The origin of lymphatic capillaries in murine testes.

It is known that the seminiferous tubules are bathed in a sea of lymph in mice, which are commonly used in reproductive and immunological studies. Although testicular lymphatic vessels arising from the tunica albuginea can be macroscopically observed in mice, the exact distribution of the lymphatic capillaries remains unclear. In the present study, we investigated the distribution of lymphatic capillaries in normal testes by immunohistochemical staining with monoclonal antibodies against lymph vessel endothelium HA-receptor 1 (LYVE-1) and a platelet endothelial cell adhesion molecule (CD31). Rapid perfusion and network remodeling in a microvascular construct after implantation.

OBJECTIVE: We have previously demonstrated the ability to construct 3-dimensional microvascular beds in vitro via angiogenesis from isolated, intact, microvessel fragments that retain endothelial cells and perivascular cells. Our objective was to develop and characterize an experimental model of tissue vascularization, based on the implantation of this microvascular construct, which recapitulated angiogenesis, vessel differentiation, and network maturation. METHODS AND RESULTS: On implantation in a severe combined-immunodeficient mouse model, vessels in the microvascular constructs rapidly in

Simultaneous pancreas and kidney transplantation: a feasible procedure in selected patients.

We analyzed the overall results of 24 simultaneous pancreas and kidney transplantations (SPK), performed in our hospital between April 1986 and June 1990. All patients had type I diabetes mellitus and end-stage renal failure. We used bladder drainage of the pancreatic exocrine secretions through a duodenocystostomy. The blood vessels of both grafts were anastomosed to the iliac vessels. The immunosuppressive management was triple-therapy with cyclosporin, azathioprine and prednisone. All organs were transplanted without matching donors and recipients for HLA. At the time of transplantation, me

Pedicled Anterolateral Thigh Flap Reconstruction of Exposed Renal Allograft.

Complete abdominal wall dehiscence is an exceedingly rare complication following kidney transplantation. Reconstruction typically consists of abdominally based flaps; however, extensive prior surgeries, ostomies, and limited recipient vessels may necessitate alternative donor sites. This case describes the use of a pedicled anterolateral thigh flap reinforced with vascularized fascia lata to reconstruct a complex abdominal wall defect exposing a renal allograft in a patient with vertebral (V), anogenital (A), cardiac (C), tracheoesophageal (TE), renal (R), and limb (L) system (V.A.C.T.E.R.L.) …

Graft Hepatic Artery Rupture Due to Carbapenem-Resistant Klebsiella pneumoniae Infection After Liver Transplant.

Carbapenem-resistant Klebsiella pneumoniae infection is a major cause of morbidity and mortality after solid-organ transplant and hematopoietic stem cell transplant. Here, we report a 57-year-old man with hepatitis B virus-related decompensated liver cirrhosis, huge splenic artery aneurysm, and hypersplenism who underwent liver transplant from a deceased brain-dead donor. Recipient sputum surveillance showed carbapenem-resistant Klebsiella pneumoniae when he entered the intensive care unit, and combined tigecycline, meropenem, and fosfomycin were administered. At 1 week posttransplant, the rec

In special situations, as for example when unfavorable conditions regarding the recipient vessels exist, primary soft tissue reconstruction with a technically less demanding flap and a longer vascular pedicle (for example m. latissimus dorsi) and secondary free bone grafting may be indicated.

[Free forearm flap as a possibility for simultaneous reconstruction of damaged peripheral circulation and skin].

The free forearm flap has become an accepted procedure for cover of defects in the lower leg due to osteitis. Because the diameter of the donor and recipient artery is equal, end-to-side anastomosis is not without problems and in such cases end-to-end anastomosis is preferred. However, if one or more arteries of the lower leg are absent it may be dangerous to do an end-to-end anastomosis because of the further disturbance of circulation. A new method is shown in which the flap vessels act as an arterial and venous bypass, which improves the circulation in the lower leg, while still perfusing t

Technical considerations in elderly renal allograft recipients.

