The historical evolution of the understanding of the submandibular and sublingual salivary glands.

The discovery of the major salivary glands was primarily a story of the establishment of the glands major excretory ducts. Occurring during the Renaissance: religious, political, and philosophical considerations played a role in defining the structure's function. We describe the history and background of these political, religious, and philosophical factors. Next, we present a translation of the original texts, describing the establishment of the submandibular and sublingual salivary glands. We place these translations into historical context and comment on their naming propriety. Initially we

[Forced sterilization 1933-1945: an attempt at coming to terms with the past].

An evaluation of the operative ledgers of the years 1932 to 1945 revealed that 509 women aged 11 to 44 years underwent involuntary sterilization at the Women's Clinic of Frankfurt University. 156 were pregnant at the time of sterilization, all but eight were aborted. Sterilizations were carried out in 161 women because of "congenital mental deficiency", and in another 178 for a variety of other diagnosis. There was no likewise information on the remaining 176 women available. The records of 272/509 women showed that the sterilizations had been proscribed by special "Boards of Racial Hygiene".

Legacy of German psychiatric genetics: hindsight is always 20/20.

The history of psychiatric genetics is informed by this paper, which serves to review the legacy of German psychiatric genetics and its antecedents during the twentieth century. It also serves as an introduction to two new annotated abstracts of basic research papers on family studies of schizophrenia by Ernst Rüdin in 1916 and by Bruno Schulz in 1932, submitted by Kenneth Kendler and Edith Zerbin-Rüdin, together with another paper by them describing the origin and activities of Rüdin's Munich School of Psychiatric Genetics (1917-1945). Our paper also introduces an invited critical summary of

Wounds beyond bars: reflecting on the urgent need for psychological support and interventions in the aftermath of Syria's regime collapse.

Decades of systemic oppression in Syria, from the 1963 state of emergency to the 2011-2024 conflict, have caused widespread psychological devastation. Arbitrary imprisonment, torture and sexual violence have been systematically weaponised. Following the fall of the Syrian regime in December 2024, freed political prisoners face severe mental health challenges due to years of inhumane conditions and trauma. This paper emphasises the urgent need for specialised mental health interventions and outlines evidence-based pathways to healing. A coordinated, multi-tiered response, integrating local and

Cholera in Africa: Current trends, challenges, and control towards elimination strategies.

Cholera remains a substantial public health challenge in Africa, despite the availability of effective vaccines, treatment, and preventive measures. Systemic issues such as inadequate water, sanitation, and hygiene (WASH) infrastructure, weak disease surveillance, socio-economic disparities, and ongoing conflict and instability drive its persistence. This article provides the current situation of cholera in Africa, highlighting systemic barriers to control, including data deficiencies, limited diagnostic capacity, and infrastructural deficits. Crucially, we underline the importance of integrat

Critical Commentary: Cities in a post-COVID world.

This paper examines the effect of the COVID-19 pandemic and its related economic, fiscal, social and political fallout on cities and metropolitan regions. We assess the effect of the pandemic on urban economic geography at the intra- and inter-regional geographic scales in the context of four main forces: the social scarring instilled by the pandemic; the lockdown as a forced experiment; the need to secure the urban built environment against future risks; and changes in the urban form and system. At the macrogeographic scale, we argue the pandemic is unlikely to significantly alter the winner-

[Community participation and control of endemic diseases in Brazil: problems and possibilities].

Community participation (CP) is considered one of the most important elements for the control of endemic diseases in poor countries, particularly for prevention and epidemiological surveillance. Mainly affecting the poorest segments of the population, endemic diseases are costly and require specific government action aimed at efficient, egalitarian, universal health system, where CP acts in a guardian, monitoring capacity and entails other specific tasks. Despite its rationality, CP has not been encouraged in countries like Brazil, due to the highly centralized nature of political power and ac

Toward an integrated and globally equitable understanding of political polarization.

Political polarization poses major societal risks, yet a globally relevant, interdisciplinary understanding remains lacking, as nearly half of all studies focus on the United States. We argue for renewed effort to bring global equity to polarization research, highlighting insights robust across countries, those unique to specific contexts, and key literature gaps. Closing these gaps means overcoming cultural and systemic barriers, including data-access restrictions and misaligned incentives. It also demands interdisciplinarity bridging traditional approaches and those addressing the polarizing

[Noise in meaningful care; plea for a noisy and improvisational care practice].

The Dutch Healthcare Evaluation & Appropriate Use program aims to ensure that only proven effective care is provided. A protocol-based healthcare system is based on an ideal that leaves little room for an improvising healthcare practice with unique patient experiences. This essay articulates the contrast between a technological-rationalist approach to healthcare and the phenomena in healthcare that fall outside this framework. Phenomena that hardly fit within the political ideal of a healthy and productive society, such as suffering, misfortune, and death. Inspired by the philosopher of scienc

Prison pharmaceutical care in the State of Pará, Brazil: determining factors for access to medicines and the right to health.

Comprehensive access to health is a global issue. One-third of the population does not have regular access to essential medicines. People Deprived of Liberty (PDL) are one of those people in a situation of unequal access. Given the uniqueness of the penitentiary system, this research aimed to identify the determining factors in the access to medicines made available by the Brazilian Unified Health System (SUS) for the PDLs in Pará, Brazil. The applied, exploratory, qualitative research was conducted from August 2019 to February 2020 using the APOTECA framework. The APOTECA framework analysis r

The danger in conservative framing of a complex, systems-level issue.

