Evidence Based Analysis Enhances Surgical Outcomes of Novice Resident Surgeons.

Evidence based practice enhances healthcare delivery and prevents unsafe procedures. While competency based assessments of resident cataract surgery are standard, evidence based analysis of refractive outcomes remains underutilized in educational curricula. This retrospective single center study evaluated refractive outcomes from 21 novice ophthalmology resident surgeons. Three independent groups were compared based on formal constant optimization for intraocular lens (IOL) calculation: non-optimized Haigis (n = 216), a0-optimized (n = 94), and a0/a1/a2-optimized (n = 121). All surgeries were

Unwanted Astigmatism and High-order Aberrations One Year after Excimer and Femtosecond Corneal Surgery.

SIGNIFICANCE: Small incision lenticule extraction (SMILE) is advanced as the most minimally invasive and least traumatic corneal procedure for correcting refractive errors using a single laser. Although SMILE obtains similar results to femtosecond laser in situ keratomileusis (LASIK) with spherical myopia, it has deficiencies in astigmatism correction. PURPOSE: The purpose of this study was to compare refractive outcomes and high-order aberrations (HOAs) between SMILE and femtosecond LASIK corneal procedures at 1 year post-operative. METHODS: Ninety-two patients (181 eyes) with myopia/myopic

[Choice of a surgical correction method in hypermetropic anisotropia in children].

The results of surgical techniques applicable to hypermetropia correction, i.e. lamellar keratotomy (LK), lamellar keratotomy with thermal keratocoagulation (LKTKC) and sub-flap photokeratectomy (LASIK), in 80 children, aged 6 to 14, with hypermetropia ranging from 3.5 to 9.5 diopters and with astigmatism ranging from 0.75 to 2.75 diopters are analyzed. The follow-up varied from 1 to 3 years. LASIK (reduction of astigmatism by 1.61 +/- 0.43 diopters or 78%) was proven to be an optimal method in the correction of astigmatic hypermetropic anisometropia. LASIK (reduction of anisometropia by 3.44

[Epikeratophakia surgery for refractive errors and keratoconus].

Epikeratophakia surgery is a relatively new surgical technique which involves suturing of prelathed corneal tissue on top of the recipient's cornea. The epikeratophakia lens is lathed according to the recipient's refractive needs and therefore can correct myopia and hyperopia; a plano lenticle can be used to correct keratoconus. In the past 3 years we operated on 25 patients (27 eyes). There were 11 cases of myopia, 7 of aphakia and 7 of keratoconus. The aphakic (hyperopic) group included patients with aphakia and traumatic aphakia in whom secondary implantation of an intraocular lens was cont

Excimer laser lamellar keratoplasty of augmented thickness for keratoconus.

PURPOSE: We describe a modified surgical technique for the treatment of patients in the early stage of keratoconus, when it is still possible to correct the astigmatic ametropia with contact lenses. METHODS: Excimer laser lamellar keratoplasty of augmented thickness (ELLKAT) is a procedure in which a deep plano excimer laser ablation is done on the host cornea and a donor lamellar button--with or without an excimer laser refractive ablation on the posterior surface--is sutured into the recipient bed. In 12 of 20 eyes, the homoplastic button, which had parallel faces and was devoid of any diop

[Chinese expert consensus on the prevention and treatment of complications of laser corneal refractive surgery (2024)].

Laser corneal refractive surgery has been carried out in China for three decades. The safety, efficacy, predictability, and long-term stability of the surgery have been well established. In the past 10 years, with the wide use of small incision lenticule extraction (SMILE) and trans-epithelial photorefractive keratectomy (T-PRK), the number of corneal laser surgeries in China has increased significantly. Although severe postoperative complications are rare, there is still the possibility of complications. Once complications occur and are not treated in time and effectively, the visual acuity a

Observation of the effect of posterior scleral reinforcement combined with orthokeratology and 0.01% atropine in the treatment of congenital myopia: a case report.

BACKGROUND: Myopia has recently emerged as a significant threat to global public health. The high and pathological myopia in children and adolescents could result in irreversible damage to eye tissues and severe impairment of visual function without timely control. Posterior scleral reinforcement (PSR) can effectively control the progression of high myopia by limiting posterior scleral expansion, improving retrobulbar vascular perfusion, thereby stabilizing the axial length and refraction of the eye. Moreover, orthokeratology and low concentrations of atropine are also effective in slowing myo

Topography-guided laser refractive surgery.

PURPOSE OF REVIEW: Topography-guided laser refractive surgery regularizes the front corneal surface irregularities to achieve the desired refractive outcome. This is particularly applicable in highly aberrated corneas, where wavefront aberrometry is often not possible. This article aims to review the recently published results of topography-guided ablations in normal regular corneas, highly aberrated corneas, and its application in conjunction with collagen cross-linking (CXL) in cases of keratectasia. RECENT FINDINGS: Topography-guided laser ablation is increasingly used with good efficacy a

Diplopia associated with refractive surgery.

When diplopia occurs after refractive surgery, a systematized approach to diagnosis and treatment is useful. First, determine if the problem is monocular or binocular. Monocular diplopia usually is caused by anterior segment complications and should be referred to an anterior segment surgeon. If the problem is binocular, determine if there is iatrogenic monovision. If monovision was created by the refractive surgery, determine if the double vision is due to fixation switch diplopia. If so, the monovision state needs to be reversed. If fixation switch is not the cause of the symptoms, try "opti

Assessment of true intraocular pressure: the gap between theory and practical data.

A precise assessment of the intraocular pressure (IOP) is crucial for diagnosis and decision making regarding treatment modalities in patients with glaucoma. Recent epidemiologic studies show that a difference of only 1 mm Hg in the mean IOP may be critical enough to determine the visual field prognosis in patients with glaucoma. However, the Goldmann applanation tonometer, which is current gold standard, is not precise enough to measure the true IOP within an error of 1 mm Hg. There are many clinically proposed correction algorithms to correctly measure IOP. However, corrections using only th

Corneal thickness in glaucoma screening, diagnosis, and management.

PURPOSE OF REVIEW: Variability in central corneal thickness (CCT) is a potent confounder of most tonometry techniques, especially Goldmann applanation tonometry. The Ocular Hypertension Treatment Study (OHTS) provided important information regarding predictive factors for the eventual development of glaucoma in patients at risk of the disease. Among the most striking of the OHTS' findings was that CCT was a powerful predictor of glaucoma risk. In this review, studies subsequent to the OHTS report will be reviewed and placed in the context of what is known about CCT and its relation with tonome

Corneal power, axial length-to-corneal radius ratio, and myopia status prior to corneal refractive surgery: a cross-study analysis.

We examined the interplay among corneal refractive power, the axial-length-to-corneal radius of curvature ratio (AL/CR), and myopia severity in patients before corneal refractive surgery. We included 1208 myopic patients with a median age of 22 years (P25 = 19, P75 = 27) and a preoperative spherical equivalent (SE) of - 4.88 D (P25 =  - 6.50, P75 =  - 3.50). Myopia status was classified using the spherical equivalent refraction into low, moderate and severe myopia. Key measurements included corneal curvature radius (CR), flat (K1) and steep (K2) corneal curvatures, anterior corneal surface ast
