The shifting landscape of psychotherapy in residency: stable framework of change.

This paper explores the impact of change on a psychiatry trainee's development through a case presentation. During training, psychiatry residents are exposed to constant transitions with frequent shifts in training sites and supervisors. Change is perhaps the most consistent characteristic of residency training. And yet, the maintenance of a changeless, stable frame is considered the gold standard for psychotherapy. The effect of a constantly shifting framework on the training therapist, the patient, and the therapy itself is considered. Reflecting on the multiple and varied themes elucidated …

Introduction to the special section on stepped care models in psychotherapy.

Not all patients need the same type and intensity of intervention. Some may be helped greatly by reading a self-help book, watching an instructional video, or using a computer program. Others could benefit from a brief psychoeducational group conducted by a paraprofessional, and still others may require long-term individual treatment from a highly trained professional therapist with specialized expertise. In an environment of limited resources, it makes sense to provide all the time, expertise, and individual attention a patient needs, but not more. Stepped care models represent attempts to ma

Clinical guidelines for psychotherapy for patients with borderline personality disorder.

In planning a course of psychotherapy for borderline patients, clinicians must take into account the heterogeneity of the clinical presentation in the borderline domain. Borderline personality disorder is usually accompanied, for example, by one or several "symptom disorders," such as an eating disorder, depression, posttraumatic stress disorder, premenstrual tension, dissociative disorder, or anxiety disorder--not to mention one or more other personality disorders. The nature of the "comorbidity" in each patient will determine which medications, if any, are applicable. The accompanying person…

Psychotherapy and integrated treatment of social phobia and comorbid conditions.

Various treatment interventions for social phobia and its comorbid conditions have been developed in the past decade, including psychodynamic, cognitive-behavioral, and psychopharmacological approaches. Because social phobia has only fairly recently been recognized, no one form of treatment has been identified as superior to another. The difficulty in recognizing social phobia and identifying its frequent comorbidity with other psychiatric disorders poses particular treatment challenges. In this light, the author suggests eight operating principles that should be implemented to facilitate an e

Outpatient psychotherapy in the United States, II: Patterns of utilization.

OBJECTIVE: The purpose of this article is to characterize the use of psychotherapy based on episode duration. METHOD: Data were analyzed from the household section of the 1987 National Medical Expenditure Survey. The authors determined the demographic characteristics, provider and reason for visit distribution, mental and physical health status, and expenditures associated with very short-term (one to two sessions), short-term (three to 10 sessions), intermediate-term (11 to 20 sessions), and long-term (> 20 sessions) psychotherapy. RESULTS: Long-term psychotherapy accounted for 15.7% of psy…

Approaches to psychotherapy in Africans: do they have to be non-medical?

Efforts at medical psychotherapy in the Lagos University Teaching Hospital are appraised. Six popular generalisations that have tended to discourage attempts at medical psychotherapy in Africa are presented and illustrated by thirteen vignettes derived from our experience in psychotherapy among Nigerians in the last six years. The vignettes encourage reappraisal of these popular generalisations with a view to modifying them and stimulating more effort by others in the same direction. Methods used are discussed and relevant literature is reviewed.

[Development of the specialty fields of psychiatry and psychotherapy in East Germany].

The development of psychiatry and/with psychotherapy ever since the beginning of the German Democratic Republic after WW2 is described and analysed. The author's main focus is on the confrontation, addition, and cooperation of both the classical medicine of matters and medicine of mind with conflicting, and at least varying paradigmatic reference. The pro and cons on shock therapy are discussed as a crucial point, here. The author holds that shock therapy interferes with the basics of a therapy where the patient is an integral and responsible part of the work as the mode of such treatment anni…

Staff resistance to interpretive psychotherapy in the in-patient community meeting.

