Clinical and endoscopic features of non-steroidal anti-inflammatory drug-induced colorectal ulcerations.

BACKGROUND AND PURPOSE: Data on the clinical and endoscopic features of non-steroidal anti-inflammatory drug (NSAID)-induced colorectal ulcerations are very limited from Taiwan. We analyzed the clinical manifestations, endoscopic and histologic findings, and outcomes of patients with NSAID-induced colorectal ulcerations in Southern Taiwan. METHODS: From January 1997 to December 2001, 4200 patients underwent total colonoscopy at a tertiary referral center in Southern Taiwan. We retrospectively analyzed the medical records and endoscopic files. The diagnosis of NSAID colorectal ulceration requi

Pelvic lymphoscintigraphy: contribution to the preoperative staging of rectal cancer.

PURPOSE: Preservation of the anal sphincter in surgery for cancer of the distal rectum in an attempt to avoid colostomy has been a main concern of colorectal surgeons. Various proposed procedures contradict oncological principles, especially with respect to pelvic lymphadenectomy. Therefore, prior knowledge of pelvic lymph node involvement is an important factor in choosing the operative technique, i.e., radical or conservative resection. Introduction of ultrasound, computerized tomography, and magnetic resonance have made preoperative study of the area possible. Nevertheless, these resources

Hybrid laparoscopic flexure takedown and open procedure for rectal resection is associated with significantly shorter length of stay than equivalent open resection.

PURPOSE: Laparoscopic-assisted, sphincter-saving resection (largest incision < 7 cm) of the middle and distal rectum is technically very difficult and, with regard to cancers, has not been demonstrated to be oncologically safe. The hypothesis of this retrospective study is that a hybrid operation that combines laparoscopic and open methods would be associated with short-term outcome benefits compared with open surgery results for patients undergoing sphincter-saving proctectomy. METHODS: A total of 31 hybrid and 25 fully open rectal resection patients were compared in this retrospective revie

Conservative treatment of rectovesical fistula after leakage following laparoscopic radical resection of rectal cancer.

Rectovesical fistula (RVF) is a multifactorial complication that can be caused by different types of laparoscopic surgery, malignant tumor invasion, and chronic inflammation. However, RVF as a postoperative complication of laparoscopic radical rectal cancer is rare and serious. Here, we describe the case of a patient with RVF secondary to pelvic infection that was induced by anastomotic leakage. The patient was managed with conservative treatment, which included double-cavity cannula irrigation-drainage, indwelling balloon urethral catheter, treating the urinary tract infection, partial parent

Recto-cavernosal fistula after radiation for rectal cancer.

Rectourethral fistula is a rare, but documented complication of rectal cancer. To our knowledge this is the first report of a recto- cavernosal fistula after chemo-radiotherapy for cancer of the rectum.

[A case of rectal carcinoma that disappeared following preoperative chemotherapy with an FT-207 suppository--confirmed by a thorough histologic examination of the resected specimen].

Presented is the case of a patient, a 56-year-old female, who had complained of bloody stool and constipation. A barium enema and endoscopic examination revealed a tumor (Type 2) with a crater surrounded by a thick embankment, extending from the anterior wall to the left wall of the lower rectum. Biopsy specimens of the tumor disclosed a well-differentiated adenocarcinoma. A FT-207 suppository (1500 mg/day) was administered preoperatively for 50 days (total dose 75 g). On February 16, 1987, the patient underwent an abdominoperineal excision. The resected specimen took on the appearance of a ch

Chronic perianal fistula: beware of rectal duplication.

A child with cystic duplication of the rectum containing ectopic duodenal mucosa and aberrant pancreatic tissue presenting as a chronic perianal fistula is reported. The duplication was removed by subtotal excision of the cyst along with mucosal sleeve resection from the common septum with the rectal wall.

Surgical anatomy of the pelvis as a guide for the total mesorectal excision technique.

The technique of total mesorectal excision (TME) has become a widely accepted component of rectal cancer resection since its introduction in the 1980s. The quality of TME remains the only way for a surgeon to influence the oncological outcomes of surgical treatment for rectal cancer. A thorough understanding of the surgical anatomy of the pelvis, particularly the pelvic fasciae, vascular supply, and lymphatic drainage of the rectum, is essential for the proper technique of TME. Functional outcomes of rectal resections also depend on meticulous dissection and respect for the anatomy of the pelv

[Expert consensus on modified Bacon procedure (2025 version)].

The modified Bacon procedure is a staged, sphincter-preserving surgical technique for low rectal tumors, which involves transanal or transabdominal division of the tumor, extraction of the specimen via the anus or abdomen, exteriorization and fixation of the proximal colon through the anus, followed by a second-stage resection of the exteriorized colon to restore intestinal continuity. This approach offers advantages such as a reduced risk of anastomotic leakage and operational safety. However, several clinical aspects lack consensus, including indications for the procedure, the optimal length
