Pulp Revascularization of Immature Dog Teeth with Apical Periodontitis Using Triantibiotic Paste and Platelet-rich Plasma: A Radiographic Study.

INTRODUCTION: This study evaluates radiographically the efficacy of 4 revascularization protocols in necrotic-infected immature dog teeth with apical periodontitis (AP). METHODS: Forty double-rooted immature premolar teeth from 4 female beagle dogs aged 5 months were used. Four teeth were left untouched as negative controls; the other 36 teeth were infected to develop pulp necrosis and AP following different treatment protocols. Four teeth were left untreated and assigned to the positive control group, and the last 28 teeth were randomly assigned into 4 experimental groups of 8 teeth: A1, sod

Regeneration and Repair in Endodontics-A Special Issue of the Regenerative Endodontics-A New Era in Clinical Endodontics.

Caries is the most common cause of pulp-periapical disease. When the pulp tissue involved in caries becomes irreversibly inflamed and progresses to necrosis, the treatment option is root canal therapy because the infected or non-infected necrotic pulp tissue in the root canal system is not accessible to the host's innate and adaptive immune defense mechanisms and antimicrobial agents. Therefore, the infected or non-infected necrotic pulp tissue must be removed from the canal space by pulpectomy. As our knowledge in pulp biology advances, the concept of treatment of pulpal and periapical diseas

Extraoral root lengthening with titanium screws.

The aim of the present study was to experimentally evaluate marginal and periapical healing in extracted and subsequently replanted teeth following extraoral cementing of apically exposed titanium root screws in the root canal. In 5 out of 6 replanted teeth, the periodontal tissues had healed with a normal periodontal membrane or healed surface resorptions indicating that only minor trauma had been inflicted during the extraoral procedure. In 3 out of 6 teeth, total or partial osseointegration of the titanium screws was seen, while a connective tissue capsule had formed around the remaining 3

Microsurgical management of radicular cyst using guided tissue regeneration technique: A case report.

BACKGROUND: Radicular cyst is a lesion of odontogenic origin that arises from epithelial remains due to periapical periodontitis caused by inflammatory reactions generated at the apex of affected teeth with infected or necrotic pulps. The therapeutic management of radicular cysts is controversial. There is only one case report of enucleation of a radicular cyst managed with microsurgery and apicoectomy, but without the use of the guided tissue regeneration (GTR) technique in the same surgical procedure. The present clinical case describes the management of a radicular cyst with microsurgical a

Microbial control and a simple regenerative endodontic protocol achieve durable apical healing and long-term root development: A 10-year case report.

Regenerative endodontics offers a biologically driven alternative to apexification for immature teeth with necrotic pulps and apical periodontitis. We report the management of an 11-year-old girl with a history of dentoalveolar trauma and an immature maxillary left central incisor (tooth 21) presenting with gray discoloration, negative sensibility, and a defined apical radiolucency (Nolla stage 9). Treatment followed a conservative, accessible protocol: disinfection with 5.25% sodium hypochlorite, placement of triple-antibiotic paste (ciprofloxacin, metronidazole, amoxicillin), and, at two wee

Macrophage polarization differs between apical granulomas, radicular cysts, and dentigerous cysts.

OBJECTIVES: Apical periodontitis can appear clinically as apical granulomas or radicular cysts. There is evidence that immunologic factors are involved in the pathogenesis of both pathologies. In contrast to radicular cysts, the dentigerous cysts have a developmental origin. Macrophage polarization (M1 vs M2) is a main regulator of tissue homeostasis and differentiation. There are no studies comparing macrophage polarization in apical granulomas, radicular cysts, and dentigerous cysts. MATERIALS AND METHODS: Forty-one apical granulomas, 23 radicular cysts, and 23 dentigerous cysts were analyz

Implant placement in fresh extraction sockets.

Recent systematic reviews have shown that the survival rate of immediate implant placement is similar to those with a delayed approach. However, preclinical models and human studies have shown that immediate implant placement per se does not preserve the anatomy of the alveolus, mainly at the buccal bone crest, leading to bony dehiscences and subsequently to soft-tissue recession, with a great impact on esthetic outcomes. On the other hand, preclinical and human studies have identified factors that may prevent bone resorption after immediate implant placement, such as anatomical/biological (al

Cemento-osseous Dysplasia Mimicking Periapical Cyst After the Orthodontic Treatment of the Mandible.

Cemento-osseous dysplasia (COD) is a non-neoplastic condition, characterized by the replacement of normal cancellous bone with fibrous tissues containing woven bone and cementum in the tooth-bearing areas. This report presents the case of a 21-year-old male who developed COD following orthodontic treatment. A panoramic radiograph showed a periapical radiolucent lesion in the right mandibular canine area. Cone beam computed tomography revealed a mixed radiolucent and radiopaque lesion. Histologically, the lesion exhibited cementum-like calcifications and woven bone scattered within the hemorrha

Human periapical cyst-mesenchymal stem cells differentiate into neuronal cells.

