Pneumococcal vaccine improves pulmonary clearance of live pneumococci after splenectomy.

The efficacy of pneumococcal capsular polysaccharide vaccines after splenectomy to decrease the incidence of postsplenectomy pneumococcal sepsis is controversial. We examined the effect of pneumococcal vaccine on clearance of live pneumococci from lungs of splenectomized and sham-operated mice following an aerosol challenge of pneumococci. Splenectomy impaired clearance of pneumococci from mouse lungs and allowed for increased translocation of pneumococci to tracheobronchial lymph nodes compared to shams (P less than 0.01). Pneumococcal vaccine improved lung clearance in both splenectomized an

Persistence of protective pneumococcal antibody following vaccination in patients with the nephrotic syndrome.

We have determined the level of persisting pneumococcal antibody in a group of nephrotic children vaccinated by us 5 years ago. Of the 19 vaccinated children, 2 have died and 1 has moved away. Sera from the remaining 16 patients were examined by radioimmunoassay to determine the antibody response to 11 of the 14 types contained in the polyvalent pneumococcal vaccine. The lowest protective level of geometric mean titre (GMT) of antibody in our laboratory is 300 ng antibody nitrogen per millilitre. 56% (9/16) of the patients showed adequate GMT 5 years after vaccination. All 9 patients had minim

Immunization of patients with Sjögren's syndrome with pneumococcal polysaccharide vaccine: a randomized trial.

Thirty-two patients with Sjögren's syndrome were randomly selected to receive either multivalent pneumococcal polysaccharide vaccine or saline placebo in a double-blind study. Among placebo treated patients, levels of antipneumococcal antibodies remained constant. Immunized patients developed significant elevations of antibody concentrations against all 12 capsular serotypes measured. Levels fell, but persisted above baseline, for 8 of the serotypes 6 months after immunization. During the 6-month study no clinical or serologic evidence of disease exacerbation occurred in either group. We concl

Optimizing adult pneumococcal vaccination in India: Evidence from serotype surveillance.

INTRODUCTION: Pneumococcal disease, caused by Streptococcus pneumoniae , is a major public health burden with high morbidity and mortality across all age groups in India. Limited adult pneumococcal vaccination policies, evolving serotype patterns, and delayed herd protection from paediatric immunization continue to challenge effective prevention of pneumococcal disease in adults. METHODS: This retrospective laboratory surveillance study includes Streptococcus pneumoniae isolates from adults (>18 years) collected between January 2018 and May 2025 at Christian Medical College, Vellore and were

Systems analysis of immunity to pneumococcal vaccination in preterm and full term infants.

BACKGROUND: Preterm infants mount lower humoral responses to the 13-valent pneumococcal conjugate vaccine (PCV13) than full-term infants; however, the long-term impact of prematurity on vaccine-induced protection remains unclear. This study investigates the innate and adaptive immune responses to a 3+1 PCV13 schedule in preterm versus full-term infants using transcriptomic profiling and B-cell immunophenotyping. METHODS: Thirty-eight infants (19 preterm,19 full-term) received PCV13 at 2,4,6, and 12 months. Polysaccharide (PS)1- and PS9V-specific memory B cells (MBCs) were enumerated and pheno

Impact of Pneumococcal Vaccination on the Occurrence of Complicated Pneumonia in Children: A Retrospective Analysis.

Complicated community-acquired pneumonia (cCAP) remains a major cause of morbidity in children. Although pneumococcal conjugate vaccines (PCVs) have reduced invasive disease, severe complications such as empyema and lung abscess persist. A retrospective analysis was conducted on 69 children treated at the University Children's Hospital Belgrade between 2019 and 2024. Data included demographic characteristics, pneumococcal vaccination status, and radiologically confirmed complications. Patients were classified by residence and vaccination status. Statistical analysis included chi-square (χ2) te

Pneumococcal vaccination status in critically ill patients: A cross-sectional study.

