Genetics of acheiropodia (the handless and footless families of Brazil). IX. Genetic counseling.

Acheiropodia offers no special problem of a counseling nature. The genetic risk can be easily estimated since the anomaly is due to a rare autosomal recessive gene with complete penetrance and little variation in expression. However, considering that acheiropod individuals have a tremendous handicap (they are born without both hands and feet), it is remarkable how they overcome this difficulty and lead an almost 'normal' life. Several examples are given in this paper. The 'philosophy' of genetic counseling is briefly discussed; emphasis is placed upon the necessity for the geneticist to presen…

"My Whole World Fell Apart": Parents Discovering Their Child Has Anorexia Nervosa.

This was the first study to examine the experience of parents who discover their child was living with anorexia nervosa (AN), thus fulfilling a critical gap in the eating disorder literature. Gadamerian hermeneutic inquiry was the guiding philosophy and method used to investigate this topic. Dialogues with parents revealed the ambiguity inherent within discovery; the isolation, betrayal, and loss felt by parents; and the complicated family dynamics occurring during the process of discovering one's child has AN. As such, when discoveries are made, parents play a vital role in the development an…

The management of ballistic trauma: an infection control perspective.

This article discusses the relationship between ballistic trauma, clostridial contamination and potential wound infection and considers the implications for care by nurses and infection control teams. An overview of ballistics is presented followed by an examination of the pathophysiology of wounding and its effects. The philosophy of war surgery is balanced against civilian concepts and the differing management strategies that prevail. It explores the causes of, and relationship between, wound contamination and the seriousness of wound infection. Nurses, inexperienced in dealing and caring fo…

[Pre-septal orbital cellulitis from odentogenic origin--combined surgical and endodontic approach: a case report].

Orbital abscesses are common infectious diseases. The etiology of orbital abscesses may vary from common sinusitis in most of the cases, to cocaine sniffing. As a result of the proximity to the brain, orbital abscesses may complicate to life threatening situations. The infectious process spreads to the orbit in several ways: hematogenous, via anatomic spaces in the maxillofacial region, direct invasion. The treatment philosophy combines surgical and Antibiotic therapy to resolve the acute phase of the disease, followed by elimination of the source. The purpose of this paper is to report a case…

Learning disabilities and mental ill health: care plans.

Comprehensive multidisciplinary assessment and treatment are particularly important for clients who present with learning disabilities and mental health problems (O'Hara and Sperlinger, 1997). This should not be restricted to professionals who work in services for people with learning disabilities and should include a wider network of professionals from statutory services and the independent sector. This arrangement can be complex as different agencies and even different departments of the same agency may differ fundamentally in their philosophy, vision and understanding of their own and other…

Fieldwork education and students with disabilities: enter the Americans With Disabilities Act.

For occupational therapy students with disabilities, making the transition from the classroom to clinical fieldwork presents an interesting challenge to clinical fieldwork supervisors and academic fieldwork advisors. The Americans With Disabilities Act (ADA) and other laws have changed the rules by requiring fieldwork sites and academic programs to make accommodations for students with disabilities. Occupational therapy's basic philosophy combined with the occupational therapist's training in task analysis, functional limitations of disabilities, modification of the environment and clinical re…

I'm shocked: informed consent in ECT and the phenomenological-self.

This paper argues that phenomenological insights regarding selfhood are relevant to the informed consent process in the treatment of depression using electro-convulsive therapy (ECT). One of the most significant side-effects associated with ECT is retrograde amnesia. Unfortunately, the current informed consent model does not adequately appreciate the full extent in which memory loss disturbs lived-experience. Through the philosophy of Merleau-Ponty, it is possible to appreciate the way in which memory loss affects a person's self-experience, with emphasis given to one's pre-reflective and embo…

Achieving the "natural look" in rhytidectomy.

Achieving the "natural look" in rhytidectomy and satisfying my aging face patient population are the primary motivating factors that have guided me to develop my personal philosophy and technique for facelifting. The following article will consider how to achieve the "natural look" in rhytidectomy using the modified deep plane-extended SMAS rhytidectomy technique, including the benefits derived from this efficient face-lift technique. It outlines how to give proper individualized consideration to each patient from the initial consultation, through the preoperative evaluation and workup, as wel…

Universalism and individuality as a structural tension in modern medicine.

This paper examines the structural tension between universalism and individuality in modern medicine. By universalism, I refer to the epistemological commitment to producing general, abstract knowledge about health and disease through classifications, generalizable models of physiological process, statistical evidence, and standardized therapeutic protocols. By individuality, I refer to the biological uniqueness of each patient as a concrete organism possessing a singular configuration of traits and circumstances. The central argument is that modern medicine has historically depended on univer

Striking the balance with epistemic injustice in healthcare: the case of Chronic Fatigue Syndrome/Myalgic Encephalomyelitis.

