Molecular biomarkers for neuromuscular disorders - challenges and future perspectives.

There is an ever-growing need for molecular biomarkers in assessing clinical course and diagnosing neuromuscular disorders, as well as in monitoring drug therapy. With the development of high throughput techniques, there has been an acceleration in the discovery of potential biomarkers. It is quite easy to find potential candidates, but difficult to validate them and translate into a clinical setting. Neuromuscular diseases (NMD) are a major challenge in terms of finding potential molecular biomarkers, mainly because of their heterogeneous aetiology and variability in phenotype, their as yet i

Electrophysiological Recordings of Evoked End-Plate Potential on Murine Neuro-muscular Synapse Preparations.

Neuromuscular junction (NMJ) is the specialized chemical synapse that mediates the transmission of the electrical impulse running along motor neuron axons to skeletal muscle fibers. NMJ is the best characterized chemical synapse and its study along many years of research has provided most of the general knowledge of synapse development, structure and functionality. Electrophysiology is the most accurate experimental procedure to study NMJ physiology and it largely contributed to the elucidation of synaptic transmission basic principles. Many electrophysiological techniques have been developed

Clinical perspectives: Treating spinal muscular atrophy.

Spinal muscular atrophy is a rare and progressive neuromuscular disease that, without treatment, leads to progressive weakness and often death. A plethora of studies have led to the approval of three high-cost and effective treatments since 2016. These treatments, nusinersen, onasemnogene abeparvovec, and risdiplam, have not been directly compared and have varying challenges in administration. In this review, we discuss the evidence supporting the use of these medications, the process of treatment selection, monitoring after treatment, the limited data comparing treatments, as well as future d

[Etiology, epidemiology, prognosis and biomechanical principles of neuromuscular scoliosis].

Deformity due to neuromuscular disease is often progressive and associated with reduced vital capacity. In general, all treatment should be performed in specialized centers, since invasive measures are associated with an increased morbidity compared to adolescent scoliosis. Derived from the etiology and the resulting biomechanical basis (characteristics), important aspects and considerations arise for all healthcare institutions from the examination interval to the duration of conservative therapy and initiation of surgical therapy. Proper monitoring and assessment are key components to identi

Divergent Responses to Training Load in Professional Australian Football: Are Insights Obtained from Generic and Running-based Assessments Complementary or Redundant?

BACKGROUND: Generic neuromuscular assessments are widely used to monitor training responses in team sports. Emerging running-based measures have been proposed to offer greater sensitivity to residual neuromuscular training effects by aligning more closely with the exercise dose, yet their practical utility remains ambiguous. Concurrent evaluation is thus necessary to identify athlete monitoring assessments that best capture meaningful responses. OBJECTIVES: We examined within-athlete relationships among common training response measures and their associations with prior weekly external traini

Dexamethasone does not diminish sugammadex reversal of neuromuscular block - clinical study in surgical patients undergoing general anesthesia.

BACKGROUND: Sugammadex reverses neuromuscular block (NMB) through binding aminosteroid neuromuscular blocking agents. Although sugammadex appears to be highly selective, it can interact with other drugs, like corticosteroids. A prospective single-blinded randomized clinical trial was designed to explore the significance of interactions between dexamethasone and sugammadex. METHODS: Sixty-five patients who were anesthetized for elective abdominal or urological surgery were included. NMB was assessed using train-of-four stimulation (TOF), with rocuronium used to maintain the desired NMB depth.

Effects of deep neuromuscular block with low-pressure pneumoperitoneum on respiratory mechanics and biotrauma in a steep Trendelenburg position.

We hypothesized that deep neuromuscular blockade (NMB) with low-pressure pneumoperitoneum (PP) would improve respiratory mechanics and reduce biotrauma compared to moderate NMB with high-pressure PP in a steep Trendelenburg position. Seventy-four women undergoing robotic gynecologic surgery were randomly assigned to two equal groups. Moderate NMB group was maintained with a train of four count of 1-2 and PP at 12 mmHg. Deep NMB group was maintained with a post-tetanic count of 1-2 and PP at 8 mmHg. Inflammatory cytokines were measured at baseline, at the end of PP, and 24 h after surgery. Inte

Immunoglobulins from amyotrophic lateral sclerosis patients enhance spontaneous transmitter release from motor-nerve terminals.

