Patients With Opioid Use Disorder Have Increased Readmission Rates, Emergency Room Visits, and Costs Following a Hallux Valgus Procedure.

BACKGROUND: Patients with a history of opioid use disorder (OUD) tend to have more complications, higher readmission rates, and increased costs following orthopaedic procedures. This study evaluated patients undergoing hallux valgus correction for their odds of increased (1) readmission rates, (2) emergency room (ER) visits, and (3) costs. METHODS: Patients undergoing hallux valgus corrections with OUD history were identified using a national Medicare administrative claims database of approximately 24 million orthopaedic surgery patients. OUD patients were matched to non-opioid use disorder (

Association Between Left Atrial Appendage Occlusion and Readmission for Thromboembolism Among Patients With Atrial Fibrillation Undergoing Concomitant Cardiac Surgery.

IMPORTANCE: The left atrial appendage is a key site of thrombus formation in atrial fibrillation (AF) and can be occluded or removed at the time of cardiac surgery. There is limited evidence regarding the effectiveness of surgical left atrial appendage occlusion (S-LAAO) for reducing the risk of thromboembolism. OBJECTIVE: To evaluate the association of S-LAAO vs no receipt of S-LAAO with the risk of thromboembolism among older patients undergoing cardiac surgery. DESIGN, SETTING, AND PARTICIPANTS: Retrospective cohort study of a nationally representative Medicare-linked cohort from the Soci

Not All Falls Are Equal: Risk Factors for Unplanned Readmission in Older Patients After Moderate and Severe Injury-A National Cohort Study.

OBJECTIVES: Readmission after acute care is a significant contributor to health care costs, and has been proposed as a quality indicator. Our earlier studies showed that patients aged ≥55 years who are injured by falls from heights of ≤0.5 m were at increased risk for long-term mortality, compared to patients by high-velocity blunt trauma (higher fall heights, road injuries, and other blunt trauma). We hypothesized that these patients are also at higher risk of readmission, compared to patients injured by high-velocity mechanisms. DESIGN AND MEASURES: Competing risks regression (all-cause unp

Iatrogenic risk factors associated with hospital readmission of elderly patients: A matched case-control study using a clinical data warehouse.

WHAT IS KNOWN: Hospital readmission within 30 days of patient discharge has become a standard to judge the quality of hospitalizations. It is estimated that 14% of the elderly, people over 75 years old or those over 65 with comorbidities, are at risk of readmission, of which 23% are avoidable. It may be possible to identify elderly patients at risk of readmission and implement steps to reduce avoidable readmissions. OBJECTIVE: The aim of this study was to identify iatrogenic risk factors for readmission. The secondary objective was to evaluate the rate of drug-related readmissions (DRRs) amon

Long-term non-invasive ventilation reduces readmissions in COPD patients with two or more episodes of acute hypercapnic respiratory failure.

BACKGROUND: Chronic obstructive pulmonary disease (COPD) patients who have had an episode of acute hypercapnic respiratory failure (AHRF) have a large 1-year risk of death or readmission. Acute non-invasive ventilation (NIV) has been shown to be an effective treatment of AHRF; and long-term NIV (LTNIV) has been shown to be an effective treatment of chronic respiratory failure in stable hypercapnic COPD. We investigated the effects of LTNIV in a group of patients with severe, unstable COPD: frequent admissions and multiple previous episodes of AHRF treated with NIV. METHODS: We conducted a ret

Thirty-day readmission rate and risk factors for patients undergoing single level elective anterior lumbar interbody fusion (ALIF).

Anterior lumbar interbody fusion (ALIF) represents a common interbody fusion technique and is advantageous given reduced risk of damage to the paraspinal muscles, posterior ligaments, and neural elements. In this study, we identified the readmission rate, common causes, and risk factors associated with single level ALIF 30-day readmission. Patients who underwent elective single level ALIF surgery from 2011 to 2013 were identified in the American College of Surgeons-National Surgical Quality Improvement Program (ACS-NSQIP) database. Segmental fusion, emergency, and trauma cases were excluded. A

Short- and long-term results of a programme for the prevention of readmissions and mortality in patients with heart failure: are effects maintained after stopping the programme?