Critically ill adults are often older and have multiple chronic conditions, but data on prior pneumococcal vaccination in this population are limited. We aimed to assess pneumococcal vaccination coverage among eligible patients and identify factors associated with pneumococcal vaccination among ICU patients. We performed a cross-sectional study in an ICU at a tertiary care hospital in Japan. We included patients admitted to the ICU from May 15, 2023, to January 4, 2024. Eligibility for pneumococcal vaccination was defined with reference to the Japanese routine vaccination program during the st

Recommendations for pneumococcal vaccination of at-risk children: a global overview (2022-2024).

BACKGROUND: Pneumococcal diseases have a major impact on childhood morbidity and mortality across the world. While any child could be infected, those with certain health conditions have an increased risk of infection and subsequent disease severity. This report provides an overview of pneumococcal vaccination policies focused on children considered to be at particular risk of pneumococcal disease. METHODS: Risk-group targeted vaccination schedules were retrieved from the World Health Organization and the European Centre for Disease Prevention and Control portals, and from countries' official

Cost-effectiveness of 20-valent pneumococcal conjugate vaccine in US infants.

BACKGROUND: As of June 2023, two pneumococcal conjugate vaccines, 20- (PCV20) and 15- (PCV15) valent formulations, are recommended for US infants under a 3 + 1 schedule. This study evaluated the health and economic impact of vaccinating US infants with a new expanded valency PCV20 formulation. METHODS: A population-based, multi cohort, decision-analytic Markov model was developed to estimate the public health impact and cost-effectiveness of PCV20 from both societal and healthcare system perspectives over 10 years. Epidemiological data were based on published studies and unpublished Active Ba

Pneumococcal vaccination status among cirrhotic patients in Italy: a neglected topic.

To date, few reports have evaluated the pneumococcal vaccination status in cirrhotic patients. No data are available for European countries. We have explored this topic and the potential independent predictors motivating lack of vaccination in Italy. Between January 1st and June 30th 2022, 1419 cirrhotic patients of any etiology were consecutively enrolled in an observational, prospective study at 8 referral centers in Italy. Adjusted odds ratios (ORs) for the association with lack of vaccination were evaluated by multiple logistic regression analysis. Overall vaccine coverage was 17.9% (8.9%

Determinants of influenza and pneumococcal vaccine uptake among preschool children in Singapore.

Young children are at increased risk of severe illness from influenza and pneumococcal infections. The World Health Organization (WHO) recommends vaccination with influenza and pneumococcal conjugate vaccine (PCV). However, in Singapore, vaccine uptake remains suboptimal relative to other routine childhood immunisations. Limited information exists regarding determinants of influenza and pneumococcal vaccine uptake in children. We estimated vaccine uptake and investigated factors associated with influenza and pneumococcal vaccination status by age group using data from a cohort study on acute r

The impact of pneumococcal conjugate vaccine on the prevalence and severity of hospitalizations for pneumonia in children.

Pneumococcal conjugate vaccines (PCV) were introduced into the Israeli national immunization plan starting with the heptavalent PCV7 in 2009 and then PCV13 in the late 2010. The objective of this study was to determine the vaccines' impact on hospitalization rates for community-acquired pneumonia on the severity of the pneumonia episodes and upon pneumococcal serotype distribution. We retrospectively reviewed all children hospitalized in our institution with pneumonia, aged between 1 and 16 years, between the years 2006 and 2015. Demographic, clinical, and laboratory data between three time pe

Predictors of Influenza and Pneumococcal Vaccination Among Participants in the Women's Health Initiative.

OBJECTIVE: Older adults typically experience higher rates of severe disease and mortality than the general population after contracting an infectious disease. Vaccination is critical for preventing disease and severe downstream outcomes; however, vaccination rates among older adults are suboptimal. We assessed predictors associated with pneumococcal and seasonal influenza vaccination among older women. METHODS: We used data from the Women's Health Initiative, a nationwide cohort of women. We ascertained seasonal influenza and pneumococcal vaccination status through a questionnaire administere
