Peri- and intraoperative EEG signatures in newborns and infants.

OBJECTIVE: The electroencephalographic derived indices have been developed for adult patients, however these monitors have not been validated for infants. METHODS: Frontal EEGs were recorded in 115 infants aged <1 year [0-3-months (N = 27), 4-6-months (N = 30), 7-9-months (N = 29) and 10-12-months (N = 29)] who received general anaesthesia with sevoflurane. Total power (µV2) and relative β-, α-, θ-, δ-power (%) were analyzed. Additionally, in 20 EEGs event marker were added (baseline, loss of consciousness, intraoperative situation, extubation) to assess perioperative EEG dynamics. RESULTS:

The Strength of Alpha Oscillations in the Electroencephalogram Differently Affects Algorithms Used for Anesthesia Monitoring.

BACKGROUND: Intraoperative patient monitoring using the electroencephalogram (EEG) can help to adequately adjust the anesthetic level. Therefore, the processed EEG (pEEG) provides the anesthesiologist with the estimated anesthesia level. The commonly used approaches track the changes from a fast- and a low-amplitude EEG during wakefulness to a slow- and a high-amplitude EEG under general anesthesia. However, besides these changes, another EEG feature, a strong oscillatory activity in the alpha band (8-12 Hz), develops in the frontal EEG. Strong alpha-band activity during general anesthesia see

Zina percutaneous screw fixation combined with endoscopic lumbar intervertebral fusion under intraoperative neuromonitoring: A case report.

INTRODUCTION: Traditional open discectomy and intervertebral fusion surgery is the common strategy for lumbar disc herniation (LDH). However, it has the disadvantages of long recovery time and severe paravertebral soft tissue injury. Zina percutaneous screw fixation combined with endoscopic lumbar intervertebral fusion (ZELIF), as a novel minimally invasive surgical technique for LDH, has the advantages in quicker recovery, less soft tissue destruction, shorter hospital stays and less pain. We report a novel technique of ZELIF under intraoperative neuromonitoring (INM) for the treatment of LDH

Resection of Myxopapillary Ependymoma of the Filum Terminale: 2-Dimensional Operative Video.

We present the case of a 48-yr-old female who presented with persistent thigh pain and was found to have a heterogeneous mass caudal to the conus most consistent with a myxopapillary ependymoma. We performed L2-3 laminectomies for tumor resection. For this procedure, we used intraoperative ultrasound as well as neuromonitoring. This video illustrates the gross pathology of a myxopapillary ependymoma, effective circumferential blunt and sharp dissection of the cauda equina from the tumor, and identification, preparation, and sectioning of the filum terminale. This case also underlines the chall

Metabolic brain measurements in the newborn: Advances in optical technologies.

Neonatal monitoring in neonatal intensive care is pushing the technological boundaries of newborn brain monitoring in order to improve patient outcome. There is an urgent need of a cot side, real time monitoring for assessment of brain injury severity and neurodevelopmental outcome, in particular for term newborn infants with hypoxic-ischemic brain injury. This topical review discusses why brain tissue metabolic monitoring is important in this group of infants and introduces the currently used neuromonitoring techniques for metabolic monitoring in the neonatal intensive care unit (NICU). Adverse Events Related to Transcranial Electric Stimulation for Motor-evoked Potential Monitoring in High-risk Spinal Surgery.

STUDY DESIGN: Prospective multicenter study. OBJECTIVE: The aim of this study was to study the incidence of nonneurologic adverse events related to transcranial electric stimulation (TES) for intraoperative spinal cord monitoring (IOM) with motor-evoked potentials (MEPs) (Tc(E)- MEPs) and determine the need for safety precautions. SUMMARY OF BACKGROUND DATA: Tc(E)-MEPs monitoring requires high-voltage multipulse TES that causes widespread muscle contraction and movement. Improved awareness of TES-induced movement-related adverse events is needed. METHODS: We analyzed data from 2643 patients

The Effect of Tumor Neovasculature on Functional Magnetic Resonance Imaging Blood Oxygen Level-Dependent Activation.

BACKGROUND: We report the case of a 40-year-old patient with a large, World Health Organization grade III oligodendroglioma in the left parietal lobe. CASE DESCRIPTION: Presurgical planning included functional magnetic resonance imaging (fMRI) localization of language, motor, and somatosensory processing. fMRI results for motor and somatosensory tasks revealed activation in perilesional regions near the surgical resection as well as deactivation in the tumor for the sensory task, suggesting decreased autoregulation in the region owing to the glioma. fMRI results showed left-hemisphere dominan

An Alternative Transcranial Motor Evoked Potential Montage to Minimize Ipsilateral "Crossover" Motor Responses.

Transcranial electrical motor evoked potential (TcMEP) is a modality utilized in intraoperative neurophysiological monitoring to assess the integrity of the corticospinal tract. Traditionally, TcMEPs are obtained by anodal stimulation of the scalp over the motor cortex of the selected hemisphere and referenced to the contralateral hemisphere. Subsequent compound motor action potential responses (CMAPs) are recorded at various muscles. The muscle responses of interest are usually those recorded in the limb contralateral to the hemisphere of stimulation. However, TcMEPs may elicit simultaneous m…

Intraoperative Test Occlusion as Adjustment of Extracranial-to-Intracranial Bypass Strategy for Unclippable Giant Aneurysm of the Internal Carotid Artery.

BACKGROUND: There is still a controversy for low-flow extracranial-intracranial or high-flow extracranial-intracranial bypass with proximal … artery was patent. Intraoperative proximal test occlusion at cervical internal carotid artery under neurophysiological monitoring, instead of preoperative balloon test occlusion, was performed to assess whether low-flow bypass was sufficient. The monitoring was unchanged during test occlusion, and the aneurysm was successfully trapped without high-flow bypass. Neither ischemic …

The pathophysiology of chronic noncommunicating hydrocephalus: lessons from continuous intracranial pressure monitoring and ventricular infusion testing.

OBJECTIVE The pathophysiology of chronic noncommunicating hydrocephalus (ncHC) is poorly understood. This present study explored whether lessons about the pathophysiology of this clinical entity might be retrieved from results of overnight monitoring of pulsatile and static intracranial pressure (ICP) and ventricular infusion testing. METHODS The study cohort included adult patients (> 20 years of age) with chronic ncHC due to aqueductal stenosis in whom symptoms had lasted a minimum of 6 months. A reference cohort consisted of age- and sex-matched patients managed for communicating HC (cHC).

Beating heart arch replacement without circulatory arrest: a standardized surgical strategy.

Deep hypothermic circulatory arrest, antegrade brain perfusion and cardioplegic arrest are commonly used organ protection techniques during aortic arch surgery. We present our surgical strategy for beating heart aortic arch surgery without circulatory arrest. Cardiopulmonary bypass access is achieved through the right and/or left axillary artery for continuous cerebral perfusion under neurophysiological monitoring and femoral cannulation through an EndoReturn cannula while using a Coda balloon catheter for visceral perfusion. Systemic cooling is applied between 28 and 32 °C.

Depth of anaesthesia monitors and the latest algorithms.

This paper reviews the existing depth of anaesthesia (DoA) monitors and their algorithms and also proposes to improve their performance from four aspects. An ideal DoA monitor should be able to suggest a personalised drug dosage, to predict and provide early warnings when dosages are inappropriate, to be portable and highly cost-effective. The limitations of the existing DoA monitors commonly include unsatisfied data filtering techniques, time delay for the monitoring responses, and inflexible and low noise immunity problems. The latest research results show that their performance can be impro
