[Stapedo-vestibular ankylosis associated with osteopathy of the promontory (author's transl)].

The authors present two unusual operative findings during surgery for stapedo-vestibular ankylosis, of which no similar cases would appear to be found in the literature. The first case posed the problem of stapedo-vestibular ankylosis as a sequel of clinically latent otitis, the only manifestations of which were radiological opacity of the mastoid and an area of osteitis of the promontory, still active, in the absence of any other progressive inflammatory phenomena or sequelae. Surgery required curettage and opening of the promontory, associated with total stapedectomy and a Teflon-inter-posit

Therapeutic developments for valosin-containing protein mediated multisystem proteinopathy.

PURPOSE OF REVIEW: Missense mutations in valosin-containing protein (VCP) can lead to a multisystem proteinopathy 1 (MSP1) with any combination of limb-girdle distribution inclusion body myopathy (IBM) (present in about 90% of cases), Paget's disease of bone, and frontotemporal dementia (IBMPFD). VCP mutations lead to gain of function activity with widespread disarray in cellular function, with enhanced ATPase activity, increased binding with its cofactors, and reduced mitofusin levels. RECENT FINDINGS: This review highlights novel therapeutic approaches in VCP-MSP in in-vitro and in-vivo mod

Persistently elevated alkaline phosphatase could be related to Paget's disease of bone in a patient receiving tenofovir disoproxil fumarate.

The prevalence of Paget's disease of bone (PDB) reaches 1-2% of all adults aged ≥55 years old. However, reports describing PDB among HIV positive patients are extremely rare. We report here the case of a HIV positive person receiving tenofovir disoproxil fumarate (DF)-based antiretroviral therapy and who had persistently elevated alkaline phosphatase (AP) revealing PDB. It is well established that tenofovir-DF use is associated with reversible increases in serum AP levels. Clinicians should bear in mind that persistently elevated AP in a person receiving tenofovir DF-based cART could be relate

Increased Prevalence of Nephrolithiasis and Hyperoxaluria in Paget Disease of Bone.

CONTEXT: Nephrolithiasis (NL) and primary hyperparathyroidism (HPTH) are metabolic complications of Paget disease of bone (PDB), but recent data regarding their prevalence in PDB patients are lacking. OBJECTIVES: Study 1: To compare the prevalence of primary HPTH and NL in 708 patients with PDB and in 1803 controls. Study 2: To evaluate the prevalence of NL-metabolic risk factors in 97 patients with PDB and NL, 219 PDB patients without NL, 364 NL patients without PDB, and 219 controls, all of them without HPTH. DESIGN: Cross-sectional multicentric study. SETTING: Italian referral centers fo

Characteristics of Early Paget's Disease in SQSTM1 Mutation Carriers: Baseline Analysis of the ZiPP Study Cohort.

Mutations in SQSTM1 are strongly associated with Paget's disease of bone (PDB), but little is known about the clinical characteristics of those with early disease. Radionuclide bone scans, biochemical markers of bone turnover, and clinical characteristics were analyzed in SQSTM1 mutation carriers who took part in the Zoledronic acid in the Prevention of Paget's disease (ZiPP) study. We studied 222 individuals, of whom 54.9% were female, with mean ± SE age of 50.1 ± 0.6 years. Twelve SQSTM1 mutations were observed, including p.Pro392Leu, which was present in 141 of 222 (63.5%) subjects. Bone sc

Paget's disease of bone in a patient on hemodialysis.

Paget's disease is a disorder of bone remodeling. Its occurrence in hemodialysis patients is very rare. Here, we report a case of Paget's disease in a 77-year-old patient on hemodialysis who presented with elevated serum level of alkaline phosphatase without any clinical or laboratory findings of secondary hyperparathyroidism. To our best knowledge, this is the first reported case of Paget's disease in a patient with end-stage renal disease from Iran.

Sarcoma arising in Paget's disease of bone: declining incidence and increasing age at presentation.

BACKGROUND: Paget's disease is presenting at older ages, with a trend to monostotic disease, associated with lower serum total alkaline phosphatase (serum total ALP) levels. Sarcomatous transformation is a rare complication, which only half a century ago had a median age of presentation of less than 60 years. We have investigated whether sarcomatous change exhibits increasing age at presentation, whether the trend to monostotic Paget's disease exists, whether male predominance in sarcoma still continues and explored factors that might affect survival. METHODS: Notes of all patients from the R

Commonly encountered radiolucencies and radiopacities of the jaws.

Radiolucencies and radiopacities of the jaws are frequently encountered during the course of routine radiographic examination. Understanding when such radiographic features indicate a need for biopsy and histopathologic examination is important for appropriate patient management.