Atherosclerosis in renal allograft recipients necessitated modification of vascular anastomoses in 62 per cent of patients older than forty years in contrast with 10 per cent in younger patients. Three forms of vascular disease occurred: atherosclerotic occlusion, tortuosity of vessels, and disruption of plaques by vascular clamps. Successful modifications of the procedure and avoidance of ischemic graft injury are described including relocation of the transplanted ureter.

Notation System for Liver Transplant Recipients: Key Information in Transplant Procedure.

Liver transplantation (LT) is a standard therapy for patients with end-stage liver disease and certain hepatic malignancies. The LT procedure has diversified due to donor shortages and advances in immunosuppressive treatments and surgical techniques. Donor organ shortages have promoted the use of living donor liver transplantation and "marginal" livers, including donation after circulatory death. ABO-incompatible liver transplants now show outcomes comparable with ABO-compatible transplants when a desensitization protocol is used. Hepaticojejunostomy, rather than duct-to-duct anastomosis, is p

Pulmonary Rhizopus infection in a diabetic renal transplant recipient.

Infectious complications after renal transplantation remain a major cause of morbidity and mortality. Mucormycosis is a rare infection in renal transplant recipients; however, mortality is exceedingly high. Risk factors predisposing to this disease include prolonged neutropenia, diabetes, and patients who are immunosuppressed (Singh N, Gayowski T, Singh J, Yu LV. Invasive gastrointestinal zygomycosis in a liver transplant recipient: case report and review of zygomycosis in solid-organ transplant recipients, Clin Infect Dis 1995: 20: 617). Life-threatening infections can occur, as this fungus h

Angiographic implications of cardiac transplantation.

Coronary vascular disease in the cardiac transplant recipient has become the third most frequent cause of death or retransplantation after infection and acute rejection. A unique pattern of concentric fibrointimal thickening develops within 1 year of cardiac transplantation; however, it is relatively inapparent on routine arteriography. The disease progresses primarily in distal vasculature, leading to progressive occlusion. Angiographically discrete lesions associated microscopically with advanced atherosclerotic plaques frequently occur in the more proximal vessels often associated with thro

Coronary atherosclerosis in transplanted mouse hearts. II. Importance of humoral immunity.

Obstructive lesions in arterial vessels of transplanted organs constitute an important factor in the late failure of these organs, especially for the heart. The absence of obstructive lesions in syngeneic donor-recipient combinations, suggests that they depend upon recipient immune responsiveness. It is controversial whether humoral or cellular aspects of the immune response predominate in the process. The present experiments employed hearts transplanted between inbred mice. After brief preoperative immunosuppression of recipients with mAbs to CD4 and CD8 determinants, hearts transplanted betw

A Needle Tip Quality-Controlled Training Protocol for Cerebrovascular End-to-Side Bypass: From Bench to Bedside.

BACKGROUND AND OBJECTIVES: Improper techniques in cerebrovascular end-to-side bypass, such as back-wall injury or skiving bites, can lead to stenosis or thrombosis. To address this challenge, we developed a needle tip quality-controlled (TQC) training protocol and evaluated its training value and clinical significance. METHODS: The TQC protocol mandates continuous visualization of the needle tip within the vessel lumen during each stitch, guided by strict suturing principles that prohibit intraluminal blind suturing. An attending neurosurgeon underwent training using the TQC protocol on a rat

Half-tied Stay Suture Technique for Cerebrovascular End-to-side Anastomosis: A Technique to Expand the View of the Hidden Ostium.

In cerebrovascular end-to-side anastomosis, thick, hard donor arteries overlying thin recipient arteries impair the view of the ostium, and may result in occlusion of the anastomosis. To improve the intraoperative view, we modified the stay sutures. After performing standard recipient arteriotomy and placing the first stay suture, we half-tied the second stay suture to leave a loop: half-tied stay suture (HSS). The thread of the HSS was secured with a clip to avoid slippage. For suturing side A, the clip pulling on the HSS was gently moved to the opposite side of side A, i.e. to side B, and th

Long-term outcomes of drug-eluting stents versus bare-metal stents in large coronary arteries.

