Management of type II endoleak after EVAR: Risk assessment and surveillance.

BACKGROUND: Type II endoleak (T2EL) is the most frequently reported complication following endovascular aneurysm repair (EVAR). Although generally considered benign and often self-limiting, spontaneous resolution does not always occur, and secondary intervention may be required. In this study, morphological and morphovolumetric parameters were compared between patients who developed T2EL and those without endoleak, in order to identify predictors of T2EL development in the pre-EVAR period. Additionally, our experience in the management of T2EL was reviewed in the context of the existing litera

Epidemiology, Risk Factors, and Management of Pregnancy-Related Chronic Back Pain.

PURPOSE OF REVIEW: Pregnancy-related back pain, defined as musculoskeletal pain arising during pregnancy and primarily attributable to gestational adaptations, is highly prevalent and often perceived as an inevitable, self-limited condition. However, a substantial proportion of individuals develop symptoms that persist beyond 3 months postpartum (herein referred to as pregnancy-related chronic back pain) with significant consequences for physical function, mental health, and quality of life. This narrative review summarizes current evidence regarding the epidemiology, mechanisms, risk factors,

Expert consensus on osteoporosis risk management in patients with sleep disorders.

A rate-limiting step in the prevention and early intervention of osteoporosis is identifying its asymptomatic onset. Accumulating evidence shows that sleep disorders are associated with an increased risk of osteoporosis. Given their early detectable and modifiable nature, integrating sleep disorder management into osteoporosis prevention and care pathways offers a novel approach for enhancing skeletal health. This expert consensus represents a collaborative effort by specialists in sleep medicine and orthopedics from across the world, integrating epidemiological, mechanistic, and interventiona

Acute Myocarditis: From Pathophysiology to Risk-Stratified Management-A Clinical Review.

BACKGROUND: Acute myocarditis is an inflammatory disease of the myocardium with an annual incidence of 4-14 per 100,000 individuals, predominantly affecting young adults. Its clinical features are frequently nonspecific, mimicking acute coronary syndrome, which makes early recognition and management challenging. The disease results from infectious (predominantly viral) and noninfectious triggers, including autoimmune disorders, immune checkpoint inhibitors and mRNA vaccines. Pathophysiology involves a dysregulated interplay between innate immunity and adaptive immunity. CLINICAL FEATURES AND

Identification and management of non-obstructive high-risk coronary artery plaque.

Despite advances in its understanding and treatment, coronary heart disease remains the leading cause of death worldwide. Acute coronary syndromes most commonly result as a consequence of the rupture of non-flow-limiting, high-risk coronary artery plaques. Advances in multimodality imaging have allowed the detailed assessment of the high-risk plaque including the assessment of plaque burden, high-risk plaque features, plaque activity and thrombosis. This in turn may help identify those patients at greatest risk, and thus benefit from intensification of pharmacotherapies or from local preventat

Recent Advances in Paediatric Vesicoureteral Reflux: Risk Assessment and Management Strategies.

Recent advances in paediatric vesicoureteral reflux (VUR) have focused on risk-stratification models to better identify patients at risk for complications such as renal scarring, persistent VUR, or breakthrough urinary tract infections (UTIs). By addressing critical clinical questions-such as predicting VUR presence, assessing the likelihood of spontaneous resolution, and determining the risk of breakthrough UTIs despite antibiotic prophylaxis-these tools enhance decision-making. Predictive models like the VURx score and ureteral diameter ratio integrate clinical, demographic, and imaging data

Endometrial Hyperplasia: Current Insights into Epidemiology, Risk Factors, and Clinical Management.

Endometrial hyperplasia (EH) comprises a spectrum of abnormal proliferative changes in the endometrium, ranging from benign glandular overgrowth to lesions with substantial malignant potential. The importance of risk stratification and early identification is highlighted by the growing recognition of EH as a precursor to endometrial cancer. The main causes of EH, according to epidemiological research, include obesity, polycystic ovarian syndrome (PCOS), metabolic dysfunction, and extended exposure to unopposed estrogen. Emerging molecular markers, histological analysis, and imaging are all nec

Management of Atherosclerotic Cardiovascular Risk in Inflammatory Bowel Disease: Current Perspectives.

Inflammatory bowel disease (IBD) is a complex condition characterized by inflammation of the gastrointestinal system, encompassing Crohn's disease and ulcerative colitis. Patients diagnosed with IBD have an increased risk of atherosclerotic cardiovascular disease. This heightened risk can be attributed to a combination of mechanisms, including traditional risk factors, chronic inflammation, intestinal dysbiosis, increased risk of thrombosis, and the use of certain medications such as corticosteroids. There are significant gaps in current knowledge, particularly regarding the management of risk

Advancements in risk stratification and management strategies in primary cardiovascular prevention.

Atherosclerotic cardiovascular disease (ASCVD) remains a leading cause of morbidity and mortality worldwide, highlighting the urgent need for advancements in risk assessment and management strategies. Although significant progress has been made recently, identifying and managing apparently healthy individuals at a higher risk of developing atherosclerosis and those with subclinical atherosclerosis still poses significant challenges. Traditional risk assessment tools have limitations in accurately predicting future events and fail to encompass the complexity of the atherosclerosis trajectory. I

Risk of candidiasis associated with interleukin-17 inhibitors: Implications and management.

The application of interleukin-17 (IL-17) inhibitors, including secukinumab, ixekizumab, brodalumab, and bimekizumab, are associated with elevated risk of candidiasis. These medications interfere with the IL-17 pathway, which is essential for maintaining mucosal barriers and coordinating the immune response against Candida species. The observational data and clinical trials demonstrate the increased incidence of candidiasis in individuals treated with IL-17 inhibitors. Brodalumab and bimekizumab pose a greater risk than secukinumab in eliciting candidiasis, whereas the data regarding ixekizuma

Management of Abnormal Cervical Cancer Screening Test: A Risk-based Approach.

Cervical cancer screening plays a major role in preventing cervical cancer. The field is based on understanding the natural history of human papillomavirus and its role in cervical cancer. Screening has evolved to assessing the risk for cervical intraepithelial neoplasia grade 3, a true cancer precursor, and performing diagnostic tests based on those risks. This article summarizes the present state of management of abnormal cervical cancer screening tests in the United States, based on the most recent 2019 American Society of Colposcopy and Cervical Pathology guidelines.
