Lone pyogenic involvement of the posterior elements of the vertebrae is a rare event. We present two cases of isolated lumbar vertebral infection of the posterior elements and review 13 cases previously reported in the literature. Clinical presentation, laboratory and radiographic findings, microbiological etiology, and treatment were evaluated. Back pain, an elevated erythrocyte sedimentation rate, and computed tomography or magnetic resonance imaging were most useful in identifying the presence and extent of infection. Antibiotic therapy with or without surgical intervention was successful i

It is unusual for osteomyelitis of the sesamoid bones to occur after a puncture wound to the foot. When the puncture occurs through tennis shoes, the risk of Pseudomonas infection is increased. Pseudomonas was the causative organism in 7 of 22 cases reported in the literature. This report will explore the causes and natural history, as well as the treatment, of these infections. Initially, basic wound care principles should be adhered to when treating these wounds. Patient awareness and close follow-up is important to ensure complete healing. Treatment of this case involved intravenous antibio…

Cryptococcus neoformans is a rarely reported cause of osteomyelitis. In most cases, no obvious underlying condition is found. Immunological laboratory data, however, are not generally available. In the present case of cryptococcal osteomyelitis, idiopathic CD4 lymphopenia was detected. This immunodeficiency is found in cases of disseminated cryptococcosis by chance. Possibly, it may be one so far unrecognized underlying condition in cryptococcal osteomyelitis.

For the diagnosis of acute and chronic osteomyelitis imaging methods have become essential. This paper reviews the potential of the different imaging modalities. When there is clinical suspicion of acute osteomyelitis plain films are still the mainstay of diagnosis. In newborns and young children this primary diagnostic modality will be supplemented by sonography. If there is a need for further imaging, MRI and the different scintigraphic methods may be used interchangeably. However, for the spine and other complex anatomical regions MRI is preferred. In contrast, three-phase bone scanning is …

Apophysomyces elegans, a member of the family Mucoraceae, was found to infect the chest wall and sternum of an immunocompetent man following minor trauma. As in previous cases, amphotericin B therapy alone was inadequate. Extensive surgical debridement was required in order to eradicate the infection.

Puncture wounds of the foot are commonly seen in the hospital emergency room. They are usually managed appropriately; however, a percentage go on to skin infection with subsequent joint and osseous involvement. This is particularly true in the adolescent patient. This manuscript was written to inform the foot specialist on the etiology, medical, and surgical management of osteomyelitis secondary to Pseudomonas aeruginosa. Also, two pediatric case histories that occurred within 6 months of each other are presented.

A case of osteomyelitis of the sternum due to Brevibacterium sp. in a neonate following mastitis of the mammary gland 20 days previously is described. The disease was successfully treated with cefazolin and oxacillin. The results of microbiological examinations and their significance are discussed.

Bone involvement in patients with Gaucher's disease is common, and some of its clinical manifestations may resemble acute osteomyelitis. In addition, many studies have emphasized the high risk of secondary infection when surgical procedures are performed at the site of the involved bone. Nevertheless, these infections have never been well documented in the literature. Presentation of a patient with Gaucher's disease and osteomyelitis due to Prevotella (Bacteroides) melaninogenica provided us the opportunity to review 10 other similar case reports documented in the literature since 1966. This r…

A case of mandibular osteomyelitis in a child affected by osteopetrosis is presented. Systemic antibiotic therapy was ineffective and surgery was required to obtain a partial control of infection.

Three children with suppurative arthritis and osteomyelitis are described to emphasise that delayed or incorrect diagnosis may lead to serious cardiopulmonary complications. In two patients, bilateral bronchopneumonia developed with pneumatocoeles, pneumothorax and empyema. The other had cardiac failure from septic pericarditis. In one case, disarticulation of the knee was needed as a life-saving measure, and the other leg developed an infected pseudarthrosis of the tibia. The causative organism in each case was staphylococcus aureus and no evidence of immunodeficiency was demonstrated.

The article presents principles of the surgical treatment of hematogenic osteomyelitis of the vertebral column and its complications developed on the basis of studying and analysis of results of operative interventions performed in 145 patients. A system of operative procedures is proposed which showed high effectiveness in treatment of both purulent infections and complications of the disease, spinal disorders in particular.

Chronic osteomyelitis is a frequent complication after compound fractures of the lower extremity. Essential in the treatment is the radical debridement of all scar tissue, muscle and infected bone. This often creates large tissue defects, which especially in the distal third of the tibia, can only be closed with a free tissue transfer and revascularized by a microvascular anastomosis. Ten patients were operated on for chronic osteomyelitis of the lower extremity. The latissimus dorsi or rectus abdominis muscle was used as a free flap. During the follow-up period (3-18 months) 9 patients have r…

Osseous actinomycosis usually results from direct invasion of bone from adjacent soft-tissue infection. Involvement of the jaw and vertebral column has been frequently reported, but involvement of the bones of the wrist or hand is rare. A patient with osteomyelitis of the distal right first metacarpal bone due to Actinomyces israelii following a punch injury during fisticuffs is described. Review of similar cases revealed a striking association of this type of infection with punch injuries; hence the term, punch actinomycosis. Analysis of the pus in these infections typically reveals sulfur gr…

Cervical osteomyelitis, an uncommon disease, is rarely associated with Pseudomonas organisms in non-drug-users. This report describes a case of cervical vertebral osteomyelitis associated with Pseudomonas aeruginosa pneumonia following panfacial trauma and closed head injury. Delays in diagnosing vertebral osteomyelitis are common because of the nonspecific nature of the history and the initial absence of clinical evidence, both of which may help to foster a generally low index of suspicion.

Marble bone disease is a rare skeletal disorder. Osteomyelitic changes in the jaw bones are frequently seen in this disease. Considering the rarity of the disease, a case of osteomyelitis of the mandible in marble bone disease is presented, together with a review of the available literature.

Hospital morbidity data from New Zealand, Western Australia, England and Wales and Scotland are reviewed. The incidence of osteomyelitis in childhood is greater in males than females, greater in European children in Australia and New Zealand than in Great Britain, and much greater in Aboriginal and New Zealand Maori children than in European children. A seasonal peak is present in late summer and autumn in both hemispheres. The reported incidence is generally declining, except in Western Australia. Local variations in incidence within New Zealand correlate with mean annual vapour pressure. The…

Yeasts belonging to the genus Torulopsis, though generally considered saprophytes, are capable of causing serious illness in compromised patients. Torulopsis infections in patients with no apparent underlying disease are rare. We have described a previously healthy man who had osteomyelitis due to Torulopsis holmii as a complication of a penetrating hand injury. Despite in vitro susceptibility of the organism to amphotericin B and flucytosine and prolonged therapy with these agents, the infection relapsed after an apparent initial response.
