Effect of Sevoflurane, Propofol, and Remifentanil on Hemodynamic Response to Pneumoperitoneum and Laparoscopic Surgery: A Randomized Controlled Trial.

BACKGROUND: Although the interaction of sevoflurane, propofol, and opioids has been investigated regarding motor response to laryngoscopy, their hemodynamic impact during surgical stimulation remains unexplored. We therefore assessed the effect of sevoflurane, propofol, and remifentanil on the hemodynamic response to pneumoperitoneum and laparoscopic surgery. METHODS: The American Society of Anesthesiologists (ASA) physical status I or II patients scheduled for elective laparoscopic surgery were randomly assigned to one of six groups with different combinations of sevoflurane-remifentanil, pr…

Pneumoperitoneum without significant bowel perforation in patients with blunt trauma: a systematic review and meta-analysis.

BACKGROUND: Free intraperitoneal air (FIA) after blunt trauma is traditionally considered a radiological marker of hollow viscus perforation requiring urgent laparotomy. However, emerging reports have described pneumoperitoneum without surgically meaningful bowel injury, raising concerns about unnecessary operations. This systematic review and meta-analysis aimed to quantify the proportion of patients with blunt trauma with computed tomography (CT)-detected FIA who had no significant bowel perforation, defined as either (1) non-therapeutic laparotomy with no identified macroscopic perforation …

Pulmonary Embolism Associated With Pneumomediastinum and Pneumoperitoneum After Laparoscopic Myomectomy: A Case Report and Literature Review.

Laparoscopic surgery is a common technique with a minimal risk of consequences for the treatment of gynecological conditions, including uterine leiomyomas. However, in rare instances, serious postoperative complications may occur. We present the case of a 51-year-old patient who underwent a laparoscopic myomectomy without intraoperative complications. The patient experienced acute dyspnea shortly after the surgery, in the absence of other accompanying symptoms. Pneumoperitoneum, pneumomediastinum, and pulmonary embolism (PE) were found during radiological assessment, which included both thorac…

Beyond the bleed: bedside ultrasound reveals tension pneumoperitoneum in a patient with suspected acute variceal bleeding.

Coffee-ground emesis in a patient with cirrhosis frequently triggers an emergency pathway for suspected variceal upper gastrointestinal blee … ly tense, distended abdomen prompted point-of-care ultrasound (POCUS), which demonstrated extensive pneumoperitoneum. Emergency percutaneous decompression via a standard paracentesis needle evacuated free intraperitoneal air under tension with rapid respiratory improvement. Urgent oesophagogastroduodenoscopy showed no evidence of upper gastrointestinal perforation …

Point-of-Care Ultrasound for Early Detection of Pneumoperitoneum Following Percutaneous Gastrostomy and the Use of the Shifting Phenomenon.

Pneumoperitoneum, the presence of free air in the abdominal cavity, is a known complication of percutaneous gastrostomy tube placement. While often benign and self-limiting, persistent or symptomatic pneumoperitoneum may indicate underlying gastrostomy dysfunction or gastrointestinal perforation, requiring prompt recognition and intervention. We report the case of a 19-year-old patient admitted to the Intensive Care Unit after a high-impact trauma, who developed abdominal pain and distension following percutaneous gastrostomy placement. Bedside ultrasound revealed the presence of reverberation…

Quality of recovery after laparoscopic cholecystectomy: a randomized trial of pneumoperitoneum pressure and neuromuscular blockade depth.

INTRODUCTION: Laparoscopic Cholecystectomy (LC) is a commonly performed surgical procedure. The pneumoperitoneum and the depth of Neuromuscular Blockade (NMB) may impact the occurrence of postoperative pain and the quality of recovery. METHODS: A randomized, double-blind, and prospective clinical trial with 124 patients undergoing LC, divided into 4 groups: SP/MB (Standard Pneumoperitoneum pressure and Moderate NMB); LP/MB (Low Pneumoperitoneum pressure and Moderate NMB); SP/DB (Standard Pneumoperitoneum pressure and Deep NMB); and LP/DB (Low Pneumoperitoneum pressure and Deep NMB). Diagnosis and management challenges of spontaneous pneumoperitoneum associated with infectious pneumonia: case report and literature review.

INTRODUCTION AND IMPORTANCE: Spontaneous pneumoperitoneum is rare and can be treated without surgery. There are no clear criteria for determining which patient with pneumoperitoneum does not require surgery. We present a patient admitted to our centre for the management of infectious pneumonitis associated with spontaneous pneumoperitoneum. PRESENTATION OF CASE: A 76-year-old white male was admitted with dyspnoea, cough and diarrhoea for three days. The patient had heart rate of 117 beats per minute, oxygen saturation of 84 % and Glasgow scale of 13/15. Pulmonary embolism was suspected. The c…

The effect of pneumoperitoneum and Trendelenburg position on optic nerve sheath diameter in patients undergoing laparoscopic hysterectomy.

