Exacerbation of Hyperbilirubinemia by Falciparum Malaria in a Patient With Coexisting Gilbert's Syndrome and Glucose-6-Phosphate Dehydrogenase Deficiency.

Gilbert's syndrome and G6PD deficiency are common genetic disorders. They both give rise to unconjugated hyperbilirubinemia through differen … a month, he still exhibited chronic unconjugated hyperbilirubinemia with recurrent elevation in his reticulocyte count but no anaemia, suggesting further episodes of compensated hemolysis. We also discuss the differential diagnosis for unconjugated hyperbilirubinemia in relation to this interesting case.

Severe Presentation of Congenital Hemolytic Anemias in the Neonatal Age: Diagnostic and Therapeutic Issues.

Congenital hemolytic anemias (CHAs) are a group of diseases characterized by premature destruction of erythrocytes as a consequence of intrinsic red blood cells abnormalities. Suggestive features of CHAs are anemia and hemolysis, with high reticulocyte count, unconjugated hyperbilirubinemia, increased lactate dehydrogenase (LDH), and reduced haptoglobin. The peripheral blood smear can help the differential diagnosis. In this review, we discuss the clinical management of severe CHAs presenting early on in the neonatal period. Appropriate knowledge and a high index of suspicion are crucial for a…

[Successful treatment of pure red cell aplasia with cyclosporin in a patient with T-cell large granular lymphocytic leukemia harboring the STAT3 D661V mutation].

T-cell large granular lymphocyte (T-LGL) leukemia is a chronic T-cell monoclonal disease that is occasionally associated with pure red cell aplasia (PRCA). A 71-year-old previously healthy man complained of physical fatigue and exhibited anemia (hemoglobin, 10.5 g/dl) with lymphocytosis (76%) showing LGL. The LGL cells expressed CD3, CD7, CD8, and T-cell receptor (TCR) -α/β. TCR-β/γ gene rearrangement was positive. He was thus diagnosed with CD8+ T-LGL leukemia. Anemia progressed with low reticulocyte count (0.11%), and the patient became blood transfusion-dependent, but no distinct abnormalit…

The unusual suspects: a curious case of acute kidney injury-Questions.

A previously healthy 8-year-old boy presented for evaluation of three weeks of daily fevers to 101 °F, myalgia, gingival swelling, mouth sores, blurry vision, and periorbital rash with new onset of dark urine. His family history was significant for rheumatologic disease without known renal involvement. Initial workup demonstrated mild anemia with hemoglobin of 10.9 g/dL, low reticulocyte count, few schistocytes on peripheral smear, thrombocytopenia with platelets of 8000/mm^3, albumin 2.5 g/dL, AST 2084 IU/L, ALT 787 IU/L, GGT 223 IU/L, creatine kinase of 10000 IU/L, ferritin of 4290 ng/mL, fi…

Elderly female with Autoimmune hemolytic anemia.

Autoimmune hemolytic anemia (AIHA) is a rare disease with an estimated prevalence of around 17/100,000. It is often difficult to diagnose and treat AIHA, especially in elderly. A 60-year-old female was admitted with the complaints of low grade fever, on-off for 6 months, progressive fatigue and dyspnea on exertion. She was transfused with three units of blood within these 6 months. Examination revealed pallor, edema, hemic murmur, and palpable liver. Hb was 2.9 gm%, T Bil 5.2 mg/dl, ESR 160 mm, and reticulocyte count 44.05%. Direct Coombs test was positive, anti-nuclear antibody (ANA) and Anti…

Twin anemia polycythemia sequence from a prenatal perspective.

OBJECTIVES: To describe the prevalence, management and outcome of spontaneous twin anemia polycythemia sequence (TAPS) diagnosed in the prenatal period. METHOD: Retrospective analysis of 142 consecutive monochorionic twin pregnancies not diagnosed with twin to twin transfusion syndrome. TAPS cases were identified based on the presence of discordant middle cerebral artery peak systolic velocity (MCA-PSV) measurements and signs suggestive of a chronic intertwin transfusion imbalance: either an elevated reticulocyte count in the anemic twin or the presence of few small unidirectional anastomoses…

In homozygous sickle cell patients, the G6PD status was assessed after correction of reticulocyte count following the micro-centrifugation method of Herz. We found that prevalence of G6PD deficiency was higher in sickle cell disease patients (21.6 %) than in normal subjects (12.3 %) (p = 0.001). No difference was found in the two groups of male sickle …

Anemia in children.

