Heterotopic ossification (HO) is the formation of bony tissues in the extraskeletal system. To date, no effective therapy has been developed for the treatment of HO, although increasing evidences have shown that inhibition of TGF-β signaling has potential as a new therapeutic approach for attenuating HO progression. Results from previous clinical trials have demonstrated that patients with malignant tumors exhibit excellent tolerability to Galunisertib, a TGF-β receptor I kinase inhibitor. However, its therapeutic potential in preventing HO and inhibitory effect on osteogenesis remain unclear.

Heterotopic ossification (HO) is a pathological bone formation based on endochondral ossification distinguished by ossification within muscles, tendons, or other soft tissues. There has been growing studies focusing on the treatment with rapamycin to inhibit HO, but the mechanism of mTORC1 on HO remains unclear. Tendon cells (TDs) are the first cells to form during tendon heterotopic ossification. Here, we used an in vivo model of HO and an in vitro model of chondrogenesis induction to elucidate the effect and underlying mechanism of mTORC1 in HO. The current study highlights the effect of rap

INTRODUCTION: Heterotopic ossification of tendons and ligaments is a painful and debilitating disease with no effective treatment. Although aging has been reported to be correlated with the occurrence and development of this disease, the mechanism remains unknown.
MATERIALS AND METHODS: In the present study, we generated Bmal1-/- mice, which disrupted the circadian clock and displayed premature aging, as an aging model to explore the role of Bmal1 in TGF-beta (β)/BMP signaling in progressive heterotopic ossification of tendons and ligaments with aging.
RESULTS: We first confirmed that BMAL1

Neurogenic heterotopic ossifications (NHOs) form in periarticular muscles after severe spinal cord (SCI) and traumatic brain injuries. The pathogenesis of NHO is poorly understood with no effective preventive treatment. The only curative treatment remains surgical resection of pathological NHOs. In a mouse model of SCI-induced NHO that involves a transection of the spinal cord combined with a muscle injury, a differential gene expression analysis revealed that genes involved in inflammation such as interleukin-1β (IL-1β) were overexpressed in muscles developing NHO. Using mice knocked-out for

Although heterotopic ossification (HO) has been reported to be a common complication of the posttraumatic healing process, the underlying mechanism remains unknown. Endothelial-mesenchymal transition (EndMT) is known to play a role in HO, and our recent study observed that neuroendocrine signals can promote HO by modulating EndMT. Melatonin, a neuroendocrine hormone secreted mainly by the pineal gland, has been documented to perform its function in the skeletal system. This study aimed at describing the expression of melatonin during the formation of HO in rat models of Achilles tendon injury

Traumatic heterotopic ossification (tHO) commonly develops in wounded service members who sustain high-energy and blast-related traumatic amputations. Currently, no safe and effective preventive measures have been identified for this patient population. Bone morphogenetic protein (BMP) signaling blockade has previously been shown to reduce ectopic bone formation in genetic models of HO. In this study, we demonstrate the efficacy of small-molecule inhibition with LDN193189 (ALK2/ALK3 inhibition), LDN212854 (ALK2-biased inhibition), and BMP ligand trap ALK3-Fc at inhibiting early and late osteog

BACKGROUND: Sciatic nerve entrapment due to heterotopic ossification is a rare late complication of the surgical treatment of an acetabular fracture.
CASE DESCRIPTION: In this article we present the case of a 34-year-old male, who developed weakness of the dorsiflexors of his right ankle several months after an open reduction of an acetabular fracture. The weakness was caused by isolated ectopic bone formation around the peroneal nerve in the presence of an anatomical variation, namely a high division of the sciatic nerve into the tibial and peroneal nerves. Surgical resection of the heteroto

BACKGROUND: Although infrequently reported in amputees previously, heterotopic ossification has proven to be a common and problematic clinical entity in our recent experience in the treatment of traumatic and combat-related amputations related to Operation Enduring Freedom and Operation Iraqi Freedom. The purpose of the present study was to report the prevalence of and risk factors for heterotopic ossification following trauma-related amputation as well as the preliminary results of operative excision.
METHODS: We identified 330 patients with a total of 373 traumatic and combat-related amputa

