Shy-Drager syndrome: diagnosis and treatment of cholinergic dysfunction.

Two patients with Shy-Drager syndrome demonstrated unusually widespread and unequivocal cholinergic dysfunction as well as the usual evidence of adrenergic insufficiency. Progressive constipation preceded impotence, nocturia, hesitancy in micturition, anhidrosis, orthostatic hypotension, and xerostomia. Nonautonomic neurologic signs appeared several years later. Cholinergic dysfunction involved eyes, lacrimal glands, salivary glands, heart, gastrointestinal tract, urinary bladder, and sweat glands. Subcutaneous administration of bethanechol chloride--a muscarinic receptor agonist--improved tea

Heart-rate variability for discrimination of different types of neuropathy in patients with insulin-dependent diabetes mellitus.

It has been shown that patients with insulin-dependent diabetes mellitus (IDDM) may reveal abnormal alterations in heart-rate variability (HRV) due to autonomic neuropathy. This study was performed to prove whether heart-rate variability can be used to stratify diabetic patients with different types of neuropathy. 48 patients with IDDM (age 17-64 yr) underwent standard function tests to assess autonomic and peripheral neuropathy. According to the results of these tests they were divided into 4 groups: Group 1: 18 patients without autonomic or peripheral neuropathy. Group 2: 13 patients with pe

Peripheral nerve involvement in Fabry's disease: Which investigations? A case series and review of the literature.

BACKGROUND: Peripheral nerve system (PNS) involvement is common in Fabry's disease (FD), predominantly affecting the small nerve fibers that are difficult to investigate with conventional electrophysiological methods. PATIENTS AND METHODS: Eighteen patients followed for Fabry's disease underwent a prospective series of electroneurophysiological explorations, including a study of the cardiac parasympathetic autonomic nervous system (ANS) and electrochemical skin conductance (ESC) tests. Data were compared with those obtained in 18 matched healthy controls. RESULTS: All patients had at least o

Endorphin-parasympathetic interactions in spinal shock.

We have recently shown that the opiate antagonist naloxone can partially reverse the hypotension which accompanies spinal cord transection, through actions on opiate receptors in the central nervous system. In the present studies we demonstrate that either vagotomy or atropine prevents the ability of naloxone to reduce the hypotension caused by spinal transection in the rat and cat. Since the peripherally acting methyl-atropine blocks the cardiovascular effects of centrally administered naloxone in this model, these findings imply an interaction between endorphin systems and central parasympat

Activating Muscarinic Receptor Signaling in Intrapancreatic Neurons Is Required for Parasympathetic Cholinergic Control of Pancreatic Cell Function.

UNLABELLED: The parasympathetic nervous system modulates hormone and digestive enzyme secretion from the pancreas. However, the mechanisms of neuroeffector transmission within the final parasympathetic pathway in the pancreas have not been elucidated. Here, we demonstrate that intrapancreatic cholinergic neurons are bona fide postganglionic neurons that functionally couple vagal input to target cells in the pancreas. In living pancreatic slices from various mice expressing genetically encoded sensors and actuators, we found that intrapancreatic neurons responded to cholinergic input via nicoti…

Soothing touch matters: Patterns of everyday mother-infant physical contact and their real-time physiological implications.

Physical contact between infants and caregivers is crucial for attachment development. Previous research shows that skin-to-skin contact after birth and frequent baby wearing in the first year predict secure attachment at 12-months. This relationship is thought to be mediated by the activation of infants' parasympathetic nervous system through caregiver touch. However, little is known about everyday touch behaviors and their impact on infants' real-time parasympathetic activity. Laboratory observations may not accurately represent real-world interactions, highlighting the need for ecologically…

Gut Neuropathies and Intestinal Motility Disorders.

BACKGROUND: The enteric nervous system plays a key role in the coordination of gastrointestinal motility together with sympathetic, parasympathetic, and extrinsic sensory pathways. In some cases, abnormalities in neural activity in these pathways contribute to disorders of gut motility. Where this is associated with damage or death of enteric neurons, usually detected by microscopy, this is considered a gut neuropathy. PURPOSE: This review summarizes recent advances in the identification of neuropathies in a range of gastrointestinal motility disorders.

Cardiac vagal recovery following acute psychological stress in human adults: A scoping review.

Vagally mediated heart rate variability (vmHRV) recovery following acute psychological stress reflects parasympathetic function and stress resilience, yet there is no synthesis on how it is measured and influenced by key factors. This scoping review mapped methodologies used to assess vmHRV recovery and examined clinical, demographic, psychological, physiological and environmental influences. Following PRISMA-ScR guidelines, a comprehensive search of MEDLINE, PsycINFO, and EMBASE databases identified empirical peer-reviewed studies on cardiac vagal recovery in adults published since 2000 that

Evidence Suggesting that the Buccal and Zygomatic Branches of the Facial Nerve May Contain Parasympathetic Secretomotor Fibers to the Parotid Gland by Means of Communications from the Auriculotemporal Nerve.

