Endoscopic endonasal approach to pituitary adenomas: Impact on adenohypophyseal function. Study of 231 cases.

PURPOSE: To identify presurgical and surgical factors associated with the development of hypopituitarism and its recovery after endoscopic endonasal transsphenoidal (EET) resection of pituitary adenomas (PAs). METHODS: Retrospective study of patients with PAs operated by the same neurosurgeon through an EET approach in two Spanish tertiary hospitals in ten years. RESULTS: 242 pituitary surgeries performed in 231 patients were analyzed. In the 154 surgeries performed in 146 patients with non-functioning PAs (NFPAs), 46.8% (n=72) presented presurgical hypopituitarism. After PAs resection, 41 o

Using Magnetic Resonance Myelography to Improve Interobserver Agreement in the Evaluation of Lumbar Spinal Canal Stenosis and Root Compression.

STUDY DESIGN: Cross-sectional retrospective study designed to assess interobserver agreement. PURPOSE: To investigate if interobserver agreement using magnetic resonance imaging (MRI) in the evaluation of lumbar spinal canal stenosis and root compression can be improved upon combination with magnetic resonance myelography (MRM). OVERVIEW OF LITERATURE: The interpretation of lumbar spinal MRI, which is the imaging modality of choice, often has a significant influence on the diagnosis and treatment of low back pain. However, using MRI alone, substantial interobserver variability has been repor

Adult Hydrocephalus Clinical Subtypes.

Adult hydrocephalus comprises 4 pragmatic categories: (1) transition; (2) unrecognized congenital; (3) acquired; and (4) suspected idiopathic normal pressure hydrocephalus. Each of these groups has unique clinical presentations and care needs that require the involvement of adult neurosurgeons for diagnosis and treatment. Patients in all 4 of these categories benefit from longitudinal care for monitoring of their symptoms or assessing response to treatment.

Rapidly Growing and Asymptomatic Skull Base Lesion.

Trochlear schwannomas typically present with diplopia and are extremely rare benign intracranial neoplasms that require the combined expertise of neurotologists and neurosurgeons for surgical excision. A combined petrosal approach offers a surgical route for removal of growing and/or symptomatic tumors. A study of a trochlear nerve schwannoma that demonstrated rapid growth but was otherwise asymptomatic was presented in this study.

Occult spinal dysraphism: a neurosurgical problem with a dermatologic hallmark.

Lumbosacral congenital cutaneous lesions have been recognized for a long time by neurologists and neurosurgeons as markers of occult spinal defects, but only a few cases have been published and discussed in the dermatologic literature. Based on a case report, we emphasize the importance of early diagnosis of these lesions, the use of new diagnostic techniques, and the correct management.

Excision of vestibular schwannomas--is there a learning curve and how best to demonstrate it?

OBJECTIVE: To demonstrate the existence of a learning curve in a consultant neurosurgeon's performance in excising vestibular schwannomas by plotting the cumulative sum (CUSUM) chart. Also, to evaluate the influence of new technology in the form of the facial nerve monitor (FNM) and the KTP-532 Laser on surgical performance. METHOD: Analysis was carried out on a prospectively collected data series of 102 consecutive cases of vestibular schwannomas excisions between 1986 and 2000 by a single neurosurgeon using the retrosigmoid approach. The CUSUM score chart of the surgical outcomes across the

How to Choose a Shunt for Patients with Normal Pressure Hydrocephalus: A Short Guide to Selecting the Best Shunt Assembly.

Most patients with hydrocephalus are still managed with the implantation of a cerebrospinal fluid (CSF) shunt in which the CSF flow is regulated by a differential-pressure valve (DPV). Our aim in this review is to discuss some basic concepts in fluid mechanics that are frequently ignored but that should be understood by neurosurgeons to enable them to choose the most adequate shunt for each patient. We will present data, some of which is not provided by manufacturers, which may help neurosurgeons in selecting the most appropriate shunt. To do so, we focused on the management of patients with i…

Harvey Cushing's surgical treatment of a pediatric patient with an intraventricular glioma.

BACKGROUND/AIMS: The combination of inadequate technology and incomplete nomenclature systems created challenges for early neurosurgeons, and contributed to the dismal prognosis for brain tumors, particularly within the pediatric population. METHODS: Following IRB approval, and by the courtesy of the Alan Mason Chesney Archives, we reviewed the Johns Hopkins Hospital surgical files from 1896 to 1912. A single case of a pediatric patient with an intraventricular glioma was selected for further review. RESULTS: Here we report the case of a 10-year-old girl who presented to the Johns Hopkins Ho…

The evaluation and diagnosis of the dizzy patient.

In conclusion, the assessment of the dizzy patient is indeed complex and far-reaching. The expertise of family practitioners, internist, neurologists, otolaryngologists, and neurosurgeons at times is essential and one should not feel inhibited about referring the patient for further evaluation to our colleagues so that the patient may best be served and resolution of the patient's complaints may result.

Contralateral approach for middle cerebral artery aneurysms with long M1 segment: report of 2 cases.

INTRODUCTION: Contralateral clipping of middle cerebral artery (MCA) aneurysms is not practiced widely and has been reported only in case series. Some of the neurosurgeons proposing this approach have even postulated that a short M1 segment is a basic requirement for performing it. CASES: We present our experience using a contralateral approach with 2 patients who had 3 MCA aneurysms located more than 2.5cm from the carotid bifurcation. All 3 aneurysms were completely occluded, as demonstrated on postoperative CT angiography, and the patients' neurological evolution was uneventful. DISCUSSIO…