UNLABELLED: The objective of the study was to evaluate whether improvements obtained during an intervention programme were maintained after the programme was stopped. 153 patients discharged with a diagnosis of heart failure (HF) were randomized to either usual care or an intervention programme, which included patient education, consultation with the cardiologist and monitoring in the Heart Failure Unit. After an average period of 16+/-8 months, the intervention programme was stopped. One year later, all the patients were re-examined to assess HF readmissions, all-cause mortality, quality of l

The utility of the Risk Assessment and Prediction Tool in predicting discharge disposition, length of stay, and readmissions in patients undergoing transforaminal lumbar interbody fusion.

OBJECTIVE: The Risk Assessment and Prediction Tool (RAPT) has been utilized to anticipate discharge needs after procedures such as total joint arthroplasty. Its usefulness for spine patients, particularly those undergoing transforaminal lumbar interbody fusion (TLIF), has not been clearly established. This study evaluated the relationship between the preoperative RAPT score and 3 postoperative outcomes: discharge destination, hospital length of stay (LOS), and 30-day readmission. METHODS: A retrospective cohort study was conducted of adults who underwent elective TLIF with a recorded preopera

Association between prognostic nutritional index and 90-day readmission in patients with acute exacerbation of chronic obstructive pulmonary disease: a multicenter study.

BACKGROUND: Chronic obstructive pulmonary disease (COPD) is marked by chronic airway inflammation and obstructive ventilation dysfunction. The prognostic nutritional index (PNI), an indicator for evaluating nutritional and immune status with confirmed prognostic value in most diseases, has an unclear association with short-term acute re-exacerbation of COPD. This study thus investigated the correlation between PNI levels and 90-day acute re-exacerbation in COPD patients. METHODS: A cohort analysis was conducted on AECOPD patients, who were grouped by PNI tertiles. The primary outcome was 90-d

Evaluating the Impact of Early Palliative Care Consultation on 30-Day and 90-Day Readmissions Among Patients With Decompensated Cirrhosis: A National Perspective.

INTRODUCTION: Decompensated cirrhosis leads to recurrent hospitalizations among patients with chronic liver disease. Addressing goals of care early can help alleviate stress, reduce health care utilization and potentially improve quality of life in these patients. Research has shown that palliative care (PC) is used infrequently and often too late in decompensated cirrhosis. Moreover, patients with cirrhosis who are denied a liver transplant are rarely referred to PC. We investigated the impact of PC consultations on unplanned readmissions among patients with decompensated cirrhosis in a natio

Effectiveness of a home-based physical exercise intervention in patients with fragility fractures on functional independence and hospital readmissions: a protocol for a randomised controlled trial.

INTRODUCTION: Patients with fragility fractures are two times as likely to suffer future fractures as their peers who have not suffered a fracture. In addition, 40% of those who suffer fragility fractures do not recover their level of functioning in terms of activities of daily living after 1 year. The present study aims to verify the hypothesis that a semipersonalised home-based exercise intervention may improve patients' independence and reduce the number of hospital admissions compared with usual care for a population that suffers fragility fractures. METHODS AND ANALYSIS: This parallel-ar

Interaction with SMS text-reminders correlate with improved medication adherence and readmission rates for congestive heart failure patients: A retrospective cohort study.

Short message service text reminders (SMS nudges) aimed to help vulnerable patient populations remember to fill their prescriptions are becoming more common but accurately measuring their effects on improving prescription fill and readmission rates remains challenging. Patients who presented to the emergency department (ED) with a primary diagnosis of congestive heart failure (CHF) were included in the study. We conducted a retrospective cohort study of CHF patients who did and did not interact with SMS nudges, then matched patients who were prescribed medications at any point in their hospita

Subclinical Congestion Evaluated by Point of Care Ultrasound (POCUS) at Discharge Predicts Readmission in Patients with Acute Heart Failure: Prognostic Cohort Study.

Background: Heart failure (HF) is a complex entity that increases the risk of adverse outcomes. Point of care ultrasound (POCUS) allows easy lung and systemic venous congestion identification. Using ultrasound to detect sub-clinical congestion at discharge may help predict readmissions and mortality. Outcomes: The primary outcome was to address 30-day rehospitalization, and as a secondary outcome we investigated readmission and mortality in patients with residual congestion assessed with POCUS. Methods: A prospective prognostic cohort study was conducted at a tertiary-level institution in Colo
