BACKGROUND: Post-operative pain following root canal therapy remains a prevalent clinical issue, especially in cases of symptomatic apical periodontitis, where inflammatory mediators such as interleukin-8 (IL-8) and substance P are critically involved in the pathophysiology. This study aimed to assess and compare the efficacy of LLLT and Calcium Hydroxide (CH) as intracanal medication in alleviating post-operative pain, percussion sensitivity, and in reducing the levels of IL-8 and substance P in periapical exudate one week after root canal instrumentation in patients with symptomatic apical p

Chronic odontogenic maxillary sinusitis (COMS), a prolonged inflammation of the maxillary sinus lasting over 12 weeks, is often a result of periapical lesions, marginal periodontitis, and complications like oro-antral communication (OAC) and fistula (OAF). OAC, commonly emerging post-teeth extraction in the lateral maxilla, lacks documented treatments using advanced platelet-rich fibrin (A-PRF). This study evaluates A-PRF's efficacy in treating COMS and immediately sealing extensive OAC. A case of a 28-year-old male with COMS linked to a periapical lesion and supernumerary molars is presented.

Resulting from bacterial infection, apical periodontitis (AP) is a common inflammatory disease of the periapical region of the tooth. The regeneration of the destroyed periapical alveolar bone and the surrounding periodontium tissues has long been a difficult task in clinical practice. These lesions are closely related to pathogen invasion and an overreactive immune response. It is worth noting that the protective healing process occurs simultaneously, in which mesenchymal stem cells (MSCs) have a crucial function in mediating the immune system and promoting regeneration. Here, we review the r

This case report describes apical infection on tooth number 24 that spread to adjacent teeth, resulting in devitalized teeth numbers 23 and 25. The 25-year-old Caucasian female patient was referred to the endodontic resident clinic because of uncontrolled apical infection. Root-end surgery and root-end filling of teeth numbers 23, 24, and 25 were performed. The histopathological diagnosis was a periapical cyst; however, the clinical surgical finding of a purulence-filled bone cavity also revealed a periapical abscess. After root-end surgery and regenerative therapy using Mineral Trioxide Aggre

Caries is the most common cause of pulp-periapical disease. When the pulp tissue involved in caries becomes irreversibly inflamed and progresses to necrosis, the treatment option is root canal therapy because the infected or non-infected necrotic pulp tissue in the root canal system is not accessible to the host's innate and adaptive immune defense mechanisms and antimicrobial agents. Therefore, the infected or non-infected necrotic pulp tissue must be removed from the canal space by pulpectomy. As our knowledge in pulp biology advances, the concept of treatment of pulpal and periapical diseas

Radicular groove is an anatomical malformation that often causes severe periodontal defects. Treatments of such an anomaly present a clinical challenge to the operator. Presented is a case of successful treatment of radicular groove associated with a maxillary lateral incisor of a 15-year-old girl. A combination of endodontics, intentional replantation, and Emdogain therapy was used. At 4-year follow-up, the patient was comfortable and complete resolution of the periapical pathology was evident.

To assess the prevalence of apical periodontitis (AP), alveolar bone loss (ABL), and the frequency of root-filled (RF) teeth in athletes, digital panoramic radiographs were obtained from 617 athletes (57.5% males; 42.5% females) with a mean age of 25.4 years (±2.8), evaluating 15.239 teeth. AP was classified using the Periapical Index (PAI). ABL was recorded when bone loss exceeded one-third of root length in more than two teeth. The RF teeth and periapical status were also recorded. Intra-examiner agreement was high (Kappa = 0.87). AP was found in 209 (33.9%) of athletes and 357 (2.3%) of tee

Apical periodontitis (AP) is featured by a persistent inflammatory response and alveolar bone resorption initiated by microorganisms, posing risks to both dental and systemic health. Nonsurgical endodontic treatment is the recommended treatment plan for AP with a high success rate, but in some cases, periapical lesions may persist despite standard endodontic treatment. Better comprehension of the AP inflammatory microenvironment can help develop adjunct therapies to improve the outcome of endodontic treatment. This review presents an overview of the immune landscape in AP, elucidating how micr

External replacement resorption (ERR) is an uncommon yet aggressive form of root resorption in which bone replaces the root following damage to the periodontal ligament. Early recognition is critical, as progression is typically silent and may ultimately result in tooth loss. This report describes a 25-year-old male patient who presented with a "sunken" lower right first molar teeth three years after biting a hard object. Clinical examination revealed infra-occlusion, a high-pitched metallic percussion tone, and delayed pulp responses. Periapical radiography and cone beam computed tomography d

BACKGROUND: Immature teeth with necrotic pulps present multiple challenges to clinicians. In such cases, regenerative endodontic procedures (REPs) may be a favorable strategy. Cells, biomaterial scaffolds, and signaling molecules are three key elements of REPs. Autologous human dental pulp cells (hDPCs) play an important role in pulp regeneration. In addition, autologous platelet concentrates (APCs) have recently been demonstrated as effective biomaterial scaffolds in regenerative dentistry, whereas the latest generation of APCs-concentrated growth factor (CGF), especially liquid phase CGF (LP
