A Case of Sheehan Syndrome 7 Years Postpartum with Transaminitis and Hyperlipidemia.

BACKGROUND Sheehan syndrome (pituitary necrosis after postpartum hemorrhage) can present in various ways, depending on the hormones that are deficient. There may be a long delay to diagnosis of over a decade because symptoms are often vague and pituitary dysfunction progresses gradually. We describe a case of a patient with acute presentation of Sheehan syndrome 7 years after the obstetric event and with no clear precipitating event. CASE REPORT A 46-year-old woman with a history of hyperlipidemia and transaminitis came to the clinic for evaluation of hypotension and syncope. She had a history

Sheehan syndrome.

Sheehan syndrome or postpartum hypopituitarism is a condition characterized by hypopituitarism due to necrosis of the pituitary gland. The initial insult is caused by massive postpartum haemorrhage (PPH), leading to impaired blood supply to the pituitary gland, which has become enlarged during pregnancy. Small sella turcica size, vasospasms (caused by PPH) and/or thrombosis (associated with pregnancy or coagulation disorders) are predisposing factors; autoimmunity might be involved in the progressive worsening of pituitary functions. Symptoms are caused by a decrease or absence of one or more

Lymphocytic hypophysitis.

BACKGROUND: Lymphocytic hypophysitis is a disorder of the pituitary gland that presents as a sellar mass lesion and/or hypopituitarism. It causes pituicyte destruction and hypopituitarism and is speculated to have an autoimmune basis. DIAGNOSIS: Lymphocytic hypophysitis should be considered in the differential diagnosis of pituitary masses and/or hypopituitarism in females who are pregnant or in the early postpartum period, especially in cases associated with other autoimmune diseases or unusual patterns of hormone deficiencies. A definitive diagnosis requires tissue biopsy. A presumptive cli

[Features of adaptation of newborns born to mothers with endemic goiter].

Relationships between chronic iodine deficiency and, among other things, endemic goiter in women and adaptation of their babies in the early neonatal period were studied. A total of 125 women with endemic goiter of the Ist-IInd degrees and their newborns were examined. The diagnosis of endemic goiter was verified by the findings of an objective examination, ultrasonic examination, and puncture biopsy of the thyroid. Blood levels of triiodothyronine and thyroxin were measured in the women on days 3-4 postpartum. In the newborns Apgar score, body mass and length at birth, degree of maturity, phy

Delayed Diagnosis of Sheehan's Syndrome Following Postpartum Hemorrhage and Emergency Hysterectomy: A Case Report.

This case report describes a 50-year-old woman diagnosed with Sheehan's syndrome, a rare form of hypopituitarism resulting from severe postpartum hemorrhage during a cesarean section in 2019, which led to cardiac arrest and emergency hysterectomy. Despite experiencing persistent fatigue, muscle pain, constipation, and mood changes for five years, her symptoms were repeatedly misattributed to anxiety, anemia, and vitamin D deficiency. A detailed review of her obstetric history, combined with endocrine testing showing central hypothyroidism, adrenal insufficiency, and low IGF-1, led to the corre

Association of thyroid function test abnormalities with preeclampsia: a systematic review and meta-analysis.

BACKGROUND: Preeclampsia is a life-threatening disorder during pregnancy and postpartum periods. Preeclampsia can affect the activity of many organs. It is very important because if this disorder is associated with changes in thyroid function, it can affect the results of maternal and fetal tests. Accordingly, the aim of this meta-analysis study was to assess the abnormalities in thyroid function tests in preeclampsia. METHODS: Studies were selected through a systematic search of the MEDLINE/PubMed, Scopus, Web of Science Core Collection, and Google Scholar databases in 31st August 2021. Also

Propylthiouracil-induced agranulocytosis in the third trimester of pregnancy.

BACKGROUND: Thionamide-induced agranulocytosis in pregnancy is a rare event that poses unique therapeutic challenges. CASE: A 37-year-old woman developed agranulocytosis while taking propylthiouracil in the third trimester. After she took broad-spectrum antibiotics and discontinued propylthiouracil, her neutrophil counts recovered. She was initially managed expectantly but later underwent an uncomplicated total thyroidectomy at 35 weeks of gestation because of patient choice coupled with worsening thyrotoxicosis. CONCLUSION: In circumstances in which thionamides are contraindicated, manageme

Pituitary thyrotroph hyperplasia mimicking prolactin-secreting adenoma.

A 32-year-old woman with persistent postpartum galactorrhea, hyperprolactinemia and a sellar configuration compatible with pituitary adenoma underwent transsphenoidal surgery. Mild hyperthyroidism had been suspected clinically preoperatively and thyroxine plus cortisol therapy was given. Morphological features of pituitary thyrotroph hyperplasia were present as shown by light microscopy, immunoperoxidase staining and electron microscopy. The thyrotroph hyperplasia was secondary to primary hypothyroidism as demonstrated by the TSH response to TRH stimulation and subsequent suppression with thyr

Feeding Ascophyllum nodosum meal or ethylenediamine dihydroiodide to transition dairy cows: Effects on milk yield and plasma concentrations of thyroid hormones and markers of metabolic status.

