A 49-year-old man was admitted to our hospital with a productive cough and bloody sputum. Peripheral blood eosinophil counts and serum IgE values were elevated. Chest radiography and chest computed tomography revealed a solitary nodule with cavitation and satellite lesions in the right middle lobe, in contact with the pleura. The diagnosis of paragonimiasis westermani was confirmed by detection of ova in the sputum and bronchoalveolar lavage fluid (BALF) samples, and by immunoserological examination. The patient was treated with praziquantel for 3 days at a daily dosage of 75 mg/kg. After trea…

To observe antibody changes after praziquantel treatment in paragonimiasis, a total of 46 serum samples from 13 serologically diagnosed patients was collected for 4-28 months. The specific antibody (IgG) levels were measured by enzyme-linked immunosorbent assay (ELISA). All but one patient who needed retreatment became symptom-free within a week. Antibody levels were dropped near to or below a cut-off absorbance (abs.) of 0.25 in varying intervals from 4 to 18 months. Of 9 patients who were retested within 3 months, 5 revealed temporary elevation of antibody level. After the elevation, the lev…

Paragonimiasis, a cause of pleuropulmonary disease, is a neglected etiology of pediatric pleural effusion. We retrospectively analyzed children with paragonimiasis-induced pleural effusion admitted to our center, aiming to summarize their diagnosis, treatment, and prognosis. A total of 103 children were included, with a mean age of 8.2 years, and 87.4% resided in rural areas. 81/91 (89.0%) were initially misdiagnosed with other infectious diseases including community-acquired pneumonia (43.9%; 40/91) and tuberculosis (16.5%; 15/91). The predominant blood test findings included hypereosinophili…

INTRODUCTION: Paragonimiasis, lung fluke disease caused by infection with Paragonimus species, is a food-borne parasitic zoonosis. The overriding symptoms of Paragonimus westermani infection include chronic cough, shortness of breath, and pleuritic pain. Extrapulmonary paragonimiasis caused by aberrant parasitic migration is known to occur in a variety of sites such as the brain, abdominal wall, and intraperitoneal cavity. Ectopic paragonimiasis is an uncommon disease that presents with a few clinical manifestations, which makes it difficult to diagnose and treat.

Paragonimiasis, a chronic zoonotic parasitic infection caused by Paragonimus species, is frequently misdiagnosed due to its nonspecific clinical presentation and low disease awareness. We report a case of a 42-year-old male who was admitted following head and chest trauma. Initial chest CT revealed bilateral pulmonary contusions, a right rib fracture, and a significant pleural effusion, consistent with traumatic lung injury. Routine blood tests showed elevated white blood cells and neutrophils, but eosinophil counts remained within the normal range. Closed thoracic drainage was performed, and …

Some paragonimiasis patients in Chongqing, southwest China, have recently exhibited pleural effusions (PEs) with massive viscous secretions. This study aimed to investigate their clinical characteristics, thereby promoting effective treatments. A 3-year retrospective review of paragonimiasis patients who were admitted for nonhomogeneous PEs at Chongqing University Three Gorges Hospital was conducted. Epidemiological data, symptoms, laboratory and imaging findings, treatments, and outcomes were analyzed. Twenty-eight patients were identified, of which 22 (78.6%) were males and 22 (78.6%) were r…

During the mobile clinic activities in Tak Province, Thailand, Paragonimus sp. eggs were found in a fecal sample of a 72-year-old Karen resident. Paragonimus DNA was amplified from the stool sample and identified to P. heterotremus. The patient did not have any symptoms. Apparent pulmonary lesion was not found on the chest X-ray. The patient admitted habitual consumption of semi-cooked or roasted waterfall crabs for several years. The waterfall crabs collected from stream near the village were found negative for Paragonimus metacercariae. In northern Thailand, paragonimiasis remains as one of …

North American paragonimiasis is well described in omnivorous and carnivorous animals on this continent. Humans are rarely infected, largely because of dietary customs, but are at risk for infection if raw or undercooked crayfish are consumed. We describe a patient with a pleuropulmonary infection due to Paragonimus kellicotti that presented as recurrent pneumothoraces and a cavitary lesion. This is the first case of North American paragonimiasis in which the diagnosis was based on the morphology of the eggs present in histologic sections.

Antibody responses in serum and cerebrospinal fluid (CSF) samples from patients with active and chronic paragonimiasis and in sera from patients on whom follow-up studies were done after praziquantel treatment were analyzed using antigens of Paragonimus westermani prepared from eggs, metacercariae, juveniles of 4- and 7-week old, adult worms and recombinant protein of 28 kDa cruzipain-like cysteine protease (rPw28CCP). The patient sera/CSFs of active and chronic paragonimiasis revealed strong antibody reactions against the crude extracts of 4- and 7-week old juveniles as well as against those …

BACKGROUND: Paragonimiasis is a parasitic disease characterized by diverse clinical manifestations, among which pulmonary involvement is the most frequently observed. However, there are no documented cases of paragonimiasis presenting as a mediastinal mass. We report a case of paragonimiasis associated with such a presentation. CASE PRESENTATION: A large mediastinal mass was identified in a 13-year-old child. Due to the limited efficacy of antibiotics and indeterminate diagnosis, thoracoscopic surgery was performed. Histopathological findings were consistent with paragonimiasis. The patient re…

CASE SUMMARY: A 1-year-old intact female domestic shorthair cat presented for evaluation of worsening respiratory distress and tachypnoea. The cat had been diagnosed with spontaneous pneumothorax 4 days earlier by the primary clinician and treated with therapeutic thoracocentesis. On physical examination, the patient exhibited decreased lung sounds, tachypnoea and increased breathing effort. Recurrent spontaneous pneumothorax was confirmed via thoracic radiography, and therapeutic thoracocentesis was repeated. Lung ultrasound performed after thoracocentesis revealed lung consolidation, pulmona
