The role of the nurse in the care of patients with breast cancer is changing because of the many options available for medical management of the disease. Breast cancer is a treatable disease. Patients survive many years. Throughout the treatment process, patients and families may be faced with numerous decisions involving initial treatment and adjuvant therapy as well as the treatment of metastatic disease. Involvement of the patient and caregiver are crucial in this process, and they must be assisted and supported by nurses who are knowledgeable about the options and the rationale behind each

BACKGROUND: Demand for primary care services has increased in developed countries due to population ageing, rising patient expectations, and reforms that shift care from hospitals to the community. At the same time, the supply of physicians is constrained and there is increasing pressure to contain costs. Shifting care from physicians to nurses is one possible response to these challenges. The expectation is that nurse-doctor substitution will reduce cost and physician workload while maintaining quality of care.
OBJECTIVES: Our aim was to evaluate the impact of doctor-nurse substitution in pr

OBJECTIVE: This study aimed to develop and implement an evidence-based nursing intervention program for vascular catheter-related infection prevention and control within a specialized nursing alliance network, guided by the Ottawa Model of Research Use.
METHODS: A quasi-experimental implementation study with historical controls was conducted from January 2023 to December 2024 in 12 departments of a specialized nursing alliance. Consecutive eligible adults with central venous catheters or PICCs were enrolled (control, n = 160; intervention, n = 164). Outcomes were analyzed using crude incidenc

BACKGROUND: Automated dispensing cabinets (ADCs) are used to store and dispense medications at the point of care. Medications accessed from an ADC before pharmacist order verification are removed using override functionality. Bypassing pharmacist verification can lead to medication errors; therefore, The Joint Commission considers overrides acceptable only in limited scenarios. During an 18-month period, the override rate in our perianesthesia care unit (PACU) was 17%, with oral midazolam accounting for roughly 40% of overrides. A multidisciplinary quality improvement (QI) project was initiate

Pompe disease or glycogen storage disease type II is a rare autosomal recessive disorder caused by a deficiency of the lysosomal enzyme a-glucosidase. Although enzyme replacement therapy (ERT) with 2 weekly intervals following was considered an effective treatment for Pompe disease in 2006, few patients can afford to receive treatment in China because of the high cost. This study aimed to examine the standard management of enzyme replacement therapy for late-onset Pompe disease among patients over the age of 14 years from a nursing perspective in order to assess operating procedures. ERT injec

Rural school nursing is inherently different from suburban or urban school nursing. This article describes the processes involved in becoming a Special Interest Group and identifying the specific needs of rural school nurses. We developed an online group focused on rural school nursing, provided a network of school nurses who experience similar contexts for mutual support and learning, and identified priorities for scholarship in the setting of rural and frontier school nursing. Rural school nursing was defined; additionally, characteristics of rural school nurse practice and unique challenges

BACKGROUND: Intensive care units are technically and emotionally complex environments where nurses frequently witness the end of life. In this process, nurses’ perceptions of good death and their attitudes and behaviors toward end-of-life care directly affect the quality of care provided. Understanding these relationships is important for improving the quality of holistic end-of-life care. METHODS: This descriptive and correlational study was conducted to examine the relationships between intensive care nurses’ perceptions of good death and their attitudes and behaviors toward end-of-life care

BACKGROUND: An external vendor providing off-the-shelf spinal orthoses to inpatients created significant costs and barriers to quality care for spinal surgery patients. A nursing leadership team initiated a quality improvement project to reduce the cost of providing off-the-shelf spinal orthoses and improve the care provided to spinal patients.
OBJECTIVE: To develop and evaluate a nursing-led process for providing off-the-shelf orthoses to spinal surgery patients and eliminate high costs.
DESIGN: Quality improvement project evaluated as a retrospective interrupted time-series.

BACKGROUND: Individuals experiencing pain while incarcerated depend on nurses, security structures, processes, and regulations for relief.
PURPOSE: The intent of this research was to understand men's experience of pain during incarceration to inform correctional nursing practice.
METHOD: Interpretive description, co-positioned with relational ethics, guided this study. Twelve incarcerated men participated in a single 1-hour interview that was audio-recorded and transcribed. The resultant text was analyzed for themes.

CONTEXT: Parents of children who die in the pediatric intensive care unit (PICU) carry memories of their child's suffering throughout a lifelong grieving experience. Given their prolonged time at the bedside, PICU nurses are poised to attend to dying children's suffering.
OBJECTIVES: We aimed to explore how PICU nurses identify, assess, and attend to EOL suffering.
METHODS: Interpretive descriptive qualitative study with thematic analysis of virtual focus groups from a geographically diverse sample of PICU nurses.
RESULTS: Nurses participated in five focus groups (N = 19). Most identified a

BACKGROUND: Immunosenescence is a complex process characterized by an age-related remodelling of immune system. The prominent effects of the immunosenescence process is the thymic involution and, consequently, the decreased numbers and functions of T cells. Since thymic involution results in a collapse of the T-cell receptor (TCR) repertoire, a reliable biomarker of its activity is represented by the quantification of signal joint T-cell receptor rearrangement excision circles (sjTRECs) levels. Although it is reasonable to think that thymic function could play a crucial role on elderly surviva

BACKGROUND: Intensive care unit (ICU) nurses working in South Korea report experiencing uncertainty about how to care for patients undergoing withdrawal of life-sustaining treatments (WLT). A lack of consensus on care guidelines for patients with WLT contributes to uncertainty, ambiguity, and confusion on how to act appropriately within current law and social and ethical norms. To date, little has been discussed or described about how ICU nurses construct meaning about their roles in caring for dying patients in the context of wider social issues about end-of-life care and how this meaning int

COVID-19 vaccination and regular testing of nursing home staff have been critical interventions for mitigating COVID-19 outbreaks in US nursing homes. Although implementation of testing has largely been left to nursing home organizations to coordinate, vaccination occurred through a combination of state, federal, and organization efforts. Little research has focused on structural variation in these processes. We examined whether one structural factor, the primary shift worked by staff, was associated with differences in COVID-19 testing rates and odds of vaccination, using staff-level data fro

BACKGROUND: Psychiatric nurses play a crucial role in patient recovery processes, and their attitudes during these processes can directly influence the recovery of individuals with mental illness. However, the concept of recovery in psychiatric disorders is perceived differently among nurses, which, in turn, can affect the patient's recovery trajectory. This study aims to explore and investigate the perspectives, thoughts and experiences of nurses working in psychiatric clinics about recovery in mental illnesses.
METHODS: The study was conducted between September and December 2021 at a Mental

This study used Hypnofertility-based interventions to determine whether these interventions would lead to increased fertility preparedness and pregnancy outcomes and decreased cortisol levels in women undergoing In Vitro Fertilization (IVF) treatment. This randomized, controlled, prospective study was conducted from November 2017 through March 2019 in 61 Turkish women with unexplained infertility (intervention group: 30, control group: 31). Hypnofertility-based nursing care included affirmations, visualization, imagination, and relaxation from the first day of treatment until the day of the pr