Anemia in children is commonly encountered by the family physician. Multiple causes exist, but with a thorough history, a physical examinati … mia and lead poisoning. Normocytic anemia has many causes, making the diagnosis more difficult. The reticulocyte count will help narrow the differential diagnosis; however, additional testing may be necessary to rule out hemolysis, hemoglobinopathies, membrane defects and enzymopathies. Macrocytic anemia may be caused by a deficiency of folic acid and/or vitamin B12 …

Clastogenicity and mutagenicity of hexavalent chromium in lacZ transgenic mice.

The clastogenic and mutagenic effects of the hexavalent chromium compound K2CrO4 in lacZ transgenic mice (Muta Mouse) were investigated. Male Muta mice were administered an intraperitoneal dose of 40 mg/kg of K2CrO4 once on each of 2 consecutive days. The K2CrO4 induced a significant increase in the peripheral blood micronucleated reticulocyte count. Also, K2CrO4 induced a statistically significant increase in mutant frequency in the liver but not in the bone marrow on day 7 after the second treatment. The reason for the failure to increase the mutant frequency in the bone marrow may have been…

Delayed appearance of markers of intravascular haemolysis in a case of paroxysmal cold haemoglobinuria.

The aetiology of haemolytic disease is diverse and the diagnosis often relies on laboratory testing. We describe a case of intravascular haemolysis, which illustrates that significant intravascular haemolysis can occur in the absence of any abnormal haematological findings. Despite gross haemoglobinuria at presentation, the haemoglobin and reticulocyte counts were both within reference limits and a normal blood film was observed. Subsequently, acute tubular necrosis occurred secondary to haemolysis, and acute renal failure was evident by day 2. However, the haemoglobin decreased slowly reachin…

Hematologic effects of antipyretic analgesics. Drug-induced agranulocytosis.

Drug-induced agranulocytosis may be type I (involving the drug, antibodies and neutrophils), type II (associated with accumulated drug toxicity in hypersensitive persons), or type III (representing different etiologies induced by immune and toxic mechanisms). The pyrazolones (amidopyrine, dipyrone and butazones), phenothiazine derivatives, antithyroid drugs, and antibiotics are thought to be causative agents in agranulocytosis. The symptoms may involve sudden onset of high fever, sore throat with ulcerative angina, or stomatitis. Diagnosis of agranulocytosis is confirmed by severe granulocytop

Hereditary spherocytosis: a study of splenectomized persons.

Sixty-one persons splenectomized for hereditary spherocytosis have been studied and compared to 36 nonsplenectomized clinically affected and to 77 nonaffected family members. Serum IgM levels were reduced by 25% and IgD levels were nearly doubled in the splenectomized group. No fatal bacterial infection was identified by history covering a total of 800 postsplenectomy years. Platelet counts remained elevated for many years after removal of the spleen, as did the number of leukocytes, lymphocytes, and monocytes. Heinz bodies were seen in 3% of red cells after splenectomy. Though within the rang

Post-transfusion changes in serum hepcidin and iron parameters in preterm infants.

BACKGROUND: Packed red blood cell transfusion is common in preterm neonates. Hepcidin acts as a negative feedback iron regulator. Iron parameters such as immature reticulocyte fraction (IRF) and high-light-scatter reticulocytes (HLR) are used to clarify iron metabolism. Very little is known about the regulation of hepcidin in preterm infants because most reports have evaluated prohepcidin. The aim of this study was therefore to evaluate serum hepcidin and establish hematological parameters in preterm infants after transfusion. METHODS: The subjects consisted of 19 newborns (10 boys) with mean
