Management of trigeminal neuralgia in sclerosteosis.

BACKGROUND: Sclerosteosis is a rare bone disorder characterized by a progressive craniotubular hyperostosis. The diagnosis of sclerosteosis is based on characteristic clinical and radiographic features and a family history consistent with autosomal recessive inheritance. The skull overgrowth may lead to lethal elevation of intracranial pressure, distortion of the face, and entrapment of cranial nerves, resulting in recurrent facial palsy or secondary trigeminal neuralgia. CASES DESCRIPTION: The authors reported cases of two siblings who were diagnosed with familial sclerosteosis and presented…

[The neurosurgical therapy of trigeminal neuralgia].

The Trigeminal Neuralgie (TN) is he most common cause of facial pain with an incidence of 27 per 100'000 patients per year. The symptoms are paroxysmal stabbing, tearing, and burning pain usually in the area of the second and third trigeminal nerve, that can be provoked by drinking, shaving, chewing or talking. If the classical drug therapy with anticonvulsants is not sufficient or must be discontinued due to side effects, then the retrosigmoidal craniotomy and micro-vascular decompression in vascular-nerve-contact ot partial rhizotomy are good pain-free procedures. Such operations are of low

Assessment and treatment of trigeminal neuralgia.

Patients with trigeminal neuralgia, a rare facial neuropathic pain, present to both medical and dental specialists. International guidelines on diagnosis and management of trigeminal neuralgia provide a useful framework for this article.

Neurectomy for treatment of intercostal neuralgia.

BACKGROUND: Intercostal neuralgia due to surgical injury of the intercostal nerve is difficult to treat. No treatment modality has given effective pain relief. Experience with other painful neuromas has demonstrated that neuroma resection and muscle implantation has been effective in the upper and lower extremities. This approach was applied to patients with intercostal neuralgia. METHODS: A retrospective review was done of 5 consecutive patients who have had neurectomy of one or more intercostal nerves. Preoperative and postoperative pain levels, patient demographics, length of follow-up, an…

[Diagnosis and treatment of trigeminal neuralgia].

Trigeminal neuralgia is a disease affecting older individuals. The clinical hallmark of trigeminal neuralgia is a sudden, excruciating paroxysm of pain in the area of the trigeminal nerve. Drug therapy is considered the first line of treatment for trigeminal neuralgia. Anticonvulsant carbamazepine has been used. If relevant pharmacotherapy has been tried without any effect, other procedures are selected. These procedures are microvascular decompression(a radical technique), glycerol trigeminal rhizotomy, percutaneous trigeminal nerve decompression and nerve block. Nerve block with neurolytic s…

Postherpetic neuralgia in the cancer patient.

Postherpetic neuralgia (PHN) is the most common and devastating complication of acute herpes zoster (HZ). HZ occurs more frequently in the patient with human immunodeficiency virus (HIV) and with certain leukemias and lymphomas. PHN occurs more frequently in the elderly, in patients with severe pain in the acute stage, and in patients with lesions in the ophthalmic branch of the trigeminal nerve. Pain from PHN is often debilitating and difficult to treat. A wide variety of therapeutic approaches have been advocated over the years, but most are not very effective. Early aggressive treatment of …

Epidemiology and management of postherpetic neuralgia.

Herpes zoster occurs rarely in immunocompetent children and infrequently in immunocompetent young adults. However, its incidence increases with age, particularly after age 50. Reactivation of varicella-zoster virus (VZV) is characterized by a rash and is generally accompanied by considerable pain, dysesthesias, and skin hypersensitivity. Chronic pain that is sometimes experienced after the rash has healed is referred to as postherpetic neuralgia (PHN), the most common complication of herpes zoster. Postherpetic neuralgia is often severe and, unfortunately, refractory to most forms of treatment…

Diagnosis and treatment of cranial neuralgias.

Both medical and surgical therapies may be used in the treatment of trigeminal neuralgia and glossopharyngeal neuralgia. Ordinarily, the treatment is medical; however, if a response to drugs is not forthcoming, or if the patient becomes toxic while taking medications or refuses to abide by an appropriate medical program, then surgical consultation should be obtained and the appropriate operation performed. The form and type of neurosurgical procedure will probably depend to a considerable extent on the expertise of the neurosurgeon and his training. Generally, in the elderly, the simplest proc…

[Etiology and therapy of trigeminal neuralgia].

Trigeminal neuralgia is an exactly defined syndrome with a non-uniform aetiology but an obviously uniform pathogenesis. A subdivision in an idiopathic and a symptomatic type is non-essential. In a part of the cases microvascular compression of the sensory root may be the cause of trigeminal neuralgia but there are some good arguments against this concept. Other causes such as multiple sclerosis, acoustic neuroma or carotid aneurysm are well known. The principle of neurosurgical procedures is either an interruption of the pain-conducting fibres or a non-specific manipulation at the Gasserian ga…

[Use of carbamazepine in trigeminal neuralgia].

The treatment efficiency was compared to blood carbamazepine++ levels in patients with trigeminal neuralgia which were treated with the drug over a long period. The parameters displayed a good (r = 0.70) correlation that resulted in a tendency to increase treatment effect with carbamazepine++ plasma concentration increase. The treatment proved most efficient and side effect-free in a concentration range of 5 to 10 micrograms/ml. The data on blood carbamazepine++ concentrations may be helpful in designing a controlled course of treatment.

[Reflexotherapy in neuralgias of the trigeminal nerve].

Acupuncture was used in 82 patients with trigeminal neuralgila. In 17 patients trigeminal neuralgia was predominantly of the central and in 65 of the peripheral genesis. Selection of points for acupuncture was based on the findings about electrical conductivity of symmetrical acupuncture points on the patient's face. The study has shown that the method of acupuncture in patients with trigeminal neuralgia should be used differentially. It is effective only in patients with peripheral trigeminal neuralgia.

Tizanidine in the management of trigeminal neuralgia.

In a double-blind study the efficacy and tolerability of tizanidine was compared with those of carbamazepine in the management of trigeminal neuralgia. Six patients were allocated to treatment with tizanidine and six to carbamazepine. After individual titration the maximum daily doses were 18 mg and 900 mg, respectively. Among the efficacy factors used, the visual analog scale (VAS) and the overall efficacy as assessed by patients and investigator turned out to be the most appropriate. The results indicate that tizanidine was well tolerated, but the effects, if any, were inferior to those of c…
