Consensus recommendations on holistic care in hereditary ATTR amyloidosis: an international Delphi survey of patient advocates and multidisciplinary healthcare professionals.

BACKGROUND: Hereditary transthyretin-mediated amyloidosis is a rare, progressive and potentially life-limiting multisystem disease, affecting every aspect of a patient's life. OBJECTIVES: This online international Delphi survey aimed to evolve clinical-patient-led practical guidance, to inspire and encourage a holistic approach to care that is managed in specialist settings by multidisciplinary teams and supported by allied healthcare professionals (HCPs) and patient advocacy groups (PAGs). DESIGN: A 14-member joint patient advocate-HCP primary panel was convened including representation fro

Treatment of Methotrexate-Associated Lymphoproliferative Disorder With Biological Therapies.

Lymphoproliferative disorders may arise as a complication of immunosuppressant medications, such as methotrexate. This case report describes a patient who developed a rare subtype of methotrexate-associated lymphoproliferative disorder. His disease initially responded well to the withdrawal of methotrexate. However, several months after diagnosis, surveillance testing revealed progressive lymphadenopathy. Owing to his multiple comorbidities and resultant poor baseline functional status, he was not a candidate for cytotoxic chemotherapy. Based on key histopathological characteristics of his rar

Intradural extra-medullary spinal cord tumor after dorso-lumbar spine fixation following 12th dorsal vertebra burst fracture: A case report with literature review.

Intradural extramedullary spinal cord tumors are rare but can cause significant neurological symptoms. We present a case of a 23-year-old male who developed progressive bilateral leg pain and lower limb weakness 2 years after undergoing posterior spinal fixation for a T12 burst fracture. Magnetic resonance imaging (MRI) revealed an intradural extramedullary tumor at the site of the previous surgery. The patient underwent surgical excision of the tumor, which was confirmed to be a Schwannoma. Postoperatively, the patient experienced significant improvement in both motor function and pain. This

Non-invasive management of severe chlamydia psittaci pneumonia presenting with hypoxemia and diarrhea: a case report.

This case report describes a rare presentation of severe Chlamydia psittaci pneumonia in a 43-year-old female patient with prominent hypoxemia and gastrointestinal symptoms, and evaluates the efficacy of standardized non-invasive integrated management for critically ill patients with this atypical phenotype. The patient was admitted with lumbago, persistent high fever, progressive dyspnea, severe hypoxemia, and intractable non-bloody watery diarrhea. Chest computed tomography (CT) revealed extensive bilateral pulmonary ground-glass opacities and consolidation. Rapid and precise etiological dia

[CaReMe in primary care: Comprehensive strategies for managing patients with cardio-renal-metabolic conditions].

The cardio-renal-metabolic (CaReMe) syndrome captures the interplay among metabolic factors, kidney damage, and cardiovascular disease, constituting a progressive and preventable process. This educational practical review for primary care provides a practical stage-based (0-4) framework to facilitate patient detection, management, and follow-up. Opportunistic screening based on clinical history and simple measures (waist circumference, blood pressure, glucose, lipid profile), complemented by estimated glomerular filtration rate and albuminuria (chronic kidney disease classification) and by vas

ENDOSCOPIC ASSISTED, TRANSFONTANELLE EXCISION OF A LARGE THIRD VENTRICULAR ATYPICAL CHOROID PLEXUS PAPILLOMA IN AN INFANT.

Choroid plexus papillomas are rare benign tumours, which are found in both adult and paediatric age groups. The 'atypical' histological subclass is rarer still. The commonest site of occurrence is in the lateral ventricles; rare locations are the cerebellopontine angle and the third ventricle. The goal of care is a gross total excision of the tumour. The histologic subtype determines further adjuvant care given to the patient. The outcome of the care of these patients depends on the histological grade, extent of surgical resection adjuvant care. We present a case of a 12 week-old infant with o

Caseworker-assigned discharge plans to prevent hospital readmission for acute exacerbations in children with chronic respiratory illness.

BACKGROUND: Chronic respiratory conditions are major causes of mortality and morbidity. Children with chronic health conditions have increased morbidity associated with their physical, emotional, and general well-being. Acute respiratory exacerbations (AREs) are common in children with chronic respiratory disease, often requiring admission to hospital. Reducing the frequency of AREs and recurrent hospitalisations is therefore an important goal in the individual and public health management of chronic respiratory illnesses in children. Discharge planning is used to decide what a person needs fo

An overview of paediatric autoimmune and genetic cholestatic liver disease for the adult physician.

Paediatric chronic liver diseases are rare, but are increasingly encountered in adult practice as survival improves and more adolescents transition to adult services. Adult physicians must be familiar with these conditions, recognise complications and implement high-quality management of these conditions to provide safe, effective care. Autoimmune liver disease in children includes autoimmune hepatitis, autoimmune sclerosing cholangitis and primary sclerosing cholangitis, with distinct serological and clinical profiles. Cholestatic disorders such as progressive familial intrahepatic cholestasi

Acute Pneumonia Like Illness and Sepsis in India: Is it Time to Suspect Pulmonary Melioidosis?