The authors propose using the in-patient psychiatry unit community meeting for large-group interpretive psychotherapy, employing the model of Winer & Lewis (1984). This model rests heavily on interpretation of group transferences in the here and now. The staff has the task of discovering patients' veiled references to them and interpreting the patients' resistance to direct expression of their experience of the staff. The authors discuss the impact of introducing the technique upon staff members. Staff members resist this new model in order to defend against self-esteem injury, loss and regres…

[Neurotic disorders in epilepsy and their treatment by technics of psychotherapy and reflexotherapy].

The paper reports on observations of the onset and development of neurotic states in patients (n = 51) suffering from various forms of epilepsy at different stages of the disease. The clinical features of the principal types of neuroses such as neurasthenia, hysteria or neurosis of obsessive states proceeding in the presence of epilepsy are considered and clinical examples are adduced. Recommendations are given on the treatment of such patients using psychotherapy (including hypnotherapy) and acupuncture combined with basic anticonvulsion treatment. Peculiar phenomena resulting from the applic…

Brief psychotherapy in late adolescence: a psychodynamic and developmental approach.

Four cases were discussed in which college students presented themselves in crisis for psychiatric treatment; each was dealing with a normal developmental crisis of late adolescence centering around separation-individuation issues. A type of brief psychotherapy was described which maintained the focus on the primary dynamic conflict and promoted ego development by interpreting the unconscious intrapsychic conflict. The incestuous, aggressive, and regressive themes were explored, and defenses were actively interpreted whenever they interfered with the awareness of autonomous strivings consisten…

[Modular acute psychotherapy-Background, concept and implications for psychiatric and psychotherapeutic training : A consensus paper of the German Society for Psychiatry and Psychotherapy, Psychosomatics and Neurology (DGPPN)].

BACKGROUND: The application of disorder-specific psychotherapy often requires a minimum level of joint treatment, clarification of goals and a therapeutic alliance. In acute psychiatric treatment these prerequisites are not always initially present: admissions can be unplanned or involuntary, diagnostic classification can be uncertain and shared goals and a viable therapeutic alliance first need to be established. OBJECTIVE: The modular acute psychotherapy (MAPT) is presented as an integrative psychotherapeutic concept for the specific requirements of acute psychiatric treatment and its impli…

[Recommendations on performing polygraphy or polysomnography in the fields of psychiatry and psychotherapy : Position paper of the working group on sleep medicine of the German Association for Psychiatry, Psychotherapy and Psychosomatics].

Difficulties in falling asleep and maintaining sleep, nonrestorative sleep and decreased daytime wakefulness represent very common but relatively unspecific health complaints. Around 100 specific sleep-related disorders will be classified in their own major chap. 7 (sleep wake disorders) for the first time in the upcoming 11th version of the International Classification of Diseases (ICD 11). With respect to the disciplines of psychiatry and psychotherapy there is a bidirectional relationship between mental health and sleep wake disorders. Sleep wake disorders can be an independent risk factor …

Assessing the distinctiveness of psychotherapies and examining change over treatment for anorexia nervosa with cognitive-behavior therapy, interpersonal psychotherapy, and specialist supportive clinical management.

OBJECTIVE: Therapist adherence to cognitive-behavior therapy (CBT), interpersonal psychotherapy (IPT), and specialist supportive clinical management (SSCM) for anorexia nervosa (AN), was examined across three phases of therapy in a randomized clinical trial. METHOD: Adherence in early, middle, and late phase therapy sessions from 53 of 56 participants in the trial was assessed using the CSPRS-AN by independent raters after listening to complete therapy sessions. RESULTS: The three forms of psychotherapy were distinguishable by blind raters. Subscale scores were higher for the corresponding t…

Asian American client perspectives on racism-related discussions in psychotherapy.

This study examined how Asian American clients experience and discuss racism-related events in psychotherapy and how therapist responses shape therapeutic outcomes. Eleven Asian American adults who had discussed at least one racism-related incident in therapy completed semistructured interviews, recounting of the racist incident in therapy and the aftermath of those conversations. Interviews were analyzed using Consensual Qualitative Research and linguistic analysis of emotion-word usage. Participants described a range of direct and vicarious racism experiences, most commonly workplace or scho…
