[Diaphragm disorders in adults].

Diaphragm disorders in adults. Diaphragm diseases, functional particularly, are little known and often underestimated by clinicians. Whether a fortuitous discovery on a chest x-ray showing an elevation of the hemidiaphragm or revealed by dyspnea, these abnormalities of the diaphragm require further investigations. The objective is to confirm the diagnosis, understanding the mechanism, to clarify the functional consequences and to consider treatment. Some dysfunctions may be temporary, contraindicating any emergency treatment apart from acute ruptures. Only symptomatic cases require a surgical

Unusual complications of empyema thoracis: diaphragmatic palsy and Horner's syndrome.

We report here a 3 month old child with empyema thoracis, who developed complications of diaphragmatic palsy and Horner's syndrome. These complications of empyema thoracis have not been reported earlier. We discuss the possible mechanisms for these complications.

[Chronic sclerosing mediastinitis: infrequent cause of unilateral diaphragmatic paralysis].

Chronic sclerosing mediastinitis (CSM) is a rare disease whose etiology varies and which usually develops through mediastinal compression syndromes, generally due to occlusion of the superior vena cava. We report a case of CSM diagnosed after a chance finding of unilateral diaphragm paralysis, a circumstance not previously reported in the literature. We review the clinical, radiological and histological features of CSM.

Diaphragmatic paralysis in motor neurone disease: use of non-invasive investigative and therapeutic techniques.

Two cases of bilateral diaphragmatic weakness are described in which the condition was the presenting feature of motor neurone disease. Inspiratory muscle strength was assessed by a non-invasive technique involving measurements of pressures generated within the mouth. One patient with severe inspiratory muscle weakness is being treated with domiciliary nasal ventilation and has returned to a good-quality life. The other patient with less severe weakness has thus far required no ventilatory support.

Familial bilateral paralysis of diaphragm. Adult onset.

At age 50 two homozygote twin brothers developed bilateral paralysis of the diaphragm. No infectious, metabolic, degenerative or proliferative disorders of the neuromuscular system and no thoracic diseases which could explain this lesion were detected during four years of follow-up. It appears thus to be an isolated lesion of genetic origin. To the best of our knowledge, no similar cases have been reported in literature.

High Positive End-Expiratory Pressure Renders Spontaneous Effort Noninjurious.

RATIONALE: In acute respiratory distress syndrome (ARDS), atelectatic solid-like lung tissue impairs transmission of negative swings in pleural pressure (Ppl) that result from diaphragmatic contraction. The localization of more negative Ppl proportionally increases dependent lung stretch by drawing gas either from other lung regions (e.g., nondependent lung [pendelluft]) or from the ventilator. Lowering the level of spontaneous effort and/or converting solid-like to fluid-like lung might render spontaneous effort noninjurious. OBJECTIVES: To determine whether spontaneous effort increases depe

[Robotic Diaphragm Plication].

Acquired unilateral hemidiaphragm elevation is characterised by dyspnoea, which is typically aggravated when lying down, bending over or during swimming. The most common causes are idiopathic or due to injury to the phrenic nerve during cervical or cardio-thoracic surgery. To date, surgical diaphragm plication remains the only effective treatment. The aim of the procedure is to plicate the diaphragm to restore its tension and thus improve breathing mechanics, increase the available space for the lung and reduce compression from abdominal organs. In the past, various techniques using open and m

Thoracoscopic Radial Diaphragm Plication.

In appropriately selected patients diaphragm plication improves quality of life by alleviating dyspnea and allowing patients to return to their routine activities. Many plication techniques exist, but the optimal surgical approach remains unclear. We report our experience with a minimally invasive radial diaphragm plication technique. It offers 2 distinct advantages: (1) suture placement avoids the phrenic nerve fibers, allowing for potential nerve recovery, and (2) the interrupted radial sutures improve the distribution of tension along the flaccid muscle and may achieve a more durable repair

Fifty years of recurring struggles with recurrent laryngeal neuropathy.

Recurrent laryngeal neuropathy appears to be a simple problem that should have a simple solution, yet the complexity and dynamic nature of laryngeal function is underappreciated. This review highlights the challenges and accomplishments that work towards that greater understanding of what is necessary to find a successful solution.

[Treatment with Bi-PAP in patients with bilateral diaphragm paralysis].

Two patients with bilateral diaphragmatic paralysis are described. The first case occurred secondary to open chest surgery; the second occurred in a polytraumatized patient. Both were successfully treated with intermittent bi-level positive airway pressure (Bi-PAP). Bilateral diaphragmatic paralysis can be related to a variety of processes, although idiopathic forms also occur. Diagnosis can be difficult and should involve a high level of clinical suspicion. Treatment and prognosis are determined by the underlying disease. Some cases may require the establishment of nighttime support ventilati

Neuralgic amyotrophy: A rare cause of bilateral diaphragmatic paralysis.

Neuralgic amyotrophy, also known as brachial neuritis, is a well described clinical entity. Diaphragmatic dysfunction, as a result of phrenic nerve root involvement (cervical roots 3 to 5), is an uncommon, but increasingly recognized association. The case of a previously healthy 61-year-old woman who, after a prodrome of neck and shoulder discomfort, presented with severe orthopnea is described. Pulmonary function and electrophysiological studies led to a diagnosis of bilateral diaphragmatic paralysis. The patient's clinical course and the exclusion of other nerve entrapment syndromes and neur

[Diaphragm paralysis as a complication of internal jugular vein puncture].

Rare complications of using the internal jugular vein for vascular access are related to puncture of neighboring organs or other structures. We report the atypical case of a 55-years-old woman with unilateral diaphragm paralysis attributed to accidental damage of the phrenic nerve during an attempt to canalize the internal jugular vein. Other more common causes were ruled out. The severe restrictive ventilatory changes produced had only minor clinical and gasometric repercussions, as previous function was normal and no acute or chronic respiratory disease was present. When ventilation is alrea