Ascophyllum nodosum (ASCO) is a brown seaweed rich in I and bioactive compounds. Previous research demonstrated that feeding incremental amounts of ASCO meal linearly decreased the plasma concentrations of nonesterified fatty acids (NEFA) and cortisol in lactating dairy cows, suggesting that ASCO may improve cows' metabolic status during the transition period. We aimed to investigate the effects of supplementing transition dairy cows with ASCO meal or ethylenediamine dihydroiodide (EDDI) on plasma concentrations of thyroid hormones and markers of metabolic status, DMI, and milk yield and compo

Serum 25-Hydroxyvitamin D and Parathyroid Hormone Levels in Non-Lactating Women with Post-Partum Thyroiditis: The Effect of L-Thyroxine Treatment.

Vitamin D deficiency seems to be implicated in the onset and progression of some autoimmune disorders. No previous study has investigated vitamin D homeostasis in post-partum thyroiditis. We compared 25-hydroxyvitamin D and parathyroid hormone (PTH) levels between four groups of non-lactating women who gave birth within 12 months before the beginning of the study: hypothyroid women with post-partum thyroiditis (group A; n = 14), euthyroid females with post-partum thyroiditis (group B; n = 14), women with non-autoimmune hypothyroidism (group C; n = 16) and healthy euthyroid females without thyr

The Relationship Between Iodine Excess and Thyroid Function During Pregnancy and Infantile Neurodevelopment at 18-24 Months.

BACKGROUND: Thyroid disease is a prevalent condition during pregnancy, and excessive iodine intake can lead to changes in thyroid function. However, research on the relationship between maternal iodine excess, thyroid hormones during pregnancy, and infantile neurodevelopment is limited. OBJECTIVES: This study aimed to explore the relationship between maternal iodine excess and thyroid hormones during pregnancy and infantile neurodevelopment. The objective was to provide evidence to support and enhance the prevention of neurodevelopmental retardation in infants. METHODS: From 2016 to 2018, a

Hyperthyroidism During Pregnancy: A Clinical Approach.

Hyperthyroidism is relatively uncommon during pregnancy. However, those caring for pregnant patients should be versed in the evaluation and management of hyperthyroidism, as there are potential maternal and fetal implications that are related to the disease and to treatment. The differential diagnosis of hyperthyroidism includes clinical and subclinical entities, as well as transient laboratory findings that are related to the pregnancy itself. The clinical management, including the indications for the use of thioamide or antithyroid medications, will be discussed in the context of pregnancy.

Cushing's disease with twin pregnancy and diabetes mellitus: a case report and literature review.

A 38-year-old Japanese woman with a history of abnormal thyroid function of non-autoimmune origin, pituitary endocrine tumor, and untreated diabetes mellitus was referred to our outpatient clinic when she became pregnant with twins. Physical findings consistent with Cushing's syndrome (CS) were absent at the time of presentation. Although baseline plasma adrenocorticotropic hormone, serum cortisol, and 24-hour urinary free cortisol excretion levels were above the upper limits of normal non-pregnant reference ranges, we could not exclude a physiological increase associated with pregnancy. No me

Determining the breast-feeding interruption schedule after administration of 123I-iodide.

Radioactivity after administration of 123I-sodium iodide was measured in breast milk samples obtained from a patient with postpartum thyroiditis. The breast milk was collected over 93 h during the infant's regular feeding times. The radioactivity in the breast milk was calculated with a 123I capsule of the same lot number as the standard source. 123I was excreted exponentially with an effective half-life of 5.5 h; 2.5% of the total radioactivity administered was excreted in the breast milk over the 93 h, 95% of which was excreted within the first 24 h, and 98.2% within 36 h. The first milk sam…

Hypoglycemia in pregnancy. Occurrence due to adrenocorticotropic hormone and growth hormone deficiency.

Severe symptomatic hypoglycemia (serum glucose level, 24 mg/dL) developed in a 23-year-old, 147.3-cm-tall woman during her late second and third trimesters of pregnancy. Endocrine studies disclosed insulin levels less than 2 microU/mL; growth hormone level less than 3 ng/mL; and cortisol level less than 1 microgram/dL. Hydrocortisone therapy corrected her hypoglycemia, and she was delivered of a healthy female infant. Postpartum, her evaluation included normal thyroid function studies, a normal thyroid-stimulating hormone response to protirelin (thyrotropin-releasing hormone), normal serum and

Reduced active thyroid hormone levels after delivery.

The effect of delivery on the serum concentration of thyroid hormones was studied in 25 euthyroid women. After delivery serum free and total T3 and T4 fell transiently with a simultaneous increase in reverse T3 while serum TSH and thyroxine binding globulin (TBG) concentrations showed no significant variation. These data suggest that i) similar to what happens in other stressful situations, delivery influences peripheral T4 metabolism, and ii) an elevation of TBG in serum in the early puerperium does not prevent these changes.

ISOLATED MATERNAL HYPOTHYROXINEMIA AND ITS PERINATAL OUTCOME IN NORTH MACEDONIA.

Isolated maternal hypothyroxinemia (IMH) is defined as the presence of low maternal total thyroxine (TT4) level in conjunction with normal maternal thyroid-stimulating hormone (TSH) level. The aim was to investigate whether IMH is associated with adverse pregnancy outcome in North Macedonia. Dried blood spot samples were obtained from 359 pregnant women meeting the inclusion criteria and analyzed for TT4 and TSH. Postpartum data were entered from their medical histories. Out of 359 women, 131 (37.42%) belonged to IMH group. There were statistically significant differences in birth weight (p=0.
