17 degrees Celsius body temperature--resuscitation successful?

The resuscitation of patients with accidental profound hypothermia is challenging. A 17-year-old man got lost on the first of January, after a New Year's Eve party in the foothills of the Alps. After a search of four hours, he was found unconscious with fixed pupils, a Glasgow Coma Scale of three points, and a body temperature below 20° Celsius. There were no signs for traumatic injuries. Initial electrocardiogram (ECG) showed no heart activity. Basic life support was begun by the mountain rescue service and continued by the medical helicopter team. The patient was transferred under continuous…

Consequences of neonatal resuscitation with supplemental oxygen.

There has been considerable controversy surrounding the optimal inspired oxygen concentration for resuscitation of term and preterm infants. We have developed a rat pup model to quantify both physiologic and biochemical parameters associated with normoxic vs. hyperoxic resuscitation. We have confirmed existing human data that hyperoxic resuscitation of rat pups is associated with a significant delay in onset of spontaneous respiratory efforts. Both 40% and 100% inspired oxygen delayed onset of respiratory activity when compared to 21% oxygen. We have also documented, in the rat pup model, that…

Paediatric cardiopulmonary resuscitation: recent changes to guidelines.

Recommended cardiopulmonary resuscitation guidelines for infants and children by health-care personnel have been updated following an extensive review of the science of resuscitation. Notable changes include a change to a single chest compression-to-ventilation ratio of 15:2, a single DC shock (2 J/kg) strategy for shockable rhythms followed by immediate uninterrupted cardiopulmonary resuscitation for 2 min, use of amiodarone (5 mg/kg) for DC-shock resistant rhythms, the routine use of carbon dioxide detection to confirm correct endotracheal tube placement and the use of therapeutic hypothermi…

Cerebrovascular resuscitation after polytrauma and fluid restriction.

BACKGROUND: There are few reproducible models of blast injury, so it is difficult to evaluate new or existing therapies. We developed a clinically relevant polytrauma model to test the hypothesis that cerebrovascular resuscitation is optimized when intravenous fluid is restricted. STUDY DESIGN: Anesthetized swine (42+/-5 kg, n=35) received blasts to the head and bilateral chests with captive bolt guns, followed by hypoventilation (4 breaths/min; FiO(2)=0.21). After 30 minutes, resuscitation was divided into phases to simulate typical prehospital, emergency room, and ICU care. For 30 to 45 min…

Resuscitation of traumatic shock: a hemodynamic review.

Shock, or tissue hypoperfusion, is a frequent complication from traumatic injury. Despite the etiology of the shock state, there is always some component of hypovolemia. The body's innate ability to compensate for impaired perfusion may mask clinical signs, leading to delays in treatment. This article presents an overview of these compensatory mechanisms and resuscitation strategies from the vantage point of routine hemodynamic monitoring.

[Neonatal resuscitation in delivery room: new advances].

The need for resuscitation of a distressed newborn in delivery room is more and more easily predictable. The two principal reasons are improvement of obstetrical survey and best perinatal regionalisation. Perinatal asphyxia and premature labour, especially before 32 weeks of gestational age, are the more frequent situation needing resuscitation at birth. A good survey of pregnancy and labor, verification of availability and efficiency of care devices and material in the delivery room are essential. In all guidelines respiratory resuscitation is today the priority in the first minutes. Non inva…

Resuscitation and emergency management for neonatal foals.

Early intervention can dramatically alter outcome in foals. Cardio-pulmonary cerebral resuscitation can be successful and clinically worthwhile when applied to foals that arrest as part of the birthing process. Readily available equipment and an ordered plan starting with addressing the respiratory system (airway and breathing) followed by the circulatory system (circulation and drugs) are the keys to success. Hypoglycemia is common in foals that are not nursing and in septic foals. Support of serum glucose can be an important emergency treatment. Respiratory support with oxygen therapy should…

Ex vivo resuscitation of adult pig hearts.

One possible way to expand the human heart donor pool is to include non-heart-beating human donors. To begin validating this approach, we developed an ex vivo cardiac perfusion circuit to support large mammalian hearts in Langendorff mode and beating-ejecting mode and to assess and improve their ischemic tolerance. In vivo hemodynamic data and heparinized blood (4.0 +/- 0.5 L) were collected from 6 anesthetized pigs. Hearts were isolated and connected to a recirculating perfusion circuit primed with autologous buffered blood (pH, 7.40). After retrograde aortic perfusion in Langendorff mode, th

Myocardial protection during resuscitation from cardiac arrest.