BACKGROUND: Long-term (>3 years) outcomes of drug-eluting stents (DES) versus bare-metal stents (BMS) in large coronary arteries were not conclusive. In addition, large coronary vessels were defined using a wide size range (≥3.0 mm) in previous studies. The aim of this study was to assess the long-term efficacy of DES versus BMS in subgroups of different vessel sizes. METHODS: A total of 1096 patients (1342 lesions) who underwent either DES or BMS implantation in large coronary vessels was followed for a mean duration of 4.5 years. Patients were divided into 4 subgroups by the reference vesse

Early and Mid-Term Results of Solid Organ Transplantation After Circulatory Death: A 4-Year Single Centre Experience.

Background and Objectives: The use of controlled donation after circulatory death (cDCD) donors has significantly increased during the past decades and successfully expanded the donors' pool. However, warm ischemia may have detrimental effects on graft function. Italian Law requires a no-touch period of at least 20 min, which is much longer compared to the 5 min accepted in most European countries. Materials and Methods This is an Italian single-centre retrospective review of all cDCD procedures performed from April 2021 to June 2025. Patients with severe brain injury undergoing withdrawal of

Successful Case of Somatostatin Analog Stopping Gastrointestinal Bleeding, One of the Most Frequent Complications After Simultaneous Pancreas-kidney Transplantation: A Case Report.

OBJECT: Pancreas transplantation has the highest surgical complication rate of all routinely performed organ transplantation procedures. The complications are not only caused by the pancreas itself but also occur due to issues with the transplant recipient. We report the case of a patient who experienced massive gastrointestinal bleeding after simultaneous pancreas-kidney transplantation (SPK), which was stopped successfully using somatostatin analog. PATIENTS AND METHODS: The patient was a 45-year-old woman with diabetes mellitus type 1 who underwent SPK with enteric drainage. She had melena

Electrothermal Lymphatic Sealing (LigaSure) Versus Conventional Hand-Tie Ligation for Control of Perivascular Lymphatics in Kidney Transplant Recipients: A Mixed Prospective-Retrospective Cohort Study.

BACKGROUND: Ligation of iliac vessel lymphatics is essential for preventing lymphocele in kidney transplant (KT) recipients. Electrothermal bipolar vessel sealer (EBVS) such as LigaSure (Medtronic) may offer technical advantages. This study aimed to evaluate the safety and efficacy of lymphatic sealing using LigaSure in KT recipients compared to conventional hand-tie ligation. METHODS: A mixed prospective-retrospective cohort study was conducted at a single center. The 'hand-tie group' included data collected from 300 KT recipients transplanted between May 2017 and May 2020 using hand-tie lym

Renal arteriolar C4d deposition: a novel characteristic of hematopoietic stem cell transplantation-associated thrombotic microangiopathy.

BACKGROUND: The mechanism of kidney injury in hematopoietic stem cell transplantation (HSCT)-associated thrombotic microangiopathy (TA-TMA) is not completely understood. Renal C4d staining is a marker of classic complement activation and endothelial injury and has been described in preliminary reports of HSCT recipients with TA-TMA. Our objective was to evaluate complement in the pathogenesis of small vessel injury in children receiving HSCT. We hypothesized that kidney tissue from children with TA-TMA would more frequently show C4d deposition compared with HSCT recipients without histologic T

Three-Vessel Anastomosis for Direct Bihemispheric Cerebral Revascularization.

BACKGROUND: In rare, complex cerebrovascular pathologies, patients may benefit from simultaneous revascularization of multiple arterial territories. Traditional strategies for these situations include the use of more than 1 interposition graft, multiple microvascular anastomoses, vessel reimplantations, and staged procedures. OBJECTIVE: To improve upon traditional revascularization strategies by describing a novel 3-vessel microvascular anastomosis. This technique combines a side-to-side and an end-to-side anastomoses to facilitate simultaneous direct revascularization of 2 arterial territori
