoms of which included hyposmia, visual loss, facial hypoesthesia, facial weakness, dysphagia, gustatory disturbance, and sensory disturbance of the trunk and ulnar side of the bilateral arms. The clinical features included swelling of the bilateral hilar lymph nodes, uveitis, an elevated serum angiotensin-converting enzyme level, and negative tuberculin reactions, which led to a diagnosis of neurosarcoidosis. Her symptoms improved after administration of steroids. An elevated cerebrospinal fluid cell

Symptomatic epineural ganglion cyst of the ulnar nerve in the cubital tunnel: a case report and brief review of the literature.

An unusual case of pain and weakness in the hand and forearm due to a ganglion cyst of the ulnar nerve at the elbow is presented. The patient was managed initially as a case of cervical disc disease and cervical spondylosis and later as a case of carpal tunnel syndrome at an another institution. Cervical radiography and cervical magnetic resonance imaging scans were inconclusive. Neurosurgical referral revealed tenderness at the right cubital tunnel, weakness of the right hand and forearm muscles, and sensory deficit along the medial border of the forearm and the hand. The diagnosis of ulnar n

Pitfalls in electrodiagnosis.

This review describes some of the factors that may lead to erroneous interpretations of electromyographic and nerve conduction studies. Such errors may be due either to technical or to biological factors, and it is imperative that the consequent limitations of the methods be considered in a diagnostic setting. Electrodiagnostic findings should always be interpreted in the clinical context, and since they are rarely specific for a particular disorder or pathology, it is necessary to satisfy several criteria to make a specific diagnosis. The aim of electromyographic examination is to ascertain w

Lack of progression of neurologic deficit in survivors of paralytic polio: a 5-year prospective population-based study.

We completed a prospective, population-based cohort study of polio survivors in Olmsted County, Minnesota, between 1986 and 1993. We identified 50 individuals who had had paralytic polio between 1935 and 1960, as representative of all 300 cases of paralytic polio in the county. We completed detailed quantitative clinical and electrophysiologic studies at entry and after 5 years. These studies demonstrated stable neuromuscular function within the cohort, although 60% of the individuals were symptomatic. In two-thirds of the symptomatic patients, the causes of their symptoms were unrelated to ea

Electrodiagnosis.

Attempts to increase the diagnostic yield and reproducibility of electrophysiological investigation of peripheral nerve disorders have led to the development of new techniques, such as measurement of nerve refractoriness, as well as the re-evaluation, refinement and modification of more conventional nerve conduction tests. Other studies have characterized the clinical and electrophysiological features of the subtypes of polyneuropathies associated with monoclonal gammopathies and have documented their natural history, providing important diagnostic and prognostic information. Techniques origin

Tibial mononeuropathy from a lower limb synovial cyst.

BACKGROUND: Tibial mononeuropathy due to compression by a synovial cyst is uncommon with fewer than 10 cases reported in the literature. METHOD: Case Study. RESULTS: Tibial motor mononeuropathy with focal conduction block between the popliteal fossa and ankle was identified by electrophysiological testing. Magnetic resonance imaging and surgical exploration identified a synovial cyst compressing the nerve just distal to the popliteal fossa. CONCLUSIONS: The combination of electrophysiological localization and imaging in cases of unexplained tibial mononeuropathy may identify an unexpected s

Hypertrophic neuropathy with complete conduction block-- hereditary motor and sensory neuropathy type III.

Hypertrophic neuropathy is a non-specific consequence of repeated demyelination and remyelination, encountered in a wide range of inherited and acquired disorders. We report an 11-year-old boy with HMSN III, a kind of hypertrophic neuropathy, with clinical, electrophysiologic and pathologic data. The electrophysiologic studies show complete conduction block in the upper and lower extremities with severe abnormal spontaneous activities. The pathologic findings of sural nerve reveal prominent hypomyelination, onion bulb formation, and severe endoneurial collagenization. Complete conduction block