BACKGROUND AND AIM: Laparoscopic hysterectomy (LH) have been frequently used because of low complication rates and short duration of hospital stay. Elevated intracranial pressure (ICP), a disadvantage of laparoscopic surgery, is caused by the Trendelenburg position (TP) and CO2 pneumoperitoneum (PP). This study aimed to evaluate TP and PP associated changes in ICP by ONSD measurements during LH. The intra-and inter-observer consistency and reliability of ONSD measurements were also investigated. METHODS: Sixty patients with were enrolled into this prospective study. ONSD for each patient was …

The complexity analysis of cerebral oxygen saturation during pneumoperitoneum and Trendelenburg position: a retrospective cohort study.

BACKGROUND: The human brain is a highly complex and nonlinear system, nonlinear complexity measures such as approximate entropy (ApEn) and sample entropy (SampEn) can better reveal characteristics of brain dynamics. However, no studies report complexity of perioperative physiological signals to reveal how brain complexity associates with age, varies along with the development of surgery and postoperative neurological complications. AIM: This study examined the complexity of intraoperative regional cerebral oxygen saturation (rSO2), aiming to reveal brain dynamics during surgery. METHODS: Thi

Dynamic Tracheobronchial Compression in Steep Trendelenburg Position With Pneumoperitoneum for Robotic-Assisted Cystectomy: A Case Report.

Tracheobronchomalacia is a weakness of the trachea and bronchi due to abnormal cartilage and muscular support leading to airway obstruction. We report a case of an adult former smoker without pulmonary symptoms who underwent robotic-assisted laparoscopic cystectomy in the steep Trendelenburg position. After repeated episodes of hypoxemia, bronchoscopic examination revealed collapse of the distal trachea and bronchi, supporting a diagnosis of tracheobronchomalacia. Tracheomalacia is an underdiagnosed condition in patients with a smoking history and may mimic other obstructive diseases. The anes

Effect of Pneumoperitoneum and Lateral Position on Oropharyngeal Seal Pressures of Proseal LMA in Laparoscopic Urological Procedures.

INTRODUCTION: A sustained and effective oropharyngeal sealing with supraglottic airway is required to maintain the ventilation during laparoscopic surgery. Previous studies have observed the Oropharyngeal Seal Pressure (OSP) for Proseal Laryngeal Mask Airway (PLMA) after pneumoperitoneum in supine and trendelenburg position, where PLMA was found to be an effective airway device. This study was conducted with ProSeal LMA, for laparoscopic Urologic procedures done in lateral position. AIM: To measure OSP in supine and lateral position and to observe the effect of pneumoperitoneum in lateral pos…

Influence of lateral position and pneumoperitoneum on oropharyngeal leak pressure with two types of laryngeal mask airways.

BACKGROUND: An important parameter to monitor adequate ventilation for laryngeal mask airway (LMA) is its oropharyngeal leak pressure (OLP). This study was designed to evaluate and compare the effect of lateral position and pneumoperitoneum on the OLP and ventilation efficiency between LMA™ Proseal (PLMA) and LMA™ Supreme (SLMA). METHODS: Patients were randomized to receive either the PLMA or the SMLA. The OLP was assessed in both the supine position and the lateral position with or without pneumoperitoneum. Minute ventilation was increased to maintain normal EtCO2 as far as possible. Ventila…

Jejunal diverticulosis found in a patient with long-standing pneumoperitoneum and pseudo-obstruction on imaging: a case report.

Small bowel diverticulosis is a rare finding within the general population and jejunal diverticulosis, specifically, is even rarer. Clinical manifestations can range from post-prandial pain, constipation and malabsorption to serious complications, such as gastro-intestinal hemorrhage, perforation and acute intestinal obstruction. Here we describe the case of an 81-year-old gentleman who presented with a three-year history of abdominal pain and weight loss. Despite unremarkable physical examination and laboratory tests, persistent pneumoperitoneum and dilated loops of small bowel were found on …

An unusual case of abdominal distension: pneumoperitoneum secondary to pneumomediastinum in a patient with chronic obstructive pulmonary disease.

A 68-year-old lady with end-stage chronic obstructive pulmonary disease presented with vomiting and abdominal pain. On examination her abdomen was grossly distended, diffusely tender and hyper-resonant. Imaging showed dilated loops of bowel and free air in the abdomen with no intestinal perforation. The free abdominal air had come down from the thorax by dissecting down around the oesophagus. A pneumomediastinum was present in her chest, secondary to her extensive emphysematous disease. She was treated conservatively and her pneumomediastinum resolved several weeks later, with subsequent resum
