BACKGROUND: Tuberculosis (TB) is the second leading cause of death worldwide due to a single infectious agent. Rates of active TB in places of prescribed detention (PPD), which include Prisons, Young Offender Institutions and Immigration Removal Centres, are high compared with the general population. PPD therefore present an opportunity to develop targeted health programmes for TB control. This audit aims to assess current service provisions and identify barriers to achieving best practice standards in PPD across London.
METHODS: Twelve healthcare teams within PPD commissioned by NHS England …

Insomnia in prison is common; however, research is limited regarding the management strategies that prison establishments employ. To address this knowledge gap, we conducted a survey to identify how insomnia is detected, diagnosed and treated in adult prisons in England and Wales. Telephone interviews with a purposive sample of health-care managers were then conducted. The survey was sent to all establishments holding adult prisoners, covering screening and assessment methods to detect insomnia; treatment options, both pharmacological and non-pharmacological; the importance of insomnia as a tr…

BACKGROUND: To measure levels of indoor pollution in relation to smoking in four English prisons.
METHODS: TSI SidePak AM510 Personal Aerosol Monitors were used to measure concentrations of particulate matter less than 2.5 μm in diameter (PM2.5) for periods of up to 9 h in selected smoking and non-smoking areas, and personal exposure monitoring of prison staff during a work shift, in four prisons.
RESULTS: PM2.5 data were collected for average periods of 6.5 h from 48 locations on 25 wing landings where smoking was permitted in cells, on 5 non-smoking wings, 13 prisoner cells, and personal m…

AIMS: To determine the lifetime and monthly prevalence of people with mental disorders and its association with sociodemographic factors and criminal risk in three Spanish prisons (Ocaña, Madrid I, II and VI).
METHOD: Cross-sectional epidemiological study of a sample of 184 inmates. Socio-demographic and criminal data were collected by an ad hoc interview. Mental disorders were assessed with the clinical version of the Structured Clinical Interview for Diagnostic and Statistical Manual of Mental Disorders Axis I Disorders (SCID-I).

Tuberculosis control measures in Brazil's prison population have been regulated for ten years under the National Prison Health System Plan. Brazilian states have different organizational models for the Tuberculosis Control Program (TCP) in their prison systems. This study evaluated TCP implementation in prisons in two Brazilian states, using a multiple case study design with a qualitative approach and a log-frame analysis and assessment. According to predefined criteria, two state cases were selected, with two analytical units for each case and one prison hospital in Case 2. We identified part…

OBJECTIVE: To improve the Zambia Prisons Service's implementation of tuberculosis screening and human immunodeficiency virus (HIV) testing.
METHODS: For both tuberculosis and HIV, we implemented mass screening of inmates and community-based screening of those residing in encampments adjacent to prisons. We also established routine systems – with inmates as peer educators – for the screening of newly entered or symptomatic inmates. We improved infection control measures, increased diagnostic capacity and promoted awareness of tuberculosis in Zambia's prisons.
FINDINGS: In a period of 9 months…

Extreme temperatures pose severe risks for population health and healthcare systems. However, little research has examined implications for incarcerated populations, who may be particularly vulnerable given their limited control over their thermal environments. Here, we show that among people imprisoned in Pennsylvania (2000-2023), both warmer and cooler temperatures were significantly associated with higher rates of mortality, transfers to outside medical facilities and prison infirmary admissions, relative to outcome-specific reference minimum threshold temperatures. As temperature extremes

Objectives. To assess relationships between changes in population levels, health care access, health care grievances, and mortality in California prisons. Methods. We examined 30 California prisons using June 2013 to May 2023 data from California Correctional Health Care Services and the California Department of Justice. Associations between prison occupancy rates and care access, health care grievances, and mortality were estimated in linear or generalized linear models controlling for staff vacancies and the characteristics of incarcerated people. Because of the prevalence of COVID-19-specif…

OBJECTIVES: In many prisons, some imprisoned individuals are trained to meet the non-personal social care needs of fellow inmates yet there is little quantitative or economic evidence of the value of these schemes. In this study we investigated how to best monitor and measure their effectiveness and cost, the current state of data collection in England and Wales and where there are data gaps.
STUDY DESIGN: Part of a larger multi-site rapid mixed-methods evaluation of social care peer-support services in prisons. This sub-study included a rapid scoping review, interviews with staff, a stakehol…

BACKGROUND: Understanding determinants of Mycobacterium tuberculosis transmission is critical to devising effective strategies to reduce its burden. Whether and to what extent symptoms influence transmission remains poorly understood.
METHODS: Between 2020 and 2022, we systematically screened persons deprived of liberty (PDL) from three prisons in Brazil for tuberculosis (TB) by symptom assessment and sputum testing with Xpert MTB/RIF Ultra. We performed QuantiFERON-TB Gold Plus (QFT) testing among cell contacts of individuals with TB and in cells with no TB case identified. We evaluated the …

BACKGROUND: People living in prisons often experience poor oral health, which could be attributed to their limited access to (dental) care, financial constraints, and a general lack of awareness and prioritization toward their oral hygiene. A pilot study involving motivational interviewing (MI) has shown promising results for improving the oral health outcomes of people living in prisons.
OBJECTIVE: The protocol for this study aims to assess the efficacy of integrated MI and oral hygiene packages in improving oral health among people living in prisons, compared to controls without added MI.

Prisons have been hotspots for COVID-19 and likely an important driver of racial disparity in disease burden. From the first COVID-19 case detected through March 25, 2022, 66,684 of 196,652 residents of California's state prison system were infected, most of them in two large winter waves of outbreaks that reached all 35 of the state prisons. We used individual-level data on disease timing and nightly room assignments in these prisons to reconstruct locations and pathways of transmission statistically, and from that estimated reproduction numbers, locations of unobserved infection events, and …

BACKGROUND: More than 50,000 older male veterans incarcerated in prisons are expected to return to their communities and utilize the Veterans Health Administration (VHA) and community healthcare systems. To support the continuity of healthcare and overall successful community reentry of older incarcerated veterans, an understanding of their health profiles and treatment utilization while in correctional care is needed.
OBJECTIVE: To assess the health status of older male veterans incarcerated in state prisons and explore demographic, military, and VHA-related factors associated with medical c…

BACKGROUND: People who experience incarceration are characterised by poor health profiles. Clarification of the disease burden in the prison population can inform service and policy development. We aimed to synthesise and assess the evidence regarding the epidemiology of mental and physical health conditions among people in prisons worldwide.
METHODS: In this umbrella review, five bibliographic databases (Web of Science, PubMed, PsycINFO, Embase, and Global Health) were systematically searched from inception to identify meta-analyses published up to Oct 31, 2023, which examined the prevalence…

BACKGROUND: Reducing avoidable healthcare-associated harm is a global health priority. Progress in evaluating the burden and aetiology of avoidable harm in prisons is limited compared with other healthcare sectors. To address this gap, this study aimed to develop a definition of avoidable harm to facilitate future epidemiological studies in prisons.
METHODS: Using a sequential mixed methods study design we first characterised and reached consensus on the types and avoidability of patient harm in prison healthcare involving analysis of 151 serious prison incidents reported to the Strategic Exe…