This article describes a case of melioidosis, a severe and potentially fatal disease caused by the Gram-negative bacillus Burkholderia pseudomallei, in a 55-year-old female in India. The disease is endemic in Southeast Asia and Northern Australia. Recently there has been an increased number of cases reported in India. The source of B. pseudomallei in India is thought to be soil and water, with the most common mode of infection being through skin contact. The clinical presentation of melioidosis in India varies greatly, making diagnosis difficult. The case presented here with a history of acute

Impact of perinatal management on neurodevelopmental outcomes in congenital heart disease.

With advancements in cardiopulmonary bypass technique and perioperative care, there has been a progressive decline in mortality associated with neonatal surgical correction of congenital heart disease (CHD). Thus, there is now increased focus on improving neurodevelopmental outcomes in CHD survivors. While the cause of these neurodevelopmental impairments is multifactorial, there is increasing evidence that structural and functional cerebral abnormalities are present before cardiac corrective repair. This suggests that in addition to patient specific risk factors, underlying cardiac physiology

A Rare Presentation of Metastatic Extragonadal Nonseminomatous Germ Cell Tumor Complicated by Suspected Transfusion-Associated Circulatory Overload.

Extragonadal nonseminomatous germ cell tumors (NSGCTs) are rare and aggressive malignancies that commonly present with advanced metastatic disease and elevated tumor markers. We present the case of a 27-year-old man with no significant past medical history who presented with abdominal pain, dark stools, fatigue, weight loss, and severe normocytic anemia. Imaging revealed a large retroperitoneal mass with extensive pulmonary and hepatic metastases, while tumor marker evaluation demonstrated markedly elevated beta-human chorionic gonadotropin levels without evidence of a primary testicular lesio

Early mobilization in neurocritical care as a threshold-regulated intervention: a narrative review of mechanistic integration and implications for neurological recovery and physiological stability.

The integration of early mobilization into neurocritical care remains constrained by uncertainty regarding how heterogeneous physiological systems respond to graded activity during neurological instability. Although early mobilization is increasingly used in intensive care, the relationships between intervention timing, intensity, neurological recovery, functional outcomes, and physiological tolerance remain incompletely defined in acute neurocritical populations. This narrative review integrates clinical and mechanistic evidence to examine how immobility and reactivation may influence interco

More Physicians and Dentists Should Participate in Clinical Trials Now!

Clinical trials offer physicians and dentists meaningful opportunities to expand patient care, strengthen professional relevance, and contribute directly to scientific progress. Many patients, particularly those with chronic, progressive, rare, or treatment‑resistant conditions, exhaust standard therapies and may benefit from investigational options available only through research. By participating in clinical trials, clinicians might provide access to emerging therapies, diagnostics, devices, and care strategies while allowing patients to remain within their trusted community practices. Guillain-Barré Syndrome Following Seasonal Coronavirus (HCoV-229E) Infection: A Case Managed Without Intensive Care.

Guillain-Barré syndrome (GBS) is an acute immune-mediated polyradiculoneuropathy often preceded by infection. We report the case of a 49-year-old man presenting with subacute, progressive, symmetric ascending weakness involving the lower and distal upper extremities, ultimately diagnosed with GBS, triggered by human coronavirus 229E infection. The patient was managed with intravenous immunoglobulin (IVIG) and close respiratory monitoring in a stepdown unit without progression to respiratory failure or need for intensive care unit (ICU) admission. This case highlights diagnostic considerations,

Early-onset fatal toxic epidermal necrolysis after first-cycle enfortumab vedotin in an elderly patient with metastatic urothelial carcinoma.

An 86-year-old man with metastatic urothelial carcinoma developed fever, mucosal erosions, and rapidly progressive widespread erythema shortly after completing the first cycle of enfortumab vedotin (EV). Skin biopsy showed full-thickness epidermal necrosis consistent with toxic epidermal necrolysis (TEN). EV was discontinued and high-dose corticosteroids, intravenous immunoglobulin, and intensive supportive care were administered; however, the patient deteriorated rapidly and died of septic shock. This case demonstrates that EV-induced TEN may occur early and progress fulminantly, underscoring

[Marfan syndrome].

Marfan syndrome is a genetic disorder of the connective tissue caused by mutations in FBN1 gene. There are approximately 2-3,000 Marfan patients in The Netherlands. It has a broad clinical spectrum with features in most organ systems. Aortic root aneurysm is the most important clinical problem in these patients as it is progressive and leads to dissection and rupture if untreated. Endocarditis prophylaxis is indicated in all Marfan patients with aortic root aneurysm and operated patients. Timely diagnosis and specialized patient care are essential for the prognosis of this disease.