It was recently reported that human periapical cysts (hPCys), a commonly occurring odontogenic cystic lesion of inflammatory origin, contain mesenchymal stem cells (MSCs) with the capacity for self-renewal and multilineage differentiation. In this study, periapical inflammatory cysts were compared with dental pulp to determine whether this tissue may be an alternative accessible tissue source of MSCs that retain the potential for neurogenic differentiation. Flow cytometry and immunofluorescence analysis indicated that hPCy-MSCs and dental pulp stem cells spontaneously expressed the neuron-spec

Immunoexpression of interleukin 17 in apical periodontitis lesions.

INTRODUCTION: Interleukin (IL)-17 expression has been detected in apical periodontitis lesions, but its role in the disease process remains unclear. The present study compared the expression of IL-17 in periradicular cysts and granulomas and evaluated the association of this cytokine with clinical and radiographic findings. METHODS: Apical periodontitis lesions (18 cysts and 20 granulomas) were obtained from 38 patients subjected to periradicular surgery. Some clinical, radiographic, and cone-beam computed tomographic features were recorded. Silanized slides containing paraffin sections were

Treatment of internal resorption with mineral trioxide aggregates: a case report.

Tooth resorption is a common sequel which follows injuries or irritation to the periodontal ligament and/or tooth pulp. The course of tooth resorption involves an elaborate interaction among inflammatory cells, resorbing cells, and hard tissue structures. The key cells which are involved in resorption are multi-nucleated giant cells. Internal root resorptions are usually non-symptomatic and they are discovered occasionally through periapical radiographs, which reveal very defined and regular outlines. Many techniques and materials have been used to fill internal resorptive defects. Among them,

Same Appointment Pre-Endodontic Dentin Sealing and Coronal Reconstruction Technique for Indirect Restorations: A Proof of Concept.

Teeth requiring endodontic treatment often present with loss of dental tissue due to caries and/or trauma. This loss compromises their strength and can lead to tooth fractures. The longer these teeth remain exposed and unprotected in the oral cavity, the greater the risk of contamination. Therefore, coronal reconstruction of structurally compromised teeth should be considered to reduce the risk of failure and ensure adequate endodontic intervention in cases of pulpal and/or periapical disease. This article presents a novel technique in which coronal reconstruction was performed prior to endodo

Mucosal antral cysts observed within a London inner-city population.

Mucosal antral cysts(MACs) appear as dome-shaped soft-tissue opacities and have prevalences of between 1.6% and 8.7% on otherwise normal panoramic radiographs. The aim of this survey was to define the prevalence in the inner-city population in London. A 14% prevalence for MACs was observed in the panoramic radiographs of 1000 consecutive patients attending King's College Hospital dental casualty department sited in inner London, which is higher than those reported for other populations. MACs were not significantly associated with the presence of signs and symptoms of periapical areas of bone l

Single-cell RNA sequencing combined with proteomics of infected macrophages reveals prothymosin-α as a target for treatment of apical periodontitis.

INTRODUCTION: Chronic apical periodontitis (CAP) is a common infectious disease of the oral cavity. Immune responses and osteoclastogenesis of monocytes/macrophages play a crucial role in CAP progression, and this study want to clarify role of monocytes/macrophages in CAP, which will contribute to treatment of CAP. OBJECTIVES: We aim to explore the heterogeneity of monocyte populations in periapical lesion of CAP tissues and healthy control (HC) periodontal tissues by single-cell RNA sequencing (scRNA-seq), search novel targets for alleviating CAP, and further validate it by proteomics and in

Periapical Cemento-osseous Dysplasia Is Rarely Diagnosed on Orthopantomograms of Patients with Neurofibromatosis Type 1 and Is Not a Gender-specific Feature of the Disease.

UNLABELLED: Several skeletal aberrations of the skull have been described for the tumor predisposition syndrome neurofibromatosis type 1 (NF1). Recently, periapical cemental/cemento-osseous dysplasia (COD) has been described in females affected with NF1. This reactive lesion of the hard tissues in tooth-bearing areas of the jaw has been proposed to represent a gender-specific radiological feature of NF1. The aim of this study was to investigate the prevalence of COD in patients with NF1. PATIENTS AND METHODS: The orthopantomograms (OPGs) of 179 patients with a confirmed diagnosis of NF1 were

Histologic and histobacteriologic observations of failed revascularization/revitalization therapy: a case report.

INTRODUCTION: Mechanical debridement plays an important role in eliminating intracanal bacteria, such as biofilm on the canal walls and bacteria in the dentinal tubules. Mechanical debridement is not recommended for root canal disinfection in revascularization/revitalization therapy. Here we report a failed revascularization/revitalization case, which could be due to inadequate root canal disinfection without mechanical removal of biofilm and bacteria in dentinal tubules. METHODS: A 6-year-old boy had a traumatic injury to tooth #9, which was avulsed and replanted within 40 minutes. The tooth