Amyotrophic lateral sclerosis (ALS) is an incapacitating neuromuscular disease of unknown etiology. Although laboratory evidence is lacking, circumstantial evidence supports the importance of immune factors in the pathogenesis of ALS. In the present study immunoglobulins from 4 of 8 ALS patients induced a significant increase in spontaneous quantal transmitter release as monitored by miniature end-plate potential (MEPP) frequency in mouse phrenic nerve-diaphragm preparations at 23 degrees C, whereas immunoglobulins from normal individuals and from patients with other neuromuscular diseases had

Andersen-Tawil syndrome: deep phenotyping reveals significant cardiac and neuromuscular morbidity.

Andersen-Tawil syndrome is a neurological channelopathy caused by mutations in the KCNJ2 gene that encodes the ubiquitously expressed Kir2.1 potassium channel. The syndrome is characterized by episodic weakness, cardiac arrythmias and dysmorphic features. However, the full extent of the multisystem phenotype is not well described. In-depth, multisystem phenotyping is required to inform diagnosis and guide management. We report our findings following deep multimodal phenotyping across all systems in a large case series of 69 total patients, with comprehensive data for 52. As a national referral

Beware of "curare cleft" like changes during unilateral capnothorax.

Capnography is a standard monitoring tool during general anaesthesia. Diaphragmatic movement with the weaning of muscle relaxant effect produces the characteristic "curare cleft" on capnography. Various artefacts can mimick this trace intraoperatively. Cautious interpretation and identification of these is essential to avoid any undue overdosing of the patients with muscle relaxants. We report "curare cleft" like artefact during ventilation with a single lumen tube in a patient with unilateral capnothorax undergoing minimally invasive esophagectomy.

The effect of match-play on acute post-match neuromuscular fatigue following Australian Football League Women's (AFLW) competition.

The Australian Football League Women's (AFLW) is the premier national women's competition in Australian Rules football. The aim of this exploratory study was to investigate the neuromuscular fatigue response to match-play and the external load correlates of this response in AFLW. Players (n = 22) wore a 10 Hz GNSS device and completed immediately pre- and post-match countermovement jumps (CMJ) on dual force plates for each match in the 2022 AFLW competitive season. Concentric, eccentric, and composite CMJ variables were selected a priori based on previously established validity, reliability, a

What percentage of neuromuscular strength reduction in older adults determines the decline in walking speed?

BACKGROUND/OBJECTIVE: Although different neuromuscular strength (NS) cutoff points have been proposed to identify low walking speed (WS) in older adults, the extent to which NS changes predict long-term WS decline remains unclear. This study aimed to determine the percentage of NS change associated with WS decline in older adults. METHODS: A longitudinal analysis was conducted using data from 4,541 participants aged ≥ 60 years in the ELSA Study. Changes in NS over four, eight, and twelve years of follow-up, compared to baseline, were categorised into nine groups: maintenance of NS within ± 5%

General anaesthetic and airway management practice for obstetric surgery in England: a prospective, multicentre observational study.

There are no current descriptions of general anaesthesia characteristics for obstetric surgery, despite recent changes to patient baseline characteristics and airway management guidelines. This analysis of data from the direct reporting of awareness in maternity patients' (DREAMY) study of accidental awareness during obstetric anaesthesia aimed to describe practice for obstetric general anaesthesia in England and compare with earlier surveys and best-practice recommendations. Consenting patients who received general anaesthesia for obstetric surgery in 72 hospitals from May 2017 to August 2018

Toward Scalable Electromyography in Oncology: A Narrative Review of Normalization Challenges and Machine Learning Innovations.

OBJECTIVES: Electromyography (EMG) is increasingly applied in oncology to monitor neuromuscular impairment, treatment toxicities, and rehabilitation outcomes. However, reliance on maximal voluntary contraction (MVC) normalization limits scalability, as many patients cannot perform safe and reliable MVCs due to fatigue, pain, or treatment-induced impairments. This narrative review evaluates the feasibility and clinical utility of machine learning (ML)-predicted MVCs as an alternative normalization method in oncology care. METHODS: Peer-reviewed articles published between 2015 and 2025 were ret

A narrative review on the potential benefits and limitations of deep neuromuscular blockade.

BACKGROUND: Neuromuscular blockade was shown to improve surgical conditions. However, the risk of residual neuromuscular blockade upon extubation prevents anaesthesiologists from maintaining complete paralysis. For this reason, deep NMB is still underused in anaesthesia. This review focused on answering six questions revolving around the use of deep NMB versus moderate NMB. METHODS: This was a non-exhaustive narrative review based on 6 selected relevant questions: does deep NMB 1) improve surgical conditions? 2) reduce surgical complications? 3) facilitate a reduction in intraoperative pneumo