One's weight is the outcome of a complex interplay of factors within which the choices we make about diet and activity are constrained and shaped by systemic forces - biological, social and economic - that fall increasingly beyond our control. "Simple" solutions that ignore the complex, systems-level characteristics of the obesity epidemic will generally fail as counter-veiling forces act to negate and undermine whatever action is taken. Selling the prevention message is not enough if politicians can choose conservative options that give the appearance of action but fail to tackle the issue. T

Role of a sentinel surveillance system in the context of global surveillance of infectious diseases.

In some nation states, sustained integrated global epidemiological surveillance has been weakened as a result of political unrest, disinterest, and a poorly developed infrastructure due to rapidly increasing global inequality. The emergence of severe acute respiratory syndrome has shown vividly the importance of sensitive worldwide surveillance. The Agency for Cooperation in International Health, a Japanese non-governmental organisation, has developed on a voluntary basis a sentinel surveillance system for selected target infectious diseases, covering South America, Africa, and Asia. The syste

Contracting-out urban primary health care in Bangladesh: a qualitative exploration of implementation processes and experience.

BACKGROUND: Contracting-out (CO) to non-state providers is used widely to increase access to health care, but it entails many implementation challenges. Using Bangladesh's two decades of experience with contracting out Urban Primary Health Care (UPHC), this paper identifies contextual, contractual, and actor-related factors that require consideration when implementing CO in Low- and Middle- Income Countries. METHODS: This qualitative case-study is based on 42 in-depth interviews with past and present stakeholders working with the government and the UPHC project, as well as a desk review of ke

Disaggregating Gender Inequality Globally: Health Systems, Gender Inequality, and Female Pan-Cancer Outcomes.

PURPOSE: Gender inequality shapes women's cancer outcomes through social, economic, and health system factors. This study disaggregated the Gender Inequality Index (GII) to identify which dimensions-reproductive health, educational attainment, workforce participation, and political empowerment-independently influence global female cancer outcomes. METHODS: We conducted an ecologic, cross-national analysis using data from 185 countries. Age-standardized female cancer mortality-to-incidence ratios (MIRs) were derived from GLOBOCAN 2022. Independent variables included the composite GII and its f

"If I am here, it is because the system failed": a critical qualitative study of global commercial clinical trials for advanced cancer in Chile.

BACKGROUND: The pharmaceutical industry is a key global health actor given its crucial role in the development and testing of medicines. Within Latin America, Chile currently has the highest number of multinational commercial clinical trials per inhabitant, predominantly for oncological therapies. Insufficient attention has been paid to context-specific conditions that enable clinical trials' successful implementation in the Global South. In examining what makes Chile an attractive hub for commercial trials and why patients enroll in them, this study seeks to elucidate the interplay among the

Community-level barriers and facilitators of indoor residual spraying for malaria control: a systematic review of qualitative evidence from sub-Saharan Africa.

INTRODUCTION: Indoor residual spraying is a key strategy in the control against malaria, which still remains a devastating public health challenge in Sub-Saharan Africa. It was recommended by the World Health Organization, yet its effectiveness is still being compromised by low community uptake. Unlike other malaria interventions, indoor residual spraying is intrusive and requires entry into private household spaces and substantial domestic preparation. Household refusal and locked structures still remain primary threats to the success of the campaign. Although qualitative studies have examine

Factors influencing uptake of COVID-19 diagnostics in Sub-Saharan Africa: a rapid scoping review.

BACKGROUND: Diagnostics are critical for preventing COVID-19 transmission, enabling disease management and engagement with care. However, COVID-19 testing uptake remained low in low- and middle- income countries in Sub-Saharan Africa (SSA) during the recent pandemic, due to issues of supply, access and acceptability. Early studies conducted outside of the region provide insight into uptake of COVID-19 testing, however there has been no systematic research within the region. The aim of this scoping review is to investigate factors influencing uptake of COVID-19 testing in different settings acr

Caring for Africa's sickle cell children: will we rise to the challenge?

BACKGROUND: Most of the world's sickle cell disease (SCD) burden is in Africa, where it is a major contributor to child morbidity and mortality. Despite the low cost of many preventive SCD interventions, insufficient resources have been allocated, and progress in alleviating the SCD burden has lagged behind other public-health efforts in Africa. The recent announcement of massive new funding for research into curative SCD therapies is encouraging in the long term, but over the next few decades, it is unlikely to help Africa's SCD children substantially. MAIN DISCUSSION: A major barrier to pro

Synthesis and translation of research and innovations from polio eradication (STRIPE): initial findings from a global mixed methods study.

BACKGROUND: Lessons from polio eradication efforts and the Global Polio Eradication Initiative (GPEI) are useful for improving health service delivery and outcomes globally. The Synthesis and Translation of Research and Innovations from Polio Eradication (STRIPE) is a multi-phase project which aims to map, package and disseminate knowledge from polio eradication initiatives as academic and training programs. This paper discusses initial findings from the knowledge mapping around polio eradication activities across a multi-country context. METHODS: The knowledge mapping phase (January 2018 - D

Causal regulations vs. political will: why human zoonotic infections increase despite precautionary bans on animal antibiotics.

Using precautionary principles when facing incomplete facts and causal conjectures raises the possibility of a Faustian bargain. This paper applies systems dynamics based on previously unavailable data to show how well intended precautionary policies for promoting food safety may backfire unless they are informed by quantitative cause-and-effect models of how animal antibiotics affect animal and human health. We focus on European Union and United States formulations of regulatory precaution and then analyze zoonotic infections in terms of the consequences of relying on political will to justif