Miranda Fricker's influential concept of epistemic injustice (Oxford University Press, Oxford, 2007) has recently seen application to many areas of interest, with an increasing body of healthcare research using the concept of epistemic injustice in order to develop both general frameworks and accounts of specific medical conditions and patient groups. This paper illuminates tensions that arise between taking steps to protect against committing epistemic injustice in healthcare, and taking steps to understand the complexity of one's predicament and treat it accordingly. Work on epistemic injust

Ferments and the AIDS virus: interspecies counter-conduct in the history of AIDS.

In the first three decades after AIDS started infecting people in the USA and Canada, before, during and after the emergence of anti-retroviral therapies, numerous "alternative and holistic treatments" for AIDS were debated, tested, circulated, written about and taught. This paper, taking a narrow focus, examines documents that reveal how some people with AIDS developed a logic of care predicated on intimate interactions with microscopic lifeforms-the AIDS virus and the bacteria involved in fermentation, in particular. Focusing on the writings of Jon Greenberg and Sandor Katz, two former membe

René Girard and the Mimetic Nature of Eating Disorders.

French historian and literary critic René Girard (1923-2015), most widely known for the concepts of mimetic desire and scapegoating, also engaged in the discussion of the surge of eating disorders in his 1996 essay Eating Disorders and Mimetic Desire. This article explores Girard's ideas on the mimetic nature and origin of eating disorders from a clinical psychiatric perspective and contextualizes them within the field of eating disorders research as well as in relation to broader psychological, sociological and anthropological models of social comparison and non-consumption. Three main themes

Suicide in ancient Greece.

The theme of suicide appears several times in ancient Greek literature. However, each such reference acquires special significance depending … tions of the gods, the relation among gods and men and the relation among the men. In ancient Greek philosophy there were the deniers of suicide, who were more concerned about the impact of suicide on society and also these who accepted it, recognizing the right of the individual to put an end to his life, in order to avoid personal misfortunes. Real suicide …

[The pineal gland as physical tool of the soul faculties: a persistent historical connection].

INTRODUCTION: The anatomic seat of the human soul has been a controversial matter of discussion in the philosophical, theological and scientific fields throughout history. One of more known hypotheses on this subject was proposed by Descartes, for whom the soul would host in the pineal gland, a brain body with a special location that would adequately address the functionalism of the human body. DEVELOPMENT: In this work, we discuss the historical influences which made possible the Cartesian model of the relationship between spirit (res cogitans) and body-machine (res extensa) and the technica

Critical inquiry and knowledge translation: exploring compatibilities and tensions.

Knowledge translation has been widely taken up as an innovative process to facilitate the uptake of research-derived knowledge into health care services. Drawing on a recent research project, we engage in a philosophic examination of how knowledge translation might serve as vehicle for the transfer of critically oriented knowledge regarding social justice, health inequities, and cultural safety into clinical practice. Through an explication of what might be considered disparate traditions (those of critical inquiry and knowledge translation), we identify compatibilities and discrepancies both

Richard Bradley: a unified, living agent theory of the cause of infectious diseases of plants, animals, and humans in the first decades of the 18th century.

During the years 1714 to 1721, Richard Bradley, who was later to become the first Professor of Botany at Cambridge University, proposed a unified, unique, living agent theory of the cause of infectious diseases of plants and animals and the plague of humans. Bradley's agents included microscopic organisms, revealed by the studies of Robert Hooke and Antony van Leeuwenhoek. His theory derived from his experimental studies of plants and their diseases and from microscopic observation of animalcules in different naturally occurring and artificial environments. He concluded that there was a micros

Coming home to Hume: a sociobiological foundation for a concept of 'health' and morality.

Assessing the normative status of concepts of health and disease involves one in questions regarding the relationship between fact and value. Some have argued that Christopher Boorse's conception of health and disease lacks such a valuational element because it cannot account for types of harms which, while disvalued, do not have evolutionarily dysfunctional consequences. I take Boorse's account and incorporate some Humean-like sociobiological assumptions in order to respond to this challenge. The possession of moral sentiments, I argue, offers an evolutionary advantage (thus falling within Bo

Liberatory pedagogy and special education.

Liberatory theory, related to critical theory, postmodern thought, and constructivism, forms the backdrop of this article, which suggests that the way in which we have conceptualized students with disabilities as the "other" has come from a distorted modernist view that encourages us to distance ourselves from one another. It is suggested that some of our most vehement debates in special education are indicative of our attempts to "nibble at the edges of postmodern thinking." Liberatory pedagogies developed throughout the world have used education as a means for oppressed peoples to define for
