Scaphoid nonunions result in a predictable pattern of wrist arthrosis. To minimize the incidence of arthrosis, the goal of treatment should be consolidation of the fracture with the scaphoid in anatomic alignment. Computed tomography and magnetic resonance imaging scans can aid evaluation of carpal collapse, scaphoid collapse, scaphoid nonunion, bone loss, and detection of osteonecrosis. Nonunion of the scaphoid waist may result in a humpback deformity, increasing the chances of further collapse and arthrosis. This collapse deformity must be approached volarly with an intercalary bone graft an

In a prospective study we performed bone scans on 84 patients who had sustained a wrist injury and in whom a scaphoid fracture was clinically suspected but could not be confirmed on the original set of five routine "scaphoid view" radiographs. In 40 patients the bone scan was normal and in 25 there was increased uptake in areas other than the scaphoid. In 19 there was localized increased uptake in the scaphoid bone. In seven of these a scaphoid fracture was subsequently demonstrated on repeat radiographs (five cases) or a CT scan (two cases). In the remaining 12 no fracture could be demonstrat…

The article describes a sequestrated portion of the carpal scaphoid bone, which is interpreted as a marginal fragmentation by partial necrosis of the scaphoid bone. In the authors' opinion, earlier writings have not proved incontestably the occurence of a genuine osteochondritis dissecans of the scaphoid bone.

BACKGROUND: *Equal contributors.Planar bone scintigraphy (PBS) is often advocated for diagnosing occult scaphoid fractures. PBS is a sensitive diagnostic modality, but lacks specificity, which may result in over-diagnosis.
PURPOSE: To examine, in a pilot study, the potential additional value of single photon emission computed tomography (SPECT) combined with low dose computed tomography (CT) for the diagnosis of an occult scaphoid fracture.
MATERIAL AND METHODS: Ten patients that underwent combined PBS and SPECT/CT for a clinically suspected scaphoid fracture, where radiographs could not det

PURPOSE: The purpose of this study was to correlate the pre-operative imaging, vascularity of the proximal pole, and histology of the proximal pole bone of established scaphoid fracture non-union.
METHODS: This was a prospective non-controlled experimental study. Patients were evaluated pre-operatively for necrosis of the proximal scaphoid fragment by radiography, computed tomography (CT) and magnetic resonance imaging (MRI). Vascular status of the proximal scaphoid was determined intra-operatively, demonstrating the presence or absence of puncate bone bleeding. Samples were harvested from th

The scaphoid is the most commonly fractured carpal bone, with frequent complications that are predisposed by its anatomical location, anatomical configuration (shape and length), and vascular supply. The most common mechanism of injury is a fall onto an outstretched hand. Imaging plays a significant role in the initial evaluation and treatment of scaphoid fractures and their complications. Radiography should be the first imaging modality in the initial evaluation and follow-up of scaphoid fractures. Computed tomography with its superb spatial resolution enables better visualization and charact

The placement of a headless screw within the scaphoid is important for successful fixation of fractured scaphoids. We ascertained the safe position of a screw using virtual collision experiments on a three-dimensionally reconstructed scaphoid bone and screw model, and applied measured angles to simple radiographs. We used images from computed tomograms of the wrists taken from 13 men to reconstruct a three-dimensional cortical image of the scaphoid. Inside the bone, we inserted a virtually reconstructed Herbert screw model and calculated the safe angle along the long axis of the scaphoid at wh…

Scaphoid nonunion develops in approximately 15% of patients following scaphoid fractures. To prevent carpal collapse, targeted diagnosis and early initiation of treatment are crucial. This paper describes the algorithm for scaphoid nonunion treatment established at the Department of Plastic and Hand Surgery at the Medical Center - University of Freiburg. We carried out a monocentric, retrospective analysis of scaphoid reconstructions for scaphoid nonunion performed at our clinic between January 2012 and December 2022. We selected among four treatment options based on the vascularity of the prox

PURPOSE: To describe a method of producing a life-sized three-dimensional (3D) solid model of a scaphoid bone by combining multidetector-row computed tomography (MDCT) volume data and rapid prototyping technique using thin paper as material, and to evaluate the dimensional accuracy of the experimental model.
MATERIAL AND METHODS: We experimentally produced life-sized 3D solid models of 14 cadavers' scaphoid bones using high-resolution MDCT volume data, and evaluated the dimensional accuracy of the 3D solid models.
RESULTS: The 3D solid model was almost perfectly produced in both size and sha…

