AIM: We aim to study the utility of perforator-based pentagonal gluteal V-Y advancement skin and subcutaneous flap (PPVYAF) in the reconstruction of perineal, pelvic, and vaginal defects, following abdominoperineal resection or total pelvic exenteration for colorectal and anorectal cancers.
METHODS: A prospectively maintained database of patients who had PPVYAF after major pelvic-perineal surgery was analyzed retrospectively between December 2014 and January 2022. A total of 110 were reconstructed with PPVYAF, of which 33 also had vaginal reconstruction.

Vacuum assisted closure is the controlled application of sub-atmospheric pressure to the local wound environment using a sealed dressing connected to a vacuum pump.
MATERIALS AND METHODS: The NPWT consists of an open-pore polyurethane ether foam sponge, an adhesive cover, fluid collection system, and suction pump that generates negative pressure. Direct application of sponge to blood vessels, bone, nerves or intact skin is avoided.

PURPOSE: We describe a technique that facilitates retrograde ureteral cannulation and stenting in patients with an ileal conduit.
MATERIALS AND METHODS: A standard flexible gastroscope is inserted into the ileal conduit. Guide wires are passed into the ureters under direct vision. Stents are inserted over the guide wires with the aid of the image intensifier.
RESULTS: The technique was used repeatedly in 1 patient with a complex history who had ureteral strictures. The procedure is technically easy and it was successful on each occasion.

The authors present a series of forty patients operated for invasive bladder tumors by radical cystectomy. All patients were older than 70 and had a complicated medical history. Mortality was 1 in 40 and the morbidity was low. The conclusion is that radical cystectomy is a safe procedure in elderly patients.

A 44-year-old woman came to our department because of abnormal cytology that persisted immediately after a radical hysterectomy for NEC of the cervix.
