Scalp squamous cell carcinoma in xeroderma pigmentosum.

CONTEXT: Xeroderma pigmentosum is a rare autosomal-recessive disorder that appears in early childhood. Squamous cell carcinoma is not uncommon in patients with xeroderma pigmentosum and mostly involving the face, head, neck, and scalp. However, squamous cell carcinoma of the scalp may exhibit an aggressive course. CASE REPORT: Here, we present a huge squamous cell carcinoma of the scalp in a three-years-old child with xeroderma pigmentosum. In addition, we illustrate the challenges of a child with xeroderma pigmentosum who grows up in a sunny environment where the possibility of early onset o…

Scalp dysesthesia related to cervical spine disease.

BACKGROUND: Scalp dysesthesia is characterized by abnormal sensations of the scalp in the absence of any other unusual physical examination findings. The pathogenesis of this condition is unknown but has been reported in the setting of underlying psychiatric disorders. Other localized pruritic syndromes, including brachioradial pruritus and notalgia paresthetica, have been associated with pathologic conditions of the spine and have been successfully treated with gabapentin. OBSERVATIONS: Among 15 women identified in a retrospective review of medical records as having been seen with scalp dyse…

Scalp edema: don't forget sunburn in children.

Scalp edema is an uncommon and striking finding in children that may alarm both parents and physicians. The objectives of this case report were to raise awareness among pediatric emergency physicians of the unusual presentation of sunburn as scalp edema. We present the case of an eight-year-old boy with sunburn of the head, presenting with scalp and face edema. Pitting edema and erythema were dominant on the forehead. Shaving of the boy's head the day before the symptoms was the most striking issue, and the sunburn healed gradually without any complications. Healthcare professionals should be …

Scalp reconstruction: regeneration with acellular dermal matrix.

BACKGROUND: Current methods of scalp reconstruction with complicating osteomyelitis of underlying bone require excision of deficient tissue and a staged reconstruction for ultimate coverage of the defect. Our group proposes a novel use of acellular dermal matrix (ADM) to preserve these deficient areas without the need for excision or staged management of osteomyelitic calvaria. METHODS AND RESULTS: From 2003 to 2008, 3 patients underwent scalp reconstruction with ADM at our facility. Elevation of the scalp for secondary operations in each case showed full incorporation of the ADM. Follow-up r…

Microsurgical replantation for child total scalp avulsion.

Total scalp avulsion is an intractable problem in clinical practice. It often occurs in female adults, but rarely in children. In this article, we described the replantation of 2 scalp segments in a 4-year-old girl. The right segment involved the hairy scalp merely, whereas the left one involved the hairy scalp, forehead, eyelid, ear, and part of the face. Warm ischemia time was about 28 hours, and operation time was 13 hours. Although an explorative operation was performed, the left segment survived partly. A relatively satisfactory aesthetic result was obtained by transplanting split skin fr…

Congenital apocrine tumour: a rare scalp tumour.

We report a rare congenital scalp tumour with histological diagnosis of a congenital apocrine adenoma with features of a tubular adenoma. Following cranial CT and MRI, the tumour was serially excised. The macroscopic and microscopic appearances and management are discussed. To our knowledge, such a case has not been previously reported.

Carotid atherosclerotic disease following childhood scalp irradiation.

OBJECTIVE: During the first half of the 20th century, scalp irradiation was a standard treatment for children suffering from Tinea Capitis. These children are now more than 50 years old, reaching the age when manifestations of atherosclerosis are common. We investigated the possible association between childhood low dose scalp irradiation and development of carotid atherosclerosis in adulthood. METHODS: The study included 145 individuals treated by irradiation in their childhood, and 150 matched control subjects with no history of irradiation. The occurrence of stroke was disregarded during t…

Managing scalp defects in sub-Saharan Africa.

OBJECTIVES: To determine the common aetiological factors of scalp defects, and outcome of management. DESIGN: A two year prospective study. SETTING: Usmanu Danfodiyo University Teaching Hospital, Sokoto, Nigeria. SUBJECTS: All consecutive patients with scalp defect from January 2001 to December 2002. MAIN OUTCOME MEASURES: Size of defect, associated bone loss, osteomyelitis, type of surgery, duration of hospital stay and complications of surgery. INTERVENTIONS: Patients went through a simple management protocol involving history, clinical examination, relevant tests and appropriate treatm…

Acquired scalp alopecia. Part II: A review.

The neutrophil-associated and infiltrative scarring alopecias are reviewed including folliculitis decalvans, tufted folliculitis, dissecting cellulitis of the scalp, acne keloidalis and follicular degeneration syndrome. The management of acquired scalp alopecia is also reviewed including newer, promising therapies. More specific agents targeting components of the androgen system will make the treatment of androgenetic alopecia more rewarding. Similarly new immunomodulatory therapies show great promise for the lymphocyte-associated alopecias and include a new generation of macrolide immunosuppr…

Vacuum extraction: does duration predict scalp injury?

OBJECTIVE: To identify variables that increase the chance of neonatal scalp injury during vacuum extraction. METHODS: We conducted a prospective observational study of 134 vacuum extraction-assisted deliveries at Olive View-UCLA Medical Center in 1995. Data collected included parity, gestational age, duration of first and second stages of labor, indication for operative delivery, station and position of fetal head, duration of vacuum application, number of "pop-offs," neonatal weight, and descriptions of scalp marks or injury. Cranial imaging studies were obtained if clinically indicated. RE…

Extensive head burns corrected by scalp extension.

BACKGROUND: Scalp extension has been revealed as a good corrective method for androgenetic alopecia and it is possible to foresee its other probable applications in the field of increasing hair-bearing areas. OBJECTIVE: The performance of Frechet's scalp extender in selected head burn cases allows the possibility of maintaining a high degree of social presentability during the patient's entire treatment period, on condition that this procedure or others are repeated. METHODS: The author discusses his experience in correcting an extensive head burn and the performance of Frechet's extender. …

Sequestrated meningoceles of scalp: extracranial meningeal heterotopia.

Sequestrated meningocele of the scalp has seldom been reported and is difficult to diagnose. Clinically it resembles dermoid cyst, hemangioma, or alopecia; radiographs and computed tomographic scans reveal no cranial bone defect, and surgery discloses no communication with the cranial cavity. Histologically, the lesion is characterized by a loose arrangement of connective tissue in dermis and subcutis, associated with flattened cells around collagen fibers (meningothelial cells). Most examples are very vascular, sometimes mimicking angioma, and about one third contain small necrotic foci. The …

Physiopathological role of bald-scalp cytosolic proteins.

The physiopathological role of androgen binding proteins in male pattern baldness (MPB) has been studied by using tritiated dihydrotestosterone (DHT) and methyltrienolone (R 1881) as ligands. DHT binding in bald scalp from subjects suffering from MPB is high (53 +/- 12 fmol/mg protein) in cytosol obtained from bald areas, being undetectable in hairy areas from the same subject. Since methyltrienolone does not bind in bald scalp cytosol, there must be no specific DHT receptor in this material. Several kinetic and molecular parameters of DHT binding in bald scalp cytosol and serum were similar i…

Congenital scalp defects associated with postaxial polydactyly.

A family is reported, several members of which had congenital scalp defects and postaxial polydactyly type A, with wide variability of expression. The hypothesis is formulated that this association is not fortuitous, but is a distinct malformation complex in which congenital scalp defects are associated with distinct malformation of the limbs.
