Complications in Eyelid Surgery.

Eyelid surgery consists of challenging reconstructive and cosmetic procedures. Because of the complex anatomy and corresponding vital functions of the upper and lower eyelids, the avoidance of eyelid complications is of vital importance. Complications after eyelid surgery include basic complications (infection, granuloma) and vision-threatening complications. Preoperative history, physical examination, surgical planning, and meticulous surgical technique must be undertaken to prevent complications after eyelid surgery. In addition, patient knowledge, expectations, and motivations must be deter

Regenerative Medicine in Facial Plastic and Reconstructive Surgery: A Review.

The field of regenerative medicine aims at enhancing tissue healing and regeneration through the exogenous addition of therapeutic growth factors and cells, often in combination with tissue-compatible scaffolds. Perhaps the biggest advances in facial plastic and reconstructive surgery (FPRS) in the coming years will be the result of regenerative medicine techniques. While many articles on regenerative medicine have been published in the FPRS literature, to our knowledge there are no reviews that describe both soft-tissue and bony regeneration strategies, including scaffolds, stem cells, growth

Cleft Lip Repair: Through the Looking Glass.

The goal of cleft lip repair is an anatomic re-creation of the normal lip elements with no vertical discrepancy between the affected and normal sides of the central philtral element. Frequently, the repaired lip appears short in the vertical dimension despite careful preoperative measurements and intraoperative maneuvers. Some may be due to postoperative scar contraction, but some may be due to a failure to consider the internal surface when reconstructing the lip. Much literature has been dedicated to marking and repairing the skin and muscle of the lip; however, scant consideration has been

[Auricular malformations].

Reconstruction of complex auricular malformations is one of the longest surgical technique to master, because it requires an extremely detailed analysis of the anomaly and of the skin potential, as well as a to learn how to carve a complex 3D structure in costal cartilage. Small anomalies can be taken care of by any plastic surgeon, providing that he/she is aware of all the refinements of ear surgery. In this chapter, we analyze retrospectively 30 years of auricular reconstruction, ranging from small anomalies to microtia (2500 cases), excluding aesthetics variants such as prominent ears.

Anatomic Reconstruction Technique for a Plantar Calcaneonavicular (Spring) Ligament Tear.

Acquired flatfoot deformity in adults is usually due to partial or complete tearing of the posterior tibial tendon, with secondary failure of other structures such as the plantar calcaneonavicular (spring) ligament (SL), which maintain the medial longitudinal arch. In flexible cases, the tibialis posterior can be replaced with the flexor digitorum longus. It is common practice to suture the SL directly in the case of a tear; however, if the tear is complete, suturing directly to the ligament alone will not be possible. Reconstruction of the ligament is needed; however, no validated methods are

Contemporary facial reanimation.

The facial nerve is the most commonly paralyzed nerve in the human body. Facial paralysis affects aesthetic appearance, and it has a profound effect on function and quality of life. Management of patients with facial paralysis requires a multidisciplinary approach, including otolaryngologists, plastic surgeons, ophthalmologists, and physical therapists. Regardless of etiology, patients with facial paralysis should be evaluated systematically, with initial efforts focused upon establishing proper diagnosis. Management should proceed with attention to facial zones, including the brow and periocu

[Failures of hallux valgus surgery].

More than 150 corrective procedures for hallux valgus exist and an incorrect choice of procedure leads to insufficient correction. Distal first metatarsal osteotomy cannot correct large deformities and degenerative changes at the metatarsophalangeal joint impede functional recovery. Incongruence of the joint must be corrected during surgery. Recurrence is most often caused by insufficient correction, especially of the first metatarsal bone. Overcorrection is often due to technical problems with the initial metatarsal osteotomy. This also applies to insufficiency of the first ray due to shorten

[The Omega "Omega" pulley plasty: a new technique for the surgical management of the De Quervain's disease].

The Omega "Omega" pulley plasty: a new technique for the surgical management of the De Quervain's disease. The De Quervain tenosynovitis is an inadequacy into the first extensor compartment between the osteo-fibrous tunnel and the tendons. This mechanical conflict generates a tenosynovitis of the extensor pollicis brevis and the abductor pollicis longus tendons. This is generally expressed by a tenderness on the radial side of the wrist over the radial styloid process. The medical management consists on corticoids infiltrations of the first extensor compartment, the avoidance of repetitive and

The use of micrografts and minigrafts in the aesthetic reconstruction of the face and scalp.

Hair transplantation by use of micrografts (one- to two-hair follicular unit grafts) and minigrafts (three- to four-hair follicular unit grafts) used in large numbers (>1000 grafts) in a single session was initially described for the treatment of male pattern baldness. More recently, the author has found many other applications, particularly in facial and scalp reconstruction. The most common causes for aesthetic hair restoration of those areas in the author's experience include hair loss resulting from aesthetic facial rejuvenation surgery, revision of unsatisfactory results from previous hai

Surgical advances. A comprehensive approach to surgical rejuvenation of the eyes.

Methods to rejuvenate the eye region include shortening of the forehead, raising the brow, improving the upper eyelids by excision of blepharochalasis tissue, and reducing lower eyelid fat pads by a transconjunctival approach and at the same time elevating the stretched lateral canthus by an effective canthopexy. The risks and complications of these procedures are minimal, circumscribing the shortcomings of conventional blepharoplasty.
