The authors treated 17 patients with far-advanced gastrointestinal cancer with extracorporeally induced total-body hyperthermia (TBHT) combined with anticancer chemotherapy. Although all patients' tumors were clinically resistant to prior chemotherapy with 1-(2-tetrahydrofurlyl)-5-fluorouracil or 5-fluorouracil and mitomycin C, three patients had an objective response to TBHT plus the same anticancer agents with or without cyclophosphamide. Environmental noise levels in the United States are increasing, yet there are few studies in which the nonauditory effects of workplace noise are assessed. In the current study, the authors examined chronic effects of noise on blood pressure and heart rate in 374 workers at an automobile plant. The authors report an unusual case of bilateral large petrous apex cephaloceles in a 14-year-old boy with a history of recurrent meningitis. Although these lesions are rare and usually asymptomatic, surgical correction is recommended if they are associated with a persistent CSF leak. In this patient, the extensive bilateral cranial defects were not adequately treated by an intracranial approach alone. Repair of a defect in the posterior pharyngeal wall, the site of a prior tonsillectomy, ultimately resulted in repair of the CSF fistula.

Melanin impregnation in squamous cell carcinoma (SCC) is an uncommon histological finding. Rare nonmelanocytic entities were previously described as having melanocyte colonization. A 57-year-old Brazilian woman was referred with a pigmented lesion in the lower lip and alveolar ridge with a prior clinical diagnosis of melanoma. The incisional biopsy of the tumor revealed an SCC with strong colonization of melanocytes in the stroma. The authors report a case of an unusual SCC variant and a brief review of the literature.

The authors evaluate the feasibility of performing endoscopic third ventriculostomy (ETV) in a series of five patients with ventricular outlet obstruction and a prior history of fourth ventricular cysticercosis. All patients underwent fourth ventricular cyst excision and presented at various times afterwards with hydrocephalus. They were all successfully managed with ETV, and no patient has required subsequent surgery during a mean follow-up period of 21 months (range 12-36 months).

Tarlov or perineural cysts are lesions of the nerve root most often found in the sacral region. Several authors recommend surgical treatment of symptomatic Tarlov cysts. However, successful surgical treatment is dependent on appropriate patient selection. In this article, we report three cases of a sacral perineural cyst, causing sciatic pain, and emphasize the usefulness of CT-guided percutaneous aspiration as an important diagnostic and prognostic procedure prior to definitive operative treatment.

The B. B. Shafiroff and A. K. Palmer modification of thumb replantation procedure proposed in 1982 was evaluated. The reconstruction or repair of the artery and sometimes digital nerve repair on the ulnar side of the thumb prior to bony fixation is the mainstay of the modification. It was used in 11 thumb replantations done by the author since 1987. The access to arteries and nerves is grossly improved and operating time shortened. Short description of 6 cases is given. All the replanted thumbs survived.

The authors observed 51 patients after mitral commisurotomy was performed for 1 to 12 years. After the operation the number of the decompensations and the rhythm disturbances had decreased. Twenty six subjects are back at the same work place, 23 retired on pension or changed their work, two died. The mitral commisurotomy appears the only ailing method in advanced cases of mitral stenosis. The active dispensary treatment is an important stage prior to the operation as well as after it.

Osteoma cutis is a term applied to bone formation in the dermis or subcutaneous tissue. It is deemed primary when occurring de novo without prior disease or secondary when associated with pre-existing inflammation, neoplasia or trauma. The authors describe a longstanding focal calcified and ossified lesion of the upper eyelid in a healthy 10-year-old girl along with a sterile corneal ulcer. The lesion most likely represented an ossified tarsal cyst or chalazion.

Intracranial arachnoid cysts are rare but the widespread use of brain imaging has disclosed many asymptomatic cysts. The authors report the case of a 37-year-old woman who presented with features suggestive of relapsing-remitting multiple sclerosis (MS). Her brain MRI revealed a moderately large arachnoid cyst in the right frontal area in addition to the periventricular plaques of MS. The cyst was an incidental imaging finding and the patient was asymptomatic prior to the development of MS.

A number of retinal disorders may present with fleck-like lesions in the retina. We describe the case of a 13-year-old girl who presented with a complaint of decreased vision and prior diagnosis of "fleck-retina." Further examination revealed that the patient had an autosomal recessive disorder associated with systemic manifestations. In the current article, the authors describe the case report and briefly review the various autosomal-recessive disorders that may present with "retinal flecks."

The authors describe a clinical case regarding a young female patient affected by sepsis due to methicillin-resistant Staphylococcus aureus (MRSA), associated to meningoencephalitis and cerebral abscess. The patient had no contact with hospitals in the months prior to illness and had always been healthy. She recovered thanks to linezolid therapy. The MRSA strain proved positive for Panton-Valentine leukocidin (PVL positive) and was therefore defined as community-acquired MRSA (CA-MRSA).

The authors report the case of a 35 year old patient that presented acutely with a false aneurysm of the tibioperoneal trunk diagnosed at Doppler US and CT angiography. Behçet's disease was considered because of a prior history of recurrent phlebitis, the presence of bipolar aphthae and a biological inflammatory syndrome. This case is of interest for two reasons: first, the atypical location of the false aneurysm, and second, the value of less invasive imaging techniques, namely Doppler US and CT angiography.

This report describes a case of invasive pulmonary aspergillosis affecting a young female patient who had benefited from a liver transplantation for a fulminant hepatic failure. Infections following liver transplantation can happen but in this case the short delay to the fatal outcome is unusual (6 days). Corticotherapy given prior to transplantation could have impaired the immune system. The authors discuss the infections due to Aspergillus species and particularly invasive pulmonary aspergillosis.

This is the first case report of a vesicocutaneous fistula related to prior radiation therapy for recurrent vulvar cancer. Urinary tract complications happen not infrequently after radiation therapy for various pelvic malignancy. It can occur as long as 30 years after cessation of such therapy. Urinary incontinence or obstructive uropathy is, by far, the most common complication. The authors report an unusual complication secondary to radiation therapy. The aetiology, presentation and management of this patient were discussed.
