Neurogenic Blepharoptosis After Medial Orbital Wall Reconstruction.

Neurogenic blepharoptosis after orbital wall reconstruction is a rare complication. It can either present as an isolated blepharoptosis, or accompany with other ocular symptoms. The authors introduce 4 cases of post-operative neurogenic blepharoptosis after reconstruction of medial orbital wall via transcaruncular approach.Immediately after the surgery, 2 adolescent patients experienced unilateral islolated complete blepharoptosis with no other ocular symptoms. The other 2 adult patients presented unilateral complete blepharoptosis accompanied by lateral displacement of the globe and evident e

Complete horizontal gaze palsy due to bilateral paramedian pontine reticular formation involvement as a presentation of multiple sclerosis: a case report.

BACKGROUND: Demyelinating central nervous system diseases include several disorders that multiple sclerosis (MS) is identified as the most common among them. Ocular movement disturbances are a typical presentation in MS patients where lesions affect the complex and interconnected pathways involved in eye motion. Centers for gaze control are located in the pons primarily; therefore, lesions involving these centers can be presented with abnormalities in gaze. However, bilateral lesions in pontine gaze centers are exceptionally rare. CASE PRESENTATION: A 16-year-old girl with bilateral horizonta

Ocular flutter as presenting manifestation of pediatric MOG antibody-associated demyelination: A case report.

A 13-year-old girl presented with a 5-day history of oscillopsia. On examination, ocular flutter and mild cerebellar signs were found. Brain magnetic resonance imaging (MRI) revealed four periventricular and subcortical non-enhancing lesions. Cerebrospinal fluid (CSF) oligoclonal bands were negative. Neuroblastoma or other malignancies were not found. She responded well to a corticosteroid-intravenous immunoglobulin (IVIG) combination and remained symptom-free for 3 years until presenting again with isolated ocular flutter. Brain MRI at this time remained atypical for classic multiple sclerosi

Ocular features of multiple system atrophy.

The aim of this paper is to gain better understanding of the ocular manifestations of multiple system atrophy (MSA), a neurodegenerative disorder rarely studied in terms of its ophthalmologic features. We performed a retrospective case series (1/1/05-12/31/14) to search for patients seen at Mayo Clinic, Rochester, MN, who had mention of MSA in the medical record and an eye examination, which yielded 285 cases. Of the 285, we identified 39 cases of true MSA. Each of these 39 patients was further reviewed for ocular abnormalities potentially related to MSA. Ocular findings potentially attributab

Neuroanatomical Structures in Human Extraocular Muscles and Their Potential Implication in the Development of Oculomotor Disorders.

The potential role of sensory feedback from human extraocular muscles has been subjected to considerable speculation in the ophthalmic literature. Extraocular muscles pull against a fairly even load and do not initiate a stretch reflex, even when the eyes are directed toward the boundaries of their respective field of action. These unique working conditions and physiological properties have led to the notion that the sensory signal arising from receptors in extraocular muscles differs from the conventional proprioceptive signal arising from their somatic counterparts. The interest in the recep

Marked improvement in opsoclonus and cerebellar ataxia after the surgical removal of a squamous cell carcinoma of the thymus: a case report.

A 69-year-old man with rapidly evolving vertigo and ataxia was admitted to our hospital. He was presented with a dysarthric speech and chaotic eye movements, identified as opsoclonus. Neurological examination revealed limb and truncal ataxias and an inability to stand unless fully assisted. A chest CT scan revealed a mass at the anterior mediastinum, which suggested paraneoplastic neurological syndrome (PNS). However, an extensive search for anti-neuronal antibodies linked to cerebellar ataxia failed to find any autoantibodies, including cell surface autoantibodies. Subsequently, a total surgi

[Intermittent elevation deficiency].

A 7-year-old boy presented with vertical double images. A paediatric examination and magnetic resonance imaging of the head did not show any pathological findings. The diagnosis was an acquired click syndrome of the superior oblique muscle. In cases with inflammatory and systemic origin the underlying disease should be treated. Local steroid therapy can be carried out for improvement of inflammation in the region of the superior oblique muscle with a high degree of suffering.

The rehabilitation of hemianopic dyslexia.

Hemianopic dyslexia is a frequent and disabling functional impairment following brain injury. This form of dyslexia is an acquired reading disorder whereby patients with homonymous visual field defects have persistent and severe reading difficulties, despite having intact language functions. Hemianopic dyslexia has received little attention from researchers and clinicians, and this article is the first to review the rehabilitation of patients with the condition. In light of advances in our knowledge about the nature and causes of hemianopic dyslexia, I critically examine the available treatmen

Oculomotor deficits indicate the progression of Huntington's disease.

The oculomotor deficits associated with Huntington's Disease (HD) are one of the earliest signs of disease onset. They include a marked delay in executing voluntary saccades and a difficulty inhibiting saccades to task-irrelevant stimuli. In addition, HD patients develop a deficit in task-switching, which can be demonstrated by the continued adherence to a rule after it has been recently changed. These deficits are likely to be the result of a progressive neural degeneration of the fronto-striatal system, which is a distinguishing feature of HD neuropathology. It is predicted that as the disea

Tourette syndrome and the eye.

BACKGROUND: Tourette syndrome is a stress-sensitive neurologic disorder characterized by involuntary vocal and motor tics. Both Tourette syndrome and the medical treatment for this condition can affect the eye. CASE REPORTS: Case 1: A 5-year-old boy was brought in to see the optometrist by his mother for excessive "flinching" of the eyes. The flinching was actually excessive blinking and, after consulting with the patient's pediatrician and a neurologist, Tourette syndrome was diagnosed. Case 2: An adult male patient complained that, on occasion, he would forcibly blink out his soft contact l

Isolated bilateral internuclear ophthalmoplegia after ischemic stroke.

A 63-year-old man suddenly developed an isolated bilateral internuclear ophthalmoplegia (INO). High-resolution brain MRI showed signal abnormalities consistent with acute ischemic stroke limited to the infra-aqueductal region of the midbrain bilaterally. This case offers graphic evidence that stroke can be responsible for an isolated bilateral INO.

Eye movement disorders in vitamin B12 deficiency: two new cases and a review of the literature.

Eye movement disorders are rarely reported in vitamin B12 deficiency. We describe two cases with eye movement disorder and vitamin B12 deficiency; one with bilateral internuclear ophthalmoplegia and the other with downbeat nystagmus. Both of the patients received replacement therapy but their eye movement disorders did not respond to treatment. We also review the nine previously reported cases.

Opsoclonus-myoclonus associated with celiac disease.

Celiac disease may be associated with various neurologic manifestations, most commonly cerebellar ataxia. This report describes a 2-year-old male who presented with opsoclonus-myoclonus syndrome including action myoclonus, palpebral flutter, opsoclonus, and ataxia. Given the severity of ataxia, the child was unable to sit or walk independently. Brain magnetic resonance imaging was normal on two occasions (4-week interval). Oligoclonal bands were found in the cerebrospinal fluid. Blood and serum examinations were unremarkable, with no evidence of infectious seroconversion. However autoantibody

Multiple system atrophy with macrosquare-wave jerks.

Macrosquare-wave jerks consist of high-amplitude saccadic intrusions occurring mainly during fixation that are observed in several disease entities, e.g., multiple sclerosis or parkinsonian syndromes, but very rarely in patients with multiple system atrophy. We present here a patient with multiple system atrophy exhibiting the oculomotor feature of macrosquare-wave jerks.
