Radiation dose reconstruction is used to estimate exposure to radiation that has occurred externally, e.g., from an atomic bomb, or internally, e.g., from radionuclide ingestion. This commentary reviews techniques for biological dosimetry that have been developed to estimate radiation doses from internal exposures, but which can also be used to estimate external exposures. The author argues for increased development and use of these biological tools.

A retrospective analysis of 160 cases of bladder tumors in females revealed that in 9 of these cases cancer in the bladder arose 1 to 22 years after radiation for uterine and breast cancer. This secondary tumor manifested in 2 females as dysuria, in one of them transition cell cancer of the bladder followed Brunno's cystitis 2 years after the cystitis diagnosis. The first bone marrow transplants (BMTs) in human patients were performed after conditioning with total body irradiation (TBI). TBI remains an important part of BMT protocols. The morbidity and mortality of BMT remains significant, but can be decreased by the introduction of optimized TBI regimens. This requires dosimetric control and a detailed analysis and description of the physics of the TBI procedure in every BMT center that utilizes TBI. Recommendations for such procedures are given. Prospective randomized studies continue to dominate issues of local regional radiation therapy for breast cancer. Detailed micturitional histories and urodynamic studies were performed in five patients with radiation myelopathy. All patients had micturitional symptoms that were irritative in five and obstructive in four, and four had urinary incontinence. Lymphomas are rarely, if ever, found to be in excess following exposure to ionizing radiation. Hodgkin's disease has never been linked to radiation, and the evidence for non-Hodgkin's lymphoma (NHL) is very weak. Epidemiological studies are the only means by which answers can be given as regards the carcinogenic effect of ionizing radiation.