We report a prospective, randomised, blinded clinical comparison of the use of indomethacin or radiation therapy for the prevention of heterotopic ossification (HO) in 75 adults who had open reduction and internal fixation of acetabular fractures through either a Kocher-Langenbeck, a combined ilioinguinal and Kocher-Langenbeck, or an extended iliofemoral approach. Indomethacin, 25 mg, was given three times daily for six weeks. Radiation with 800 cGy was delivered within three days of operation. Plain radiographs were reviewed and given Brooker classification scores by three independent observe

PURPOSE: Heterotopic ossification (HO) is common following acetabular surgery. Evidence comparing external beam radiation (XRT) and nonsteroidal anti-inflammatory drugs (NSAIDs) for HO prophylaxis remains conflicting. This study evaluated changes in XRT utilization during the COVID-19 pandemic and compared the efficacy of XRT versus NSAIDs.
METHODS: A retrospective review was conducted of 180 patients undergoing open acetabular fracture fixation at a level 1 trauma center from 2018 to 2021. High-risk patients received XRT or indomethacin if XRT was declined. Outcomes included prophylaxis meth

Neurological heterotopic ossification can rapidly arise in the periarticular muscles following spinal cord injury. However, the specific mechanism of how the injured central nervous system regulates heterotopic bone formation in peripheral tissues remains largely unexplored. Here we first demonstrate through single-cell RNA sequencing and spatial transcriptomic analysis that fibroadipogenic progenitor cells (FAPs) contribute to heterotopic ossification in injured muscle tissues following spinal cord injury. Mechanistically, we define a neuro-immune-bone axis in which endothelial tip cells with

OBJECTIVES: Neurogenic heterotopic ossification (NHO) is extra-skeletal bone formation induced by neurologic conditions such as spinal cord injury (SCI) and traumatic brain injury (TBI). The definite treatment of established NHO is surgery and the most common complication of surgery is recurrence. This study compared three common postsurgical tertiary prevention strategies: radiotherapy, pharmacological treatment, and no additional intervention.
MATERIALS AND METHODS: Comprehensive searches on PubMed, Embase, Web of Science, and Cochrane Library were conducted until June 2023. Studies' qualit

BACKGROUND: Heterotopic ossification (HO) involves abnormal bone formation in soft tissues near joints, commonly occurring after elbow trauma or surgery, leading to pain and functional limitations. Previous studies have primarily characterized HO distribution based on bony landmarks, lacking a detailed investigation into the characteristics of its distribution in periarticular soft tissue in post-traumatic elbows. This study aimed to (1) develop a muscle-guided classification system using computed tomography (CT) to map HO relative to elbow muscle-tendon units and (2) investigate correlations

The post-traumatic formation of myotendinous heterotopic ossification or myositis ossificans traumatica has been an unforeseen complication of muscle contusion-related hematoma in people with hemophilia and von Willebrand's disease. While ultrasound imaging has revolutionized the detection of hemarthrosis and hemarthropathy in bleeding disorders, its clinical use has expanded to the detection of inter- and intramuscular hematomas, as well as the formation of traumatically induced heterotopic ossification. This retrospective multi-center case series includes 29 cases of post-traumatic heterotop

The presence of heterotopic ossification (HO) after primary total knee replacement (TKR) is rare and associated with limited mobility and stiffness of the knee. This study aimed to identify if the arthroscopic debridement after TKR could decrease HO and improve the function and range of motion. Thirty HO patients after TKR were retrospectively separated into 2 cohorts. 15 patients of group A accepted the arthroscopic debridement, while 15 patients of group B only had non-operative treatment, mainly including oral nonsteroidal anti-inflammatory drugs (NSAIDs) and rehabilitative treatment.