BACKGROUND: The auriculotemporal nerve is one of the peripheral nerves that communicates with the facial nerve. However, the function of these communications is poorly understood. Details of how these communications form and connect with each other are still unclear. In addition, a reliable anatomical landmark for locating these communications during surgery has not been sufficiently described. METHODS: Microdissection was performed on 20 lateral hemifaces of 10 soft-embalmed cadavers to investigate facial-auriculotemporal nerve communications with emphasis on determining their function. The

[The parasympathetic direct pathway from the midbrain to the ciliary muscle in cats and monkeys].

The visceral, namely the parasympathetic component of the oculomotor complex which directly innervates ciliary muscle was investigated using horseradish peroxidase (HRP) in three cats and three monkeys. A unilateral total iridectomy to abolish innervation to the iris was made in all animals. After 1 month to allow fiber degeneration in the iris, HRP was injected into the ciliary muscle of ipsilateral eye. Distribution of the labeled cells at the midbrain and at the ciliary ganglion was examined. Labeled cells were present in the ciliary ganglion and midbrain in all three cats and three monkeys

Sluggishness of pupillary light contraction in patients with Menière's disease.

The pupillary light responses of both eyes were analysed simultaneously at both the attack and/or quasiattack stages and the interval stage in 10 patients with Meniere's disease, using a computed pupillograph. A sluggishness of pupillary contraction on the affected side was found at the attack and/or quasiattack stage in 8 out of 10 cases, and disappeared at the interval stage in 7 out of the 8 cases. However, no significant differences between the left and right pupillary contractions were revealed during caloric irrigation of the left or right ear in 10 normal subjects. Therefore, it can be

Lacrimal sweating in a patient with Reader's syndrome.

A patient is described with a right-sided Reader's syndrome and extensive sweating in the frontal area above the right eye. This pathological sweating can be explained by sprouting of lacrimal parasympathetic fibres in the previous denervated sympathetic sudomotor pathways to that area.

Relationship between psychosomatic diagnoses and various autonomic indicators: which diagnoses have stronger associations?

BACKGROUND: Various diagnoses in psychosomatic medicine have been found to exhibit features of autonomic activity, primarily characterized by low parasympathetic activity. However, because depression, anxiety, and somatic symptoms often coexist, it remains to be clarified whether certain diagnoses are more strongly associated with autonomic activity than others. We also aim to explore whether the relationship between diagnosis and autonomic activity is through a top-down (from brain to autonomic nervous system) mechanism. METHODS: A total of 502 participants were included in this study. All p

State- and frequency-dependence in autonomic rebalance mediated by intradermal auricular electroacupuncture stimulation.

BACKGROUND: Vagus nerve stimulation (VNS) improves diseases such as refractory epilepsy and treatment-resistant depression, likely by rebalancing the autonomic nervous system (ANS). Intradermal auricular electro-acupuncture stimulation (iaES) produces similar effects. The aim of this study was to determine the effects of different iaES frequencies on the parasympathetic and sympathetic divisions in different states of ANS imbalance. METHODS: We measured heart rate variability (HRV) and heart rate (HR) of non-modeled (normal) rats with the treatment of various frequencies to determine the opti

Reproducibility of tilt table testing in patients with vasovagal syncope and its relation to variations in autonomic nervous system activity.

To assess the variability of head-up tilt table testing, 35 patients with vasovagal syncope, shown by a positive tilt table test, underwent a second test 1 week later. Also, on the day before each test, spectral and time-domain indexes of heart rate variability were derived from Holter recordings to examine the stability of autonomous nervous system activity in these patients. Fifteen healthy volunteers served as a control group and also underwent two tilt table tests with prior Holter monitoring. Twenty-one (60%) of the 35 patients had a second positive test. None of the patients in the contr

Autonomic regulation of the immune system in cardiovascular diseases.

The autonomic nervous system is a powerful regulator of circulatory adjustments to acute hemodynamic stresses. Here we focus on new concepts that emphasize the chronic influence of the sympathetic and parasympathetic systems on cardiovascular pathology. The autonomic neurohumoral system can dramatically influence morbidity and mortality from cardiovascular disease through newly discovered influences on the innate and adaptive immune systems. Specifically, the end-organ damage in heart failure or hypertension may be worsened or alleviated by pro- or anti-inflammatory pathways of the immune syst
