Successful Full-term Pregnancies After High-dose Pelvic Radiotherapy for Ewing Sarcoma: A Case Report.

Survivors of childhood cancer are at risk of long-term sequelae that arise as a consequence of cancer treatment. Radiation and chemotherapy treatment in pediatric female patients can have detrimental impacts on fertility, particularly in those with pelvic tumor involvement. We report 2 successful natural full-term pregnancies with vaginal delivery in a woman 12 years after biopsy, irradiation (55.5 Gy), and multi-agent chemotherapy for treatment of pelvic Ewing sarcoma. Both children were born healthy, with no complications in pregnancy or delivery. Fertility preservation and risk assessment f

Primary Extraosseous Ewing Sarcoma of the Thoracic Spine Presenting as Chest Pain Mimicking Spinal Schwannoma.

BACKGROUND: Extraosseous Ewing sarcoma (EES) usually has a pseudocapsule and high vascularity, making it well circumscribed and focally dense with contrast enhancement on magnetic resonance imaging (MRI). Consequently, it is difficult to diagnose and distinguish from other spinal tumors, based on pretreatment radiologic findings alone. Here, we present a case of EES involving the thoracic spinal column, which was suspected to be spinal schwannoma through pretreatment radiologic findings. CASE DESCRIPTION: A 54-year-old woman was admitted to our hospital with upper back and left-sided chest pa

Pulmonary Nodule Size <5 mm Still Warrants Investigation in Patients With Osteosarcoma and Ewing Sarcoma.

BACKGROUND: Osteosarcoma (OS) and Ewing sarcoma (ES) have a high propensity to develop pulmonary metastases. Lung lesions with calcification, peripheral location, and size >5 mm are more likely to represent malignant metastases. We evaluated the incidence of malignancy in nodules 5 mm or less to potentially guide decisions between biopsy and observation. MATERIALS AND METHODS: A retrospective review of patients <25 years of age with metastatic OS and ES treated at our institution between 2001 and 2014 who had undergone pulmonary nodule biopsy was performed. Computed tomographic scans were rev

Primary Ewing Sarcoma / Primitive Neuroectodermal Tumor of the Kidney: A Clinicopathologic Study of 23 Cases.

Primary Ewing sarcoma / primitive neuroectodermal tumor (ES) of the kidney is a rare neoplasm with limited clinicopathologic data. We report 23 such cases with no history of ES elsewhere in the body. The patients included 13 male and 10 female, aged 8-70 years (mean, 31 years). The average tumor size was 11.7 cm (range, 5-20 cm). Microscopic analysis showed predominantly lobular growth (n = 14), with focal papillary (n = 3), alveolar (n = 1), and hemangiopericytoma-like (n = 1) patterns. Several tumors (n = 11) exhibited robust mitotic activity (>10 mitoses/10 high-power fields). Necrosis (n =

Multiple Ewing Sarcoma/Primitive Neuroectodermal Tumors in the Mediastinum: A Case Report and Literature Review.

Ewing sarcoma/primitive neuroectodermal tumors (ES/PNET) are high-grade malignant neoplasms. These malignancies present very rare tumors of thoracopulmonary area and even rarer in the mediastinum. In our knowledge, ES/PNET presented with multiple mediastinal masses has not been reported previously. We experienced a case of a 42-year-old man presented with gradual onset of left-side pleuritic chest pain. A contrast-enhanced chest computed tomography (CT) scan showed separate 2 large heterogeneously enhancing masses in each anterior and middle mediastinum of the left hemithorax. Positron emissio

From the Thoracic Outlet to the Brachial Plexus: A Rare Presentation of Pediatric Ewing Sarcoma.

Ewing sarcoma of the chest wall (CWES) is a highly aggressive mesenchymal tumor, the incidence of which in children under five years of age remains extremely rare. Primary involvement of the first rib and the apex of the thorax presents specific anatomical challenges, leading to severe compressive complications at an early stage. We report the case of a four-year-and-two-month-old girl admitted for right-sided neuropathic arm pain that had been present for three months and was initially attributed to a minor trauma. The gradual onset of upper extremity muscle atrophy, motor deficit predominant

Prognostic impact of tumor-associated macrophages in pediatric Ewing sarcoma, National Cancer Institute (2010-2020).