PURPOSE OF REVIEW: Successful treatment of cardiac arrest requires that an electrically stable and mechanically competent cardiac activity be promptly reestablished. However, many interventions used to attempt to reestablish cardiac activity may also inflict additional myocardial injury and, in turn, compromise resuscitability. In this review, we examine mechanisms of such myocardial injury and discuss potential new strategies for myocardial protection during resuscitation from cardiac arrest. RECENT FINDINGS: Efforts are currently directed at understanding underlying mechanisms of myocardial…

Intrauterine resuscitation: active management of fetal distress.

Acute fetal distress in labour is a condition of progressive fetal asphyxia with hypoxia and acidosis. It is usually diagnosed by finding characteristic features in the fetal heart rate pattern, wherever possible supported by fetal scalp pH measurement. Intrauterine resuscitation consists of applying specific measures with the aim of increasing oxygen delivery to the placenta and umbilical blood flow, in order to reverse hypoxia and acidosis. These measures include initial left lateral recumbent positioning followed by right lateral or knee-elbow if necessary, rapid intravenous infusion of a l…

Advances in fluid resuscitation of hemorrhagic shock.

The optimal fluid for resuscitation in hemorrhagic shock would combine the volume expansion and oxygen-carrying capacity of blood without the need for cross-matching or the risk of disease transmission. Although the ideal fluid has yet to be discovered, current options are discussed in this review, including crystalloids, colloids, blood and blood substitutes. The future role of blood substitutes is not yet defined, but the potential advantages in trauma or elective surgery may prove to be enormous.

Current concepts in cardiopulmonary resuscitation in adults.

Cardiopulmonary resuscitation (CPR) provides artificial circulation and ventilation during cardiopulmonary arrest. CPR is further categorised as basic life support (BLS), advanced cardiac life support (ACLS) and postresuscitation support. BLS consists of provision of a patent upper airway, ventilation and circulation of blood by closed chest cardiac compressions. ACLS includes use of specialised equipment to maintain the airway, early defibrillation and pharmacologic therapy. Successful outcome from an arrest depends on the total duration of an arrest and early defibrillation, as ventricular f…

Logistics of parenteral fluids in battlefield resuscitation.

The paper discusses the substantial reduction in weight and volume of the fluids of resuscitation that is possible and desirable on the basis of sound physiology and the vast experience of the U.S. Army in four major wars in this century. We note the major shift in emphasis from massive colloid and whole blood in World War II and Korea to massive crystalloid and packed cells in Vietnam and the serious complications with which this was associated. These complications were edematous in nature and best known as the Da Nang lung, or adult respiratory distress syndrome, multiorgan dysfunction syndr…

Outcome of resuscitation following unexpected apparent stillbirth.

There are few data to inform a decision to resuscitate babies who are unexpectedly stillborn. The outcome for 42 successfully resuscitated stillborn children, of whom 62% survived to be discharged home, is reported. Of the survivors, a poor outcome with severe disability was found in 23% (including one postneonatal death), equivocal outcome was found in 15% (two mild hypertonia; two with mild hemiplegia and no associated other disability) and 62% were free of any impairment at follow up 20 months to 8 years later. In 39 (93%) fetal problems had been identified and the resuscitation team was pr…

Neonatal resuscitation using the laryngeal mask airway.

BACKGROUND: For a newborn requiring positive-pressure ventilation (PPV), the American Heart Association recommends bag-and-mask ventilation, a challenging procedure for those inexperienced in neonatal resuscitation. The objective of this prospective study was to evaluate the laryngeal mask airway (LMA) as an alternative method of airway management in neonates requiring PPV at birth. METHODS: With the approval of the institutional ethics committee, consent was obtained from women in labor at a tertiary care-perinatal center. Inclusion criteria consisted of an expected birth weight of at least …

Intra-abdominal complications following prolonged cardiopulmonary resuscitation.

A patient who underwent prolonged cardiopulmonary resuscitation developed an acute abdomen in the early post-arrest period which required laparotomy. Operative findings were of a perisplenic haematoma and infarction of the caecum. The latter has not previously been reported in this setting. A review of the literature reveals that many abdominal complications may occur following external cardiac massage. Difficulties in investigation and management are discussed.
