Nurse practitioner scope of practice and the prevention of foot complications in rural diabetes patients.

PURPOSE: Diabetic foot ulcers afflict a quarter of type-2 diabetes mellitus patients and are associated with higher mortality rates among people with diabetes. Routine primary and preventive care is essential to both prevent and treat foot ulcers before they can contribute to further adverse outcomes. One approach for expanding this care to people with diabetes in rural communities is increasing the practice authority of nurse practitioners. This study examines whether the presence of nurse practitioner practice authority is associated with fewer foot ulcer complications in rural populations a

Community Perspectives on Japanese Encephalitis Risk and Prevention in an Endemic Region of Australia.

OBJECTIVE: Japanese encephalitis (JE) is a mosquito-borne disease caused by JE virus (JEV) infection, detected for the first time in south-eastern Australia in 2022. In New South Wales (NSW), detections of JEV in mosquitoes and animal hosts, human JE cases, and climate and environmental considerations have informed which areas are considered high risk for JEV and which populations are eligible for vaccination (funded by Australian states and territories). However, early evidence indicates slower-than-expected uptake in these high-risk areas. We aimed to explore how community members and health

Ethical issues in the response to Ebola virus disease in US emergency departments: a position paper of the American College of Emergency Physicians, the Emergency Nurses Association and the Society for Academic Emergency Medicine.

The 2014 outbreak of Ebola Virus Disease (EVD) in West Africa has presented a significant public health crisis to the international health community and challenged US emergency departments to prepare for patients with a disease of exceeding rarity in developed nations. With the presentation of patients with Ebola to US acute care facilities, ethical questions have been raised in both the press and medical literature as to how US emergency departments, emergency physicians, emergency nurses and other stakeholders in the healthcare system should approach the current epidemic and its potential fo

Enhancing the Therapeutic Relationship Between Mental Health Case Managers and Individuals With Serious Mental Health Conditions: A Participatory Action Research Study in Community-Based Services.

This study examined how community mental health case managers, most of whom were mental health nurses, modified their day-to-day practice to strengthen the therapeutic relationship within community services. Using a participatory action research design across 10 centres in Catalonia, Spain, 24 case managers and 105 patients engaged in two action-reflection cycles. Data from reflective diaries and focus groups were analyzed using inductive thematic analysis. Through cyclical observation and collaborative reflection, participants identified key facilitators and barriers to developing the therape

An exploration of current leg ulcer management practices in an Irish community setting.

OBJECTIVE: To establish the prevalence of leg ulceration in the Irish Health Service Executive (HSE) mid-western region and to determine the level of assessment and treatment patients have been receiving in the local community. METHOD: Before the introduction of a Doppler training programme, all public health and community health nurses working in the Irish HSE mid-western area were requested to complete an audit form on each patient being treated for leg ulceration during a predefined one-week period. This form recorded details on patient demographics, ulcer aetiology, assessment and treatme

Preconception care: practice and beliefs of primary care workers.

BACKGROUND: A number of lifestyle modifications and medical interventions can be of benefit to maternal and neonatal health, when applied prior to conception. These include smoking cessation, supplementation with folic acid, cessation or moderation of alcohol intake and improvement of diabetic control. However, preconception care (PCC) is not widely practised in the UK, despite being apparently acceptable to health professionals and to women of childbearing age. OBJECTIVES: The aims of the study were to describe the current practice of PCC in Barnsley and to assess the beliefs and attitudes o

Understanding consumer preferences for care of adolescents with pelvic inflammatory disease.

OBJECTIVE: The objective of this study is to estimate consumers' maximum willingness-to-pay (WTP) for follow-up PID services by physicians and community health nurses (CHNs), differences by consumer type (adolescents versus parents), and the differences in health-provider predicted WTP consumer estimates and actual consumer WTP estimates. METHODS: In this IRB-approved study, a contingent valuation method was used to collect WTP data regarding co-payments to physicians or nurses for clinical service delivery from the consumers of adolescent PID services (parents and adolescents) and health pro

Trauma-Informed Care in Pediatric Oncology Nursing.

BACKGROUND: Childhood cancer is highly stressful and potentially traumatic for families. While trauma-informed care (TIC) aims to address these impacts, little is known about how pediatric oncology nurses understand and use TIC in practice. METHOD: This study was conducted using a qualitative descriptive design to explore pediatric oncology nurses' perceptions of trauma and trauma-informed care. Fifteen nurses with at least 1 year of experience working in a pediatric oncology unit in a city hospital in Türkiye were recruited through purposive sampling. Data were collected between June and Jul

Real world challenges in delivering person-centred care: A community-based case study.

Community nurses face many challenges when trying to practice evidence-based, person-centred care. Ongoing concerns regarding the impact of the 2013 Francis Report ( Ford and Lintern, 2017 ) suggest that individualised and holistic care is an impossible dream, one made harder when the client appears uncooperative. This paper presents a case study that sets out how some of these challenges were met in a potentially difficult situation experienced by a student nurse and her mentor in practice, in which the student was supported to further examine and explore issues that may have influenced the s

Urinary catheters: selection, management and prevention of infection.

Urinary catheterization is a common procedure for both hospital and community patients. Nurses make many of the decisions in regard to both catheter selection and subsequent catheter care. These decisions, for example, in the type of catheterization (intermittent, indwelling, urethral or suprapubic) should be made on an informed basis. Choosing the optimum catheter material and size can benefit catheter care. Encrustation can be a problem for some catheterized patients and nurses are often involved in trying to prevent or treat it. This and other aspects of catheter management including infect

Nursing facility and home and community based service need criteria in the United States.

Over the past two decades states have expanded the delivery of long term care from institutions to the home and community. Although state Medicaid need criteria for the home and community based services (HCBS) waiver program must be equivalent to those for nursing facility (NF) criteria, other screening procedures for these services are not specified. This study examined variations in need criteria, methods for determining need, and assessment forms for NF and HCBS across the states, and identified whether these procedures were more stringent for HCBS than for NFs. The NF minimum need criteria

Nurse-physician communication and quality of drug use in Swedish nursing homes.

The objective was to explore the impact of the quality of nurse-physician communication on the quality of psychotropic drug use in Swedish nursing homes, while controlling for resident mix and other nursing home characteristics. Data were collected from a sample of 36 Swedish nursing homes providing care for 1645 residents. Drug use data, along with residents' demographic characteristics, were obtained from residents' medication administration lists. Ward nurses reported other residents' characteristics (e.g. diagnosis and frequency of behavioral problems), and facility characteristics were ob

Community leaders' perceptions of single, low-income mothers' needs and concerns for social support.

Purposes of this study were to (a) assess and describe needs and concerns of single, low-income mothers during their transition to parenthood from the perspective of 16 Midwestern community leaders working closely with families and (b) evaluate social support mechanisms that are available for families.