BACKGROUND: Ewing sarcoma (ES) is the second most common primary malignant bone tumor in children and adolescents. Tumor-associated macrophages (TAMs) are a key component of the tumor microenvironment. While increased TAMs infiltration is linked to poor prognosis in various solid tumors, limited data are available regarding their role in sarcomas. METHODS: This retrospective study aimed to evaluate the frequency, density, and prognostic impact of TAMs in pediatric patients with ES treated at the Pediatric Oncology Department, National Cancer Institute (NCI), Cairo University. Immunohistochemic

Targeted and molecular therapies in Ewing sarcoma: a comprehensive review of preclinical and clinical advances.

INTRODUCTION: Ewing sarcoma (EWS) is an aggressive small round blue cell malignancy driven by EWSR1::ETS fusions, with poor survival outcomes in metastatic and relapsed disease. While multimodal chemotherapy remains the cornerstone of therapy, emerging agents hold potential to overcome resistance and improve outcomes. METHODS: A systematic search of PubMed, Scopus, and Google Scholar through February 2026 identified 1,166 records. Following duplicate removal and two-stage screening, 87 preclinical and clinical studies were included. Data were extracted and synthesized narratively, with emphas

Combination treatment with mitoxantrone and doxorubicin induces multidrug resistance in Ewing Sarcoma CDX reporter xenografts.

Ewing Sarcoma (ES) is an aggressive malignancy with limited treatment options in advanced disease. This study aimed to identify efficacious therapeutic candidates from FDA-approved drugs via in silico screening and evaluate the potential of the top candidate, mitoxantrone (Mito), alone and in combination with standard-of-care doxorubicin (Doxo). In silico screening was performed against an ES-specific molecular expression profile. Mito was validated in ES in vitro cellular models, and combination effects with Doxo were assessed by cytotoxicity assays. In vivo efficacy was evaluated in ES cell-

Complete pathological response of a primary renal Ewing sarcoma to neoadjuvant chemotherapy: A case report.

Primary renal Ewing sarcoma (PRES) is an exceptionally rare and aggressive malignancy with a poor prognosis. The optimal management strategy remains poorly defined due to its rarity. We report a case of a 39-year-old female with PRES. The patient received six cycles of neoadjuvant chemotherapy (VDC/IE regimen), followed by right radical nephrectomy. Histopathological examination revealed dense fibrocollagenous and inflammatory tissue with no viable tumor cells, confirming a pathological complete response (pCR). This case highlights the critical importance of a multimodal treatment approach int

Rib-Origin Ewing Sarcoma With Cranial Nerve Involvement From Orbital Metastasis: A Rare Case Report.

Ewing sarcoma (ES) is an aggressive tumor of the bone in young adults, commonly originating from the pelvis and long bones. It is characterized by chromosomal translocation t(11;22) (q24;q12), resulting in the EWSR1-FLI1 fusion gene. It presents with metastasis commonly to the lungs and other soft tissues. While the metastasis to lungs is common, orbital involvement with cranial involvement is exceptionally rare. The case highlights the need for the early detection and aggressive treatment in order to reduce mortality in patients ES. A 23-year-old South-Asian male presented with a 2 month hist

Extraskeletal Ewing sarcoma primarily arising in the stomach and omentum in children: a case series.

Primary visceral extraskeletal Ewing sarcoma is exceedingly rare; gastric and omental primaries pose diagnostic challenges by mimicking benign gastrointestinal conditions. We present a 17-year-old with a gastric mass and a 12-year-old with an omental primary, mismanaged as iron deficiency anemia and functional constipation, respectively. Diagnoses were confirmed via imaging, endoscopy, and histopathology with EWSR1::ERG and EWSR1::FLI1 fusion detection. Both received neoadjuvant chemotherapy for cytoreduction and organ-sparing surgery. The first underwent wedge gastrectomy with positive serosa

Renal ewing sarcoma in a young female: a case report and review of targeted therapy.

