Surgical management of isthmic spondylolisthesis with radicular pain.

Forty-six patients with low-grade isthmic spondylolisthesis (27 grade I and 19 grade II) and radicular pain underwent surgery after failure of conservative treatment for 6 months. Fusion and decompression was carried out in 23 patients (group 1) and fusion without decompression in the other 23 patients (group 2). Instrumentation was used in 16 patients who had instability and hypermobility as seen by dynamic radiography. Results were assessed functionally and radiographically and graded according to Stauffer and Coventry. The follow-up was an average of 20 (12-36) months. In group 1 there were

Update on pain management for advanced genitourinary cancer.

PURPOSE: Pain is a significant problem in many patients with genitourinary malignancy at all stages of disease. Optimal pain control becomes a primary concern as disease progresses and other therapies are exhausted. The selection of the most appropriate therapy becomes difficult without an understanding of the underlying mechanisms of pain and the available therapies. MATERIALS AND METHODS: A review of the literature regarding the mechanisms and assessment of pain syndromes was performed. All available therapies were investigated with respect to conservative management with opioid medications

Reflex sympathetic dystrophy: alternative modalities for pain management.

For the patient presenting with early symptoms (< 6 months) I usually start treatment with a dose pack of methylprednisolone, analgesics, and daily occupational/physical therapy for 2 weeks (Fig. 2). If they do not respond within the first week, I add stellate ganglion blocks and acupuncture to the treatment regimen. For patients presenting with established chronic pain, I immediately start them on a dose pack of methylprednisolone for 1 to 2 weeks, a nonsteroidal anti-inflammatory such as indomethacin, 50 mg 3 times a day for 10 days and then switch to 75 mg twice daily until there is a respo

Surgical management of groin pain of neural origin.

BACKGROUND: An approach to surgical management of the patient with groin pain is described based on our experience with 54 patients, six of whom had bilateral symptoms. History and physical examination are sufficient to relate the pain to one or more of the lateral femoral cutaneous (LFC), ilioinguinal (II), iliohypogastric (IH), or genitofemoral (GF) nerves. STUDY DESIGN: Retrospective analysis of patients with groin pain is reported, with emphasis on cause, involved nerves, and outcomes of operative management. The LFC was decompressed. The II, IH, and GF nerves were resected. Outcomes were

Videotape increases parental knowledge about pediatric pain management.

UNLABELLED: Pediatric pain management often depends on parents recognition and assessment of their child's pain and their beliefs as to whether the pain should be treated. Parental misconceptions concerning pain assessment and pain management may therefore result in inadequate pain treatment, particularly in patients who are too young or too developmentally handicapped to self-report their pain. We hypothesized that viewing a concise, educational videotape would provide parents with instructive information that could correct misconceptions concerning pain and pain management in children. To do…

[Algorithm in low back pain management in adults].

Currently recommended management for low back pain in adults is presented. The algorithm consists of two phases. In a patient presenting acute low back pain with no previous history thorough clinical evaluation (phase I) should aim at exclusion of cauda equina syndrome, fracture, neoplasm, infection, progressive neurological deficit, or chronic pain syndrome. If none of these condition is responsible for low back pain 4-6 weeks therapy depending on the severity of symptoms should be commenced. Then clinical reevaluation is indicated and if pain persists patient should be referred to a speciali

Subcutaneous morphine pump for postoperative hemorrhoidectomy pain management.

PURPOSE: Many anorectal procedures are currently being performed on an outpatient basis, hemorrhoidectomy being the exception because of the need for parenteral narcotics postoperatively. We investigated the effectiveness of a subcutaneous morphine pump (SQMP) for outpatient posthemorrhoidectomy pain control. METHODS: In Phase 1 of our study, 22 patients undergoing radical hemorrhoidectomy were started on an SQMP protocol postoperatively. Twenty-nine patients received conventional postoperative narcotic dosing. In Phase 2, 19 patients enrolled in an SQMP protocol underwent hemorrhoidectomy in

Approaches in Opioid Pharmacology: Toward Safer Pain Management.

Opioids are crucial for pain management, but the traditional mu-opioid receptor agonists are associated with considerable risk, such as addiction, overdose, and serious side effects such as respiratory depression. This review aims to emphasize the recent developments in the development of safer opioids, including approaches such as mixed agonists at MOR, KOR, and DOR, biased agonists at MOR, KOR, and DOR, and allosteric modulation. Other approaches, such as peripherally acting opioids and agonists at MOR splice variants, are also discussed as safer options for pain management without the risk …

The Cutting Edge: Interventional Strategies for Pain Management.

When caring for spine patients, accurate diagnosis is essential, as interventional treatments are only effective when the true pain generator is correctly identified. Diagnostic procedures generally fall into provocative tests, which reproduce pain, and analgesic blocks, which relieve pain through anesthetic injections. However, both approaches are limited by false positives, particularly with uncontrolled medial branch or sacroiliac blocks. Controlled comparative blocks improve accuracy, but predictive value remains modest unless combined with history, examination findings, and imaging. This

Neuraxial anesthesia and pain management for cesarean delivery.

Optimal neuraxial anesthesia for cesarean delivery requires a thorough understanding of patient, obstetrical, surgical, and anesthesia-related factors which can impact pain during and after cesarean delivery. While not all cesarean deliveries are the same from an obstetrical standpoint, not all anesthetics provide the same degree of anesthetic blockade and postcesarean analgesia; therefore, context is crucial to provide patients with a safe and pain-free experience. Communication between obstetrical and anesthesia teams is key to ensure that the anesthetic approach is tailored to the clinical

Cannabinoids for Acute Pain Management: Approaches and Rationale.

PURPOSE OF THE REVIEW: Acute pain management remains a challenge and postoperative pain is often undermanaged despite many available treatment options, also including cannabinoids. RECENT FINDINGS: In the light of the opioid epidemic, there has been growing interest in alternative care bundles for pain management, including cannabinoids as potential treatment to decrease opioid prescribing. Despite the lack of solid evidence on the efficacy of cannabinoids, their use among patients with pain, including those using opioids, is currently increasing. This use is supported by data suggesting that…
