We treated five patients with hemispheric ischemic stroke with intravenous recombinant tissue plasminogen activator (rtPA), within 3-6 h after stroke onset. Regional cerebral blood flow was evaluated with single photon emission computed tomography (rCBF-SPECT) before and after treatment. One patient with aphasia and a moderately severe hemiparesis, who had a small flow deficit, was treated 5 h and 30 min after the onset of his stroke and had a prompt and complete recovery. The post treatment rCBF-SPECT showed normal flow. One patient with a very large flow deficit died of transtentorial hernia

Thrombolytic agents may be useful in the treatment of cerebral ischemia caused by arterial thrombosis or embolic occlusion. A trial of intravenous human tissue-type plasminogen activator (rt-PA) was carried out in seven male Sprague-Dawley rats subjected to embolic cerebral ischemia, with eight control animals. One-hour-old autogenous blood clot was injected into the internal carotid artery. A 30-minute infusion of 10 micrograms/kg/minute of rt-PA or saline followed. Areas of ischemia at two hours post-embolization were assessed by digital image processing of serial iodo-14C-antipyrine autorad

The inhibitory effect of a placental urokinase inhibitor on the one- and two-chain form of melanoma cell derived plasminogen activator (PA) was investigated. The melanoma cell PA has been shown to be similar to or identical with the PA found in normal tissue. With constant concentration of placental inhibitor the rate of inhibition of the two-chain form was fast in contrast to that of the one-chain form. With constant incubation time but increasing concentrations of the placental inhibitor the two-chain form was inactivated to a greater extent at lower concentrations of placental inhibitor tha

PURPOSE: Plasminogen activator inhibitor (PAI) -1 is a key regulator of endogenous fibrinolysis and is widely acknowledged as a its primary inhibitor. Beyond this function, PAI-1 also plays a role in macrophage migration during inflammatory conditions. This study aims to investigate the contribution of PAI-1 to the progression of uveitis.
METHODS: We quantified PAI-1 protein levels in vitreous samples collected from uveitis patients utilizing a magnetic bead-based multiplex immunoassay. To clarify the changes in PAI-1 expression in uveitis, experimental autoimmune uveoretinitis (EAU) was indu

BACKGROUND: Plasminogen activator inhibitor-1 (PAI-1) plays a key role in modulating fibrinolysis. However, its role in the thrombotic autoimmune disease known as antiphospholipid syndrome (APS) remains understudied.
METHODS: We characterized PAI-1 expression by APS endothelial cells through single-cell RNA sequencing (scRNA-seq) of skin biopsies from APS patients with active livedo racemosa and age-matched controls. Total and active PAI-1 were quantified in plasma samples from 167 patients with APS, 37 antiphospholipid antibody-positive carriers without clinical APS, 18 patients with a histo

Urokinase (uPA) and urokinase plasminogen activator receptor (uPAR/PLAUR) mediate fibrinolysis and matrix remodeling. Liberation of monocyte/macrophage (MO/Mφ)-bound soluble uPAR (suPAR) occurs after infection with coronaviruses (CoVs) such as severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), correlating with systemic inflammatory stress and end-organ injury severity. How suPAR liberation from MO/Mφs occurs after CoV infection, and whether suPAR induction is exclusive to coronavirus infection or occurs from other clinically significant coagulopathy-inducing pneumotropic viral infec

BACKGROUND: Intrapleural tissue plasminogen activator (tPA) and DNase are used when drainage of loculated pleural effusions is inadequate, but the real-world performance of pragmatic once-daily protocols remains uncertain.
METHODS: We performed a retrospective cohort study across seven hospitals between January 1, 2019, and December 31, 2020. Adults who received intrapleural tPA 20 or 50 mg once daily with optional DNase 5 mg once daily for complicated pleural effusion, empyema, or hemothorax were included. The primary outcome was treatment success, defined as survival to hospital discharge w