Background Headless screw fixation is the current gold standard of surgical repair for scaphoid fractures. However, maintaining reduction of certain types of scaphoid fractures is challenging with a compression screw. Plate fixation may offer superior fixation in some scaphoid fractures, particularly those with comminution, nonunion, segmental bony defects, and osteopenic or osteoporotic bone. Purpose This study questions whether method of fixation is a determinant in load to failure in segmental scaphoid fractures, and whether any fixation provides a greater mechanical advantage in simulate

Treatment of scaphoid fractures has been focused mainly on achieving union, with less attention to restoring normal scaphoid shape and orientation. Malalignment of one carpal bone will disrupt the kinetics of the entire wrist. The dorsal intercalated segment instability deformity associated with scaphoid waist nonunion is a nondissociative form of carpal instability. It has to be reduced in the treatment of scaphoid waist nonunions to avoid kinetic problems that will lead to arthritic changes. Computerized tomography scanning has become indispensable to visualize the humpback deformity clearly

Fractures of the scaphoid are the most common of all fractures of the carpal bones. Scaphoid collapse and angular deformity may develop after fractures. The tenuous vascular anatomy of the scaphoid, the inherent biomechanical instability of scaphoid fractures, and the difficulty of radiographic diagnosis are the culprits of nonunion. The treatment of established scaphoid nonunion is difficult, and more so is the treatment of failed nonunion after grafting. One common method for treating scaphoid nonunion with angular deformity is length restoration using a wedge bone graft and Herbert screw fi

INTRODUCTION: This study investigated the association between capitate morphology and the success rates of non-vascularized bone graft (NVBG) and vascularized bone graft (VBG) operations for scaphoid nonunion.
METHODS: We retrospectively analysed 213 patients with established scaphoid non-union who underwent either non-vascularized bone graft (n = 75) or vascularized bone graft (n = 138). We analysed risk factors, including demographics, medical history, implant type, previous surgery, bone graft donor site and capitate morphology for their association with treatment outcome. We also assessed

This study was designed to confirm the results of Finkenberg et al. (J Hand Surg 1993;18A: 4-7), who found a high sensitivity (100%) and specificity (95%) of the intrasound vibration method in diagnosing occult scaphoid fractures. These occult scaphoid fractures are not visible on x-ray films, but clinically the patients are suspected of having a scaphoid fracture. A vibratory apparatus is placed over the anatomical snuff-box and a vibration of 100 mW is emitted; a painful sensation is produced if the scaphoid is fractured. Thirty-seven consecutive patients with a clinically suspected scaphoid

In 1991, Carlos Zaidemberg described a new technique to repair scaphoid non-unions with a vascularized bone graft harvested from the radial styloid process. An anatomic study based on 30 dissections after colorized latex injection established the constancy of the radial styloid process's artery, while showing that its origin, course and length were subject to variations. In a retrospective series of 38 cases over a period of 10 years, the vascularized bone graft was indicated for: (1) scaphoid non-union with the presence of avascular changes of the proximal fragment (23 cases); (2) failed prio

Scaphoid fracture is, in most cases, usually still treated conservatively. The disadvantages of long-term immobilization are stiffness of the wrist joint, loss of strength and higher costs. The osteosynthesis of the scaphoid fracture with the Herbert bone screw restores the exact form and length and the normal position of the scaphoid to the other carpal bones. It prevents non-unions, arthrosis and other late complications. The Freehand method is used as a percutaneous technique without utilization of a jig. The scaphoid fracture is treated with closed reduction and osteosynthesis is performed

INTRODUCTION: Diagnosing scaphoid fractures remains challenging. High-resolution peripheral quantitative computed tomography (HR-pQCT) might be a potential imaging technique, but no data are available on its feasibility to scan the scaphoid bone in vivo.
METHODOLOGY: Patients (≥18 years) with a clinically suspected scaphoid fracture received an HR-pQCT scan of the scaphoid bone (three 10.2-mm stacks, 61-μm voxel size) with their wrist immobilized with a cast. Scan quality assessment and bone contouring were performed using methods originally developed for HR-pQCT scans of radius and tibia. Th…

Bilateral stress fractures of the scaphoid bones in a gymnast are reported and discussed with special consideration of the possible differential diagnoses; bipartite scaphoid and pseudarthrosis of the scaphoid bone. The relevance of the mechanical factors involved and the abduction and dorsiflexion of the wrist in gymnastics are suggested as likely pathomechanisms.

Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association2026eng
The success of arthroscopically assisted scaphoid nonunion surgery depends on multiple factors, most importantly the time between injury and surgery. The optimal bone grafting option for a particular scaphoid nonunion case depends on multiple factors including scaphoid bone loss and concurrent wrist pathology. Autograft and allograft are both considered reasonable bone grafting options depending on the details of the case.
