Retinal arterial macroaneurysm with supravalvular pulmonic stenosis (RAMSVPS), also known as familial retinal arterial macroaneurysm syndrome, is an autosomal recessive disorder associated with many life-threatening complications. The disease is characterized by progressive multiple retinal arterial macroaneurysms, retinal arterial beading, and recurrent vitreous hemorrhage. Here, we describe the first case of RAMSVPS associated with peripheral ischemia evident on ultra-widefield fluorescein angiography. A 23-year-old otherwise healthy woman presented to our ophthalmology emergency room with a

BACKGROUND: In this case report, we present for the first time central retinal artery occlusion (CRAO) and central retinal vein occlusion (CRVO) as a complication of persistent hyaloid artery (PHA).
CASE PRESENTATION: In August 2019, a six-year-old male patient manifested right eye (RE) excessive tearing, conjunctival injection and pain. On examination, RE demonstrated light perception and intraocular pressure of 36 mmHg. The diagnoses of neovascular glaucoma, CRVO and CRAO were established as affirmed with fluorescein angiography (FA). PHA was not reported. Extensive work-up and family histo…

BACKGROUND: Retinal artery occlusion (RAO) is caused by embolic occlusion of retinal arteries. Previous strokes in the medical history were reported in up to 20% of patients. According to data from a computational blood flow analysis based on magnetic resonance imaging of a patient with RAO, about 90% of ascending emboli are washed into the brain, leaving speculations about further embolic sources for RAO, such as plaques or stenosis of the ophthalmic artery. The aim of this study was to examine the reduced caliber of the ophthalmic artery in magnetic resonance angiography (MRA).

Retinal arterial occlusion causes acute, painless vision loss, and it requires immediate emergency care. There are two separate arterial systems (retinal and ciliary) in the retina, and in most cases only the central retinal artery and its branches supply blood to the inner retinal layers. Cilioretinal artery is an anatomical variant, which can also supply blood to the macula from the ciliary arterial system, and in the case of a retinal arterial occlusion, the cilioretinal artery could save central vision. We report a case of a 67-year-old woman who suffered a central retinal arterial occlusi…

A 15-year-old boy underwent neuroradiologic embolization of the left internal maxillary artery with polyvinyl alcohol to stop traumatic epistaxis after failure of surgical clipping and nasal packing. Selective catheterization of the external carotid artery before embolization showed a faint choroidal blush. Although the procedure provided hemostasis, embolization to the central retinal artery and ciliary arteries resulted in loss of vision. The route of the emboli to the eye was via the anastomotic network of the lacrimal artery supplied by the external carotid artery system. Neuroradiologic e…

OBJECTIVE: To determine whether retinal arterial wall reflectivity is associated with future cardiovascular disease (CVD) in a large community-based Japanese cohort.
DESIGN: Community-based cohort study.
PARTICIPANTS: A total of 6264 adults from the Nagahama Study (baseline 2012-2016) without a history of CVD.
METHODS: Retinal arterial wall reflectivity was measured on peripapillary OCT images at baseline. Participants were followed for approximately 7 years, and incident CVD events, including stroke and coronary artery disease (CAD), were identified through medical records. Associations we

OBJECTIVE: Ocular symptoms and signs often herald hemispheric neurological events associated with extracranial cerebrovascular disease. However, the presence of a Hollenhorst plaque (HP) or retinal artery occlusion (RAO) and the risk of stroke is unclear. The purpose of this study was to review the outcomes of all patients who presented with a HP or RAO at a single institution.
METHODS: Between 2000 and 2005, the management and outcome of 130 consecutive patients with a diagnosis of HP, central RAO, or branch RAO (ICD-9 codes 362.30 to 362.33) were reviewed. Patients with transient monocular …

To assess the diagnostic value of an extensive cardiac screening and of carotid artery duplex scanning in patients suspected of suffering from retinal embolism, we examined 41 consecutive patients (mean age 59.6 years, range 36-74) who presented either with amaurosis fugax or with a retinal artery occlusion. In spite of extensive investigations, we found no cause in 27 patients (66%). In 11 patients (27%), symptoms were likely to be due to a stenosis or an occlusion of the ipsilateral carotid artery. In only 1 patient (2%), the heart was likely to be a source of embolism. We conclude that in p…

We report a case of central retinal artery occlusion following a minor ocular injury. The occlusions are an uncommon but well following some surgical operations on the eyeball, the eyelids or the nose. They probably result from an arterial spasm which trigger mechanisms are not very well known.

Patients with retinal artery or vein obstruction received 150 mg of bifemelane hydrochloride daily for 1 to 11 months and other treatment (urokinase, steroid hormone, hyperbaric oxygen, or laser photocoagulation) or the latter without bifemelane. After treatment with bifemelane, improvements in visual acuity were seen in 12 of 17 eyes with central retinal artery obstruction, in three of four eyes with central retinal vein obstruction, and in five of nine eyes with branch retinal vein obstruction. Among the patients not receiving bifemelane, improvements were seen in two of six patients with ce…

A case control study was conducted to identify the systemic and ocular risk factors for retinal arterial macroaneurysms. Forty-three patients with 52 photographically confirmed macroaneurysms were located. Forty-three age-matched, race-matched concurrent control patients were also identified. The patients with macroaneurysms had decreased visual acuity (p less than 0.0001) and a higher prevalence of hypertension (p = 0.037), female sex (p = 0.099), and retinal vein occlusions (p = 0.055) than controls. In patients with both a macroaneurysm and venous occlusion there was a 12.0 times higher pre

We studied 40 patients with a total of 44 retinal arterial macroaneurysms. All patients were followed up for at least six months. Macroaneurysms (MAs) have variable clinical presentations and are still frequently misdiagnosed before fluorescein angiography. Haemorrhagic MAs were most frequently misdiagnosed (75%), and had a sudden onset with a relatively poor visual outcome. Patients with these MAs had higher systolic blood pressures and significantly fewer associated retinal vein occlusions (p less than 0.05) than other types of MA. Exudative MAs caused a gradual onset of symptoms, were frequ

A young adult demonstrated a spontaneous vitreous hemorrhage in an eye which harbored two retinal arterial loops, with bleeding originating in the area of the larger loop. These congenital vascular anomalies characteristically originate on the optic disk and terminate by anastomosing with or forming a retinal vessel. Although uncommon, congenital retinal arterial loops can no longer be thought innocuous and must be considered when decreased visual acuity occurs.

BACKGROUND: Ischemic stroke (IS) and retinal ischemia (IR) share similar vascular risk factors, but differ in their risk for subsequent or recurrent stroke and therapeutic options. This study characterizes the cardiovascular risk profiles and magnitude of atherosclerosis of the carotid artery of patients with central retinal artery occlusion (CRAO) in relation to the presence of the retrobulbar "spot sign" on orbital color-coded sonography (OCCS).
METHODS: We performed a retrospective analysis on the detailed cardiovascular risk factors and neuroimaging data in patients with IR presenting bet…

BACKGROUND: Central retinal artery occlusion can result in permanent vision loss. Effective treatment is lacking.
METHODS: We conducted a phase 3, double-blind, double-dummy, randomized, controlled trial involving adults with acute, nonarteritic central retinal artery occlusion who had symptom onset within 4.5 hours before treatment. Patients were assigned, in a 1:1 ratio, to receive intravenous tenecteplase (at a dose of 0.25 mg per kilogram of body weight) and oral placebo or intravenous placebo and oral aspirin (at a dose of 300 mg). The primary end point was vision recovery, defined as a …