BACKGROUND: During invasive migration, cancer cells use secreted proteases to remove various extracellular matrix components. The urokinase plasminogen activator (uPA) and its receptor (uPAR), together with kallikreins, perform key functions in this proteolytic network through control of various proteases. Plasma kallikrein (PKa) is an important activator of pro-uPA on the cell surface.
OBJECTIVES: To examine the role of plasma kallikrein in malignancy, with a particular focus on breast cancer.
METHODS: The MCF-7 (non-metastatic) and MDA-MB-231 (metastatic) breast cancer cell lines were anal

Acute kidney injury (AKI) frequently follows hematopoietic cell transplantation (HCT). Soluble urokinase-type plasminogen activator receptor (suPAR) is a biomarker of AKI in the general population. We evaluated suPAR and its association with AKI requiring dialysis (AKI-D) in patients undergoing allogeneic HCT (alloHCT). Performance of suPAR was compared with serum creatinine (sCr) and neutrophil gelatinase-associated lipocalin (NGAL). Data were obtained from the Blood and Marrow Transplant Clinical Trials Network 1202 cohort, an observational study of 1709 alloHCT recipients. Adults aged ≥18 y

Plasminogen activator inhibitor 1 (PAI-1), encoded by SERPINE1, contributes to age-related cardiovascular disease (CVD) and other aging-related pathologies. Humans with a heterozygous loss-of-function SERPINE1 variant exhibit protection against aging and cardiometabolic dysfunction. We engineered a mouse model mimicking the human mutation (Serpine1TA700/+) and compared cardiovascular responses with WT littermates. Serpine1TA700/+ mice lived 17% longer than did littermate control mice. Under l-NG-nitro-arginine methyl ester-induced (l-NAME-induced) vascular stress, Serpine1TA700/+ mice exhibite

Aging-related sarcopenia and osteoporosis are musculoskeletal disorders characterized by accelerated muscle and bone loss. Plasminogen activator inhibitor-1 (PAI-1), a fibrinolysis inhibitor, is involved in various pathological conditions, including sarcopenia and osteoporosis; however, its roles in aging-related sarcopenia and osteoporosis have yet to be fully investigated. Therefore, we investigated the roles of PAI-1 in aging-related sarcopenia and osteoporosis using PAI-1-gene-deficient and wild-type mice. Aging-related changes in muscle and bone were assessed by comparing the values in 24

BACKGROUND: Submacular hemorrhage (SMH) is a sight-threatening disorder. Choroidal neovascularization secondary to age-related macular degeneration, polypoidal choroidal vasculopathy, trauma, angioid streaks, and pathological myopia are a few important causes. The conventional treatment of massive SMH is vitrectomy with manual removal of the clot with extensive retinectomy with/without tissue plasminogen activator (tPA). The usual dose of subretinal tPA is 10-25 µg.
PURPOSE: To describe a new surgical approach in a case of massive SMH with retinal detachment without retinectomy.

BACKGROUND: Cardiovascular disease (CVD) is an important cause of morbidity and mortality in peritoneal dialysis (PD) patients. Cardiovascular calcification (CVC) is highly prevalent in PD patients and could predict their cardiovascular mortality. Soluble urokinase plasminogen activator receptor (suPAR) is closely associated with coronary artery calcification in hemodialysis patients and is an important predictor of CVD. However, the role of suPAR in PD patients is poorly understood. We investigated the relationship between serum suPAR and CVC in PD patients.
METHODS: Abdominal aortic calcifi

Acute pulmonary embolism (APE) is a debilitating condition with high incidence and mortality rates. APE is widely treated with the serine protease urokinase or urokinase-type plasminogen activator (uPA) that functions by resolving blood clots via catalyzing the conversion of plasminogen to plasmin. Treatment with recombinant uPA has been shown to increase endogenous expression of uPA and its cognate receptor, uPAR; however, the mechanisms for this induction are not known. Using an in vitro hypoxia/reoxygenation model in bronchial epithelial BEAS-2B cells, we show that induction of hypoxia/reox
