Injury of the Ascending Reticular Activating System in Patients With Fatigue and Hypersomnia Following Mild Traumatic Brain Injury: Two Case Reports.

We report on patients with post-traumatic fatigue and hypersomnia who showed injury of the lower portion of the ascending reticular activating system (ARAS) between the pontine reticular formation (RF) and the intralaminar thalamic nucleus (ILN) following mild traumatic brain injury (TBI), using diffusion tensor tractography (DTT).Two patients with mild TBI resulting from a car accident were enrolled in this study. Patient 1 was a 51-year-old woman showed abnormalities as 6.9 (cut off: 3.7 points) and 18 (cut off: 10) on the Fatigue Severity Scale and the Epworth Sleepiness Scale at 11 months …

Somnolence and stuttering as the primary manifestations of a midbrain stroke.

BACKGROUND: Stroke can occasionally manifest with non-lateralizing findings such as somnolence and stuttering. We describe a case and discus … the diencephalic-mesencephalic junction can manifest with stuttering. Defective projections of the reticular formation to the supplementary motor area, damaged extrapyramidal circuits, and/or aberrant propioceptive feedback due to involvement of the mesencephalic nucleus of the trigeminal nerve are the proposed pathophysiological mechanisms. Somnolence can also be p …

Distribution of somatostatin-28 (1-12) immunoreactivity in the diencephalon and the brainstem of the dog.

The term somatostatin refers to a family of peptides, mainly somatostatin-14, somatostatin-28 and somatostatin-28 (1-12), which are the clea … he brachium inferior colliculus, the retrorubral area, the dorsal raphe nucleus, the myelencephalic reticular formation and the dorsal motor nucleus of the vagus. Less dense population of somatostatin cells were localized in other diencephalic and brainstem nuclei. The distribution of labeled fibers was even broader as in addition to those above mentioned there were …

Eight and a Half Syndrome: A Rare Case of Pontine Infarction.

INTRODUCTION: This case report describes "one-and-a-half" syndrome with supranuclear facial palsy. CASE: 39 years old male hypertensive patient presented with right facial nerve palsy, internuclear ophthalmoplegia and conjugate gaze palsy. Neuroimaging showed acute posterior pontine infarct at medial longitudinal fasciculus, paramedian pontine reticular formation, abducens nerve nucleus and seventh nerve fascicular zone. OBSERVATION: "One-and-a-half"syndrome is characterized by a lateral gaze palsy in one direction and internuclear ophthalmoplegia in the other. Our patient had Right sided on…

Electrical stimulation of the midbrain central gray facilitates lateral vestibulospinal activation of back muscle EMG in the rat.

Electrical stimulation in the midbrain central gray in urethane-anesthetized female rats increased responses of the deep back muscles lateral longissimus and medial longissimus to stimulation of the lateral vestibular nucleus (LVN). During central gray stimulation, LVN stimuli led to larger muscle responses, recruitment of new motor units, and decreased latency of muscle response. Effective central gray sites are hypothesized to act through axons descending to medullary reticular formation. Results are consistent with participation of these neuronal groups in the activation of lordosis behavio…

Identification of the location of vestibular lesions on the basis of vestibulo-ocular reflex measurements.

Quantitative analysis of electro-oculographic recordings of eye movement in response to precise visual and vestibular stimuli makes possible the differentiation of three categories of vestibular syndromes due to pathological changes in three different parts of the visual vestibulo-ocular reflex arc: (1) decreased vestibulo-ocular reflex gain (e.g., decrease in slow component velocity), but normal fast components and visual-vestibular interaction (labyrinth and eighth nerve); (2) normal slow component velocity but abnormal fast components to all stimuli (pontine or medullar reticular formation)…

Mitochondrial Differentiation during Spermatogenesis: Lessons from Drosophila melanogaster.

Numerous diseases can arise as a consequence of mitochondrial malfunction. Hence, there is a significant focus on studying the role of mitochondria in cancer, ageing, neurodegenerative diseases, and the field of developmental biology. Mitochondria could exist as discrete organelles in the cell; however, they have the ability to fuse, resulting in the formation of interconnected reticular structures. The dynamic changes between these forms correlate with mitochondrial function and mitochondrial health, and consequently, there is a significant scientific interest in uncovering the specific molec

Mesencephalic neuronal populations: new insights on the ventral differentiation programs.

The midbrain is a complex structure where different functions are located. This formation is mainly involved in the visual and auditory info … ch as the oculomotor complex, the dopaminergic substantia nigra and the ventral tegmental area, the reticular formation and the periacueductal grey. Knowledge of the genetic cascades involved in the differentiation programs of the diverse populations will be extremely important to understand not only how the midbrain develops, but how degenerative pathologies, such a …

[Neurobiology of trigeminal pain].

The brainstem trigeminal complex integrates somatosensory inputs from orofacial areas and meninges. Recent studies have shown the existence of a double representation of pain within the brainstem, at the level of both caudalis and oralis subnuclei. Noxious messages are mainly conveyed by C-fibers that activate the subnucleus caudalis neurons. These neurons in turn activate the subnucleus oralis whose neurons share similar features with the deep spinal dorsal horn neurons. In contrast with the nearness of the laminar organization of the dorsal horn, the vertical organization of the trigeminal c

Dorsal vagal nucleus involvement relates to QTc-prolongation after acute medullary infarction.

BACKGROUND: Infarction of the medulla has been associated with prolongation of the QTc, severe arrhythmia, and sudden cardiac death, yet the … he maximum lesion overlap included the medial aspect of the gigantocelluar reticular nucleus of the reticular formation. CONCLUSION: We found that the DVN is a key anatomical substrate related to QTc-prolongation. Further studies with more patients and high-resolution, volumetric MRI are needed to confirm our findings.

Our aim wa … gative BOLD in the lateral and mesial parietal lobe, caudate nuclei, and additionally the brainstem reticular formation. The event-related time course differed between the thalamus, the parietal cortex, and the pons and caudate nuclei. Small discrete or multiple punctate areas of hyperintense signal (HS) in the reticular formation, putamen, medial leminiscus, or white matter tracts were imaged in 55% of the subjects reporting high fatigue and in none of those reporting low fatigue. Enhanced activity of the activating and deactivating centers, to a different extent in different animals, would produce restlessness in some animals and sedation in others, accompanied by a mixture of fast and slow waves in the EEG. …

Myoclonus.

Myoclonus is defined as sudden, brief, shocklike, involuntary movements caused by muscular contractions or inhibitors. Myoclonic movements have now been recognized to have many possible variants and pathophysiologic features. Myoclonus may arise from several sites within the neuraxis, of which the cortex and brain stem reticular formation are the most common. An etiologic classification scheme and electrodiagnostic tests are useful for clinical purposes. Therapy is limited and usually involves symptomatic treatment with valproic acid or clonazepam. Careful attention to the basic characteristic…

Periaqueductal microdysplasia in children with congenital aqueductal stenosis.

The brainstems of children with aqueductal stenosis (AS) and controls were examined histologically and immunohistochemically. In the periaqueductal area of children with AS, there was astrogliosis, loss of neurons (tyrosine-hydroxylase (TH) reactivity) and decreased myelination (myelin basic protein reactivity). In locus coeruleus, there was a decreased neuronal cell density (TH-positive neurons) in those with AS. In other areas of the brainstem (medial reticular formation), there appeared to be more TH and substance P reactive neurons. These alterations (dysplasia of slight degree) in neurons…
