[Gasserian ganglion block for trigeminal neuralgia under dexmedetomidine sedation].

We report a case of successful procedural sedation using dexmedetomidine (DEX) in a 68-year-old woman undergoing left gasserian ganglion block for intractable trigeminal neuralgia. DEX was chosen to provide an effective sedation and clear communication about the injection of drugs or thermocoagulation. After 15 minutes of DEX administration at 0.8 microg x kg(-1) x hr(-1), nerve block needle insertion was bearable. The patient could feel a weakening of the sensation of her left maxilla and mandible after receiving a 0.4 ml mepivacaine test dose. High-frequency thermocoagulation elicited tolera…

Anesthesia with remimazolam versus dexmedetomidine and midazolam for the endoscopic submucosal dissection of the esophagus, stomach, and colorectum: A retrospective comparative study.

Objective Endoscopic submucosal dissection (ESD) requires prolonged and stable sedation. Although midazolam combined with dexmedetomidine (MDZ+DEX) is widely used, delayed postoperative awakening and hemodynamic instability remain concerns, particularly in elderly patients. This study aimed to compare the efficacy and safety of remimazolam (RMZ) with MDZ+DEX for sedation during ESD. Methods This single-center retrospective study was conducted in Japan. The primary outcome was the quality of postoperative awakening. The secondary outcomes included procedural outcomes and sedation-related advers

Effect of conscious sedation and deep sedation on the vagal response characteristics during ganglionated plexus ablation.

INTRODUCTION: We aimed to determine the effects of conscious and deep sedation on vagal response (VR) characteristics during ganglionated plexus (GP) ablation. METHODS: Forty consecutive patients undergoing GP ablation for vasovagal syncope were divided to receive conscious sedation with midazolam (Group 1, n = 29) or deep sedation with the midazolam-propofol combination (Group 2, n = 11). VR was defined on three levels. R-R interval increase of >50% (Level 1); R-R interval increase of 20%-50% (Level 2); and R-R interval increase of <20% (Level 3). RESULTS: The ratio of Level 1 VR during abl

Ciprofol-lidocaine combination for sedation in gastrointestinal endoscopy: a systematic review and meta-analysis protocol.

Ciprofol is a recently introduced intravenous hypnotic agent with sedative efficacy comparable to that of propofol while conferring superior haemodynamic stability and a lower incidence of adverse events. Intravenous lidocaine, widely employed as an adjuvant to propofol-based sedation, has been shown to reduce hypnotic requirements and may facilitate recovery. Nevertheless, no systematic synthesis has yet appraised the combined use of these two agents in the context of gastrointestinal endoscopy. The present protocol delineates the methodology for a systematic review and meta-analysis designed

The Effects of Music Therapy on Patients Undergoing Colonoscopy in a Tertiary Care Hospital at Karachi, Pakistan: A Comparative Study.

A colonoscopy is a procedure commonly used for the diagnosis of colorectal diseases but may be associated with high levels of anxiety and discomfort, which can lead to complications during sedation and decreased patient comfort and cooperation. This study was conducted at a tertiary care hospital in Karachi to assess the effectiveness of music therapy on pre-procedural anxiety, sedation requirements, pain, and procedure duration among patients undergoing colonoscopies. This comparative study included 110 outpatients. The intervention arm received non-lyrical soft music through earphones before

Propofol compared with combination propofol or midazolam/fentanyl for endoscopy in a community setting.

This retrospective cohort study evaluated procedural efficiency and patient satisfaction in patients who had received propofol, midazolam/fentanyl/propofol (MFP), or midazolam/fentanyl, as sedation for either esophagogastroduodenoscopy or colonoscopy. Questionnaires about procedural times and patient satisfaction were administered. Use of propofol for colonoscopy resulted in shorter time (minutes) from induction to start of procedure (mean +/- standard deviation: propofol, 1.3 +/- 0.57; MFP, 3.2 +/- 2.2; midazolam/fentanyl, 3.8 +/- 2.7; P < .04) and shorter procedure time (propofol, 13 +/- 0.3

Dexmedetomidine vs. Propofol-Fentanyl for Bispectral Index (BIS)-Guided Sedation During Awake Fiberoptic Intubation in Neurosurgical Patients With Anticipated Difficult Airways: An Observational Study.

Introduction Awake fiberoptic intubation (AFOI) is commonly employed in neurosurgical patients with anticipated difficult airways to minimize spinal cord compression and intracranial pressure fluctuations, although procedural discomfort remains a major limitation. This prospective observational study evaluated the AFOI with two commonly used conscious sedation regimens-dexmedetomidine and a propofol-fentanyl combination-guided by Bispectral Index (BIS) and the Observer's Assessment of Alertness/Sedation (OAA/S) scale, along with spray-as-you-go (SAYGO) topical anesthesia. A Prospective Comparative Non-randomized Study of Ketofol Versus Midazolam Sedation in Flexible Bronchoscopy.

Background Flexible bronchoscopy requires reliable sedation that preserves respiratory drive while ensuring patient comfort and procedural efficiency. Midazolam, a benzodiazepine, is widely employed but is associated with respiratory depression, unpredictable recovery kinetics, and compromised oxygenation, particularly in patients with underlying pulmonary disease. Ketofol, a fixed-ratio admixture of ketamine and propofol, exploits the pharmacodynamic complementarity of its constituents: ketamine maintains airway reflexes and sympathetic tone, while propofol confers rapid-onset sedation and sw

Audiovisual Distraction During Mitral Transcatheter Edge-to-Edge Repair: A Sedation-Sparing Randomized Trial.

BACKGROUND: Conscious sedation is increasingly used for mitral transcatheter edge-to-edge repair (M-TEER), but balancing patient comfort with sedative-related hemodynamic and neurocognitive risk remains challenging in frail older adults. OBJECTIVES: We investigated whether audiovisual distraction (AVD) with video glasses reduces propofol requirements and improves patient experience during M-TEER performed under conscious sedation. METHODS: In this prospective randomized controlled trial, 60 patients undergoing M-TEER were assigned 1:1 to AVD (video glasses with nature documentaries and headp

Feasibility of remimazolam rescue sedation for neonatal MRI: a case report.

BACKGROUND: High-quality neonatal imaging often necessitates sedation or anesthesia to minimize motion artifacts. However, procedural sedation for non-painful pediatric examinations outside the operating room presents unique challenges due to their distinct cardiopulmonary physiology and drug metabolism. Although remimazolam is associated with minimal hemodynamic and respiratory effects in adults, evidence supporting its use in neonatal MRI remains limited. CASE DESCRIPTION: We report the first successful application of remimazolam for MRI sedation in an early-term neonate (37 ⁴⁄₇ weeks' gest…

Applications of bispectral index monitoring in the pediatric intensive care unit.

The bispectral index (BIS) monitor is an electroencephalographic recording device that generates a single numeric value. It has traditionally been used to measure anesthetic depth and avoid awareness in the operating room setting. In this report, the authors present different scenarios in the pediatric intensive care unit that demonstrate the potential applications of the BIS monitor outside of the operating room setting. The scenarios presented include use of the BIS monitor during titration of barbiturate coma, procedural sedation, sedation assessment during mechanical ventilation, and sedat…