Vertebroplasty in focal Paget disease of the spine. Case report.

Paget disease is an idiopathic metabolic disease of bone that may involve the axial and appendicular skeleton. In up to one third of patients there may be pagetoid involvement of the spine, which can cause back pain and vertebral collapse, with instability or myeloradiculopathy. Although medical therapy is the mainstay of treatment, decompressive surgery or stabilization may be required. The authors report on a case of localized Paget disease of the spine treated successfully by performing percutaneous vertebroplasty. They propose this procedure as a useful intervention that can be undertaken

Paget's disease of the spine: unusual features and complications.

Paget's disease of bone is a common disorder, with the spine being involved in over 50% of cases. This pictorial review illustrates unusual features and complications of the disease as related to the spine. Unusual features include location in the atlanto-axial region, lytic vertebral Paget's disease and Pagetic ankylosis. Complications related to the spine are mainly neurological due to spinal stenosis, compression fractures and sarcomatous degeneration

Coccygeal fracture and Paget's disease presenting as acute cauda equina syndrome.

Paget's disease of bone affects a significant percentage of adult and elderly patients. Although generally asymptomatic, the inflammatory changes and hypervascularity of the affected bone is prone to pathologic fracture and resultant hemorrhage. Epidural hematoma is well-described with vertebral fractures and can present as acute cauda equina syndrome. We describe a case in which an elderly female with Paget's disease sustained a minor coccygeal fracture and developed local hemorrhage and edema, which caused a sacral plexopathy that presented identical to acute cauda equina syndrome. Thirty cases of concurrent Paget's disease and primary hyperparathyroidism: sex distribution, histomorphometry, and prediction of the skeletal response to parathyroidectomy.

Studies of the effect of parathyroidectomy (PTX) on bone turnover in patients with the combination of primary hyperparathyroidism (PHPT) and Paget's disease (PD) are largely limited to case reports. The etiology of the combination is disputed. We report 30 patients and their biochemical (n = 17) and histomorphometric (n = 4) responses to PTX in 18. All 18 patients except one had a post-PTX fall in plasma alkaline phosphatase (pAP). There was a significant positive correlation between the degree of post-PTX fall in pAP and both the preoperative plasma total corrected calcium (CaC) (P < 0.01) an

Increased nucleolar organizer regions in osteoclast nuclei of Paget's bone disease.

The etiology of Paget's disease of bone is still unknown but several studies have reported a viral origin. At the electron microscopic level, characteristic nuclear and cytoplasmic inclusions have been found and mimic paramyxoviral nucleocapsids in osteoclasts (Oc). Sarcomatous degeneration is observed in 2% of pagetic patients. Nuclear organizer regions are parts of the nucleolus involved in the synthesis of ribosomes, and they contain nonhistone proteins that can be silver stained (AgNORs) on the interphasic nuclei. AgNOR number is known to correlate with the proliferative activity of the ce

Revision total hip arthroplasty with correctional femoral osteotomy in Paget's disease.

Varus alignment of the femoral component is associated with femoral component loosening in total hip arthroplasty performed for Paget's disease. Irregular and hemorrhagic bone, along with angular femoral deformity, was encountered during revision total hip arthroplasty in three pagetic patients. A diaphyseal femoral osteotomy facilitated cement removal and provided an opportunity for correction of the deformity. The step-cut configuration of the osteotomy provided intrinsic rotational stability of the femoral segments around a modular, long-stem cementless implant. Excellent clinical and radio

Comparison of the efficacy and bioequivalence of two oral formulations of tiludronate in the treatment of Paget's disease of bone.

Tiludronate, an oral bisphosphonate used to treat Paget's disease of bone, is currently being studied as a treatment for osteoporosis. A multicenter, open-label, parallel-group study was performed to compare the efficacy of two tablet formulations of tiludronate in the treatment of Paget's disease. Eighty-eight patients with active Paget's disease were recruited. The diagnosis was based on radiologic evidence of bone lesions, and all patients included in the study had serum alkaline phosphatase (SAP) levels equal to or more than twice the upper normal value of the local laboratory that assayed

Peyronie's disease is associated with Paget's disease of bone.

Peyronie's disease is an idiopathic disorder in which an inflammatory fibrosis occurs in the tunica albuginea of the corpora cavernosa which causes the erect penis to become deformed. Peyronie's disease has a prevalence of 1% in men over age 50 years. Paget's disease of bone is a chronic skeletal disease with areas of increased bone turnover leading to pain, deformity, and in some cases arthritis. Because of a high rate of Peyronie's disease in subjects in a Paget's disease industry-sponsored drug trial, we asked whether there was an association between Peyronie's disease and Paget's disease o