Ewing sarcoma (ES) is an aggressive neoplasm predominantly affecting pediatric and adolescent populations. Renal involvement in ES is exceedingly rare, representing less than 1% of all renal malignancies. Herein, we present the case of a 22-year-old female diagnosed with renal Ewing sarcoma (RES) accompanied by renal vein thrombosis. The patient reported a one-month history of persistent left lumbar pain, prompting hospitalization. Magnetic resonance imaging identified an extensive left suprarenal mass measuring 13.5 × 10.5 × 4.5 cm, with concurrent renal vein thrombosis. The comprehensive eva

Retroperitoneal Ewing Sarcoma Masquerading as an Intraabdominal Abscess in a Teenager: A Rare Diagnostic Challenge.

Retroperitoneal Ewing sarcoma is a rare and aggressive extraskeletal variant of Ewing sarcoma, often presenting with nonspecific symptoms that can delay diagnosis. We discuss the case of a 19-year-old male who presented to the emergency room with progressive left abdominal and flank pain. Imaging revealed a retroperitoneal, necrotic mass and biopsy with immunohistochemistry confirmed retroperitoneal Ewing sarcoma with NKX2.2 and CD99 positivity. Fluorescence in situ hybridization (FISH) analysis showed that 98% of cells had an EWSR1 gene rearrangement. For treatment, the patient underwent a CT

Muscular Cystic Lesions: A Highly Misdiagnosed Extraosseous Ewing Sarcoma: Two Case Reports and Literature Review.

BACKGROUND: A retrospective analysis was carried out on two cases of extraosseous Ewing sarcoma (ES) that were initially misdiagnosed as lymphatic malformations, with a focus on clinical manifestations, imaging characteristics, and other relevant case data. A comprehensive review of the literature was performed to enhance the understanding of cystic extraosseous ES. CASE PRESENTATION: Both cases in this study originated from cystic lesions in the muscular interstitial space. Due to the absence of distinctive clinical manifestations and imaging features, the diagnosis is primarily dependent on

Protein Kinase Inhibitor Alpha Drives Vincristine Resistance in Ewing Sarcoma via cAMP-EPAC Signaling Reprogramming.

Ewing sarcoma (ES) is an aggressive bone malignancy with poor outcomes for chemotherapy-resistant patients, yet the mechanisms underlying vincristine resistance remain unclear. Here, we identify protein kinase inhibitor alpha (PKIA) as a critical driver of chemoresistance through cAMP-EPAC signaling reprogramming. Transcriptomic analysis of vincristine-resistant ES cells revealed PKIA upregulation, which correlated with poor survival in clinical cohorts (HR = 2.14, p < 0.001). Mechanistically, PKIA overexpression elevated intracellular cAMP levels but suppressed PKA activity, instead activatin

Oncolytic Maraba Virus MG1 Mediates Direct and Natural Killer Cell-Dependent Lysis of Ewing Sarcoma.

BACKGROUND: Ewing sarcoma (EWS) is a rare cancer of the bone and soft tissue, most prevalent in children and young adults. The treatment of EWS has progressed relatively little in over 30 years. Survival rates for patients, particularly those with metastatic and/or relapsed disease remain poor, highlighting the urgent need for innovative treatment options. METHODS: Here, we have explored the therapeutic potential of the oncolytic Maraba virus strain MG1 using various in vitro models of EWS, including established cell lines, doxorubicin-resistant derivatives, spheroid cultures and primary pati

Uterine transposition for fertility preservation in extra-skeletal ewing sarcoma: the first French case report.

We present the first French case of uterine transposition for fertility preservation in a 25-year-old nulligravida patient with rectovaginal extra-skeletal Ewing sarcoma. After receiving induction chemotherapy, the uterus was transposed to the upper abdomen via laparotomy. During the same procedure, a posterior pelvectomy was performed to achieve wide surgical margins for tumor removal. To mitigate digestive toxicity from subsequent pelvic radiotherapy, a breast expansion prosthesis was placed. The patient experienced regular menstruation postoperatively, indicating preserved uterine function.
