Functional Characterization of Glycoprotein Nonmetastatic Melanoma Protein B in Scleroderma Fibrosis.

Glycoprotein nonmetastatic melanoma protein B (GPNMB) is involved in various cell functions such as cell adhesion, migration, proliferation, and differentiation. In this study, we set forth to determine the role of GPNMB in systemic sclerosis (SSc) fibroblasts. Dermal fibroblasts were isolated from skin biopsies from healthy subjects and patients with diffuse cutaneous (dc)SSc. GPNMB was upregulated in dcSSc fibroblasts compared to normal fibroblasts, and correlated negatively with the modified Rodnan skin score. In addition, dcSSc fibroblasts secreted higher levels of soluble (s)GPNMB (147.4

Idiopathic and L-tryptophan-associated eosinophilic fasciitis before and after L-tryptophan contamination.

Recently, a causative association has been made between the ingestion of levotryptophan (L-tryptophan) and the eosinophilia-myalgia syndrome (EMS), a new entity manifested by peripheral blood eosinophilia, myalgias, constitutional symptoms, and cutaneous edema with fibrosis. Contaminated levotryptophan preparations produced at a single manufacturing company between October 1988 and June 1989 have been implicated in all EMS cases. In this study, we analyzed retrospectively 49 patients with cutaneous fibrosis for a history of levotryptophan use. Levotryptophan ingestion prior to the onset of the

Successful treatment with baricitinib of linear morphea following the lines of Blaschko mimicking lichen striatus.

Linear morphea, also known as linear scleroderma, is a localized form of scleroderma characterized by the presence of lesions that follow a linear distribution pattern. Apart from the typical inflammation and fibrosis of the skin, the linear subtype of morphea often affects underlying structures such as muscles and bones, which can lead to functional limitations. Lichen striatus, a linear inflammatory skin condition, primarily affects children aged 5 to 15 years. Interestingly, both diseases can exhibit lesions that follow the lines of Blaschko. Here we report a case with linear morphea follow

Vigorous exercise-induced unilateral eosinophilic fasciitis: rare and easily misdiagnosed subtype.

The hallmark of eosinophilic fasciitis (EF) is painful symmetric skin stiffness and limb sclerosis with deep fascia inflammation. Two cases of unilateral EF have been reported in the literature but were not related to strenuous physical activity. In this report, a young female presenting with unilateral scleroderma-like skin changes, blood and tissue eosinophilia, and hypergammaglobulinemia proven to be EF after strenuous physical activity is described. This case represents the first case reported in the current literature of unilateral EF after vigorous exercise. A skin to muscle biopsy and m

Serum levels of tenascin-C in collagen diseases.

Tenascins are a family of large multimetric extracellular matrix (ECM) proteins. Among them, large molecular weight variant tenascin-C is known to be specifically expressed in pathological conditions. However, no link between tenascin-C and collagen diseases has been established. The aim of our study was to determine the serum tenascin-C levels in patients with various collagen diseases, and to evaluate the possibility that serum levels of tenascin-C can be a useful marker for collagen diseases, correlating with the pathogenesis. Serum tenascin-C levels of 33 patients with scleroderma (SSc), 1

[Scleroderma induced by chemical agents. Description of a case and review of the literature].

The authors describe a case of a 24 years old man affected by eosinophilic fasciitis likely due to chemical exposure. The patient handled and inhaled a compound containing sodium hypochlorite, sodium hydrate and cationic surface agents. The clinical picture was characterized by sudden onset, oedema and pain on the limbs; laboratory tests revealed an erythrocyte sedimentation rate of 55 mm/hour, polyclonal hypergammaglobulinemia and eosinophilia. The histologic examination of a deep musculo-cutaneous biopsy showed an infiltrate composed of lymphocytes and macrophages localized in the fascia and

Solitary Morphea Profunda: A Case Report.

Morphea is an autoimmune connective tissue disease that leads to sclerosis of skin and underlying subcutaneous tissue due to excessive collagen deposition. It is also called localized scleroderma. Morphea profunda is one of the morphological variants of morphea. It involves the deep dermis, subcutaneous tissue, fascia and muscle. Morphea profunda is characterized by one or more deep sclerotic plaque, thickened skin with hypo or hyper-pigmentation occurring mostly in abdomen and extremities. Lesions usually progress slowly and remain stable for years. Diagnosis of morphea is made by characteris

Morphological and clinical picture of the morphea in the oral cavity.

There are several types of morphea with different levels of connective tissue involvement and morphological manifestations. In this mini review, it was pointed out the most important morphological and clinical aspects of localised scleroderma in the oral cavity. The case presented in this article supports the scientific information and is described with details. The morphea of mucous membrane which was clinically suspected, was proved by histopathological examination of the sample. The unusual location of the local findings posed a diagnostic challenge. The case history should be significant d

A surgical approach for stable lesions of linear scleroderma.

A 29-year-old Japanese woman presented with alopecia lesions in her occipital region and was diagnosed as linear scleroderma. Topical steroids along with psoralen and UVA light therapy were applied, but the lesion persisted. A surgical approach was therefore selected to treat the lesion. The resection was successful without any recurrence for 1 year. Sclerotic lesions on the exposed areas can cause cosmetic disfigurement, and surgery may be a useful modality for treating stable linear scleroderma.

Incidental epidermolytic hyperkeratosis in progressive systemic sclerosis.

Incidental epidermolytic hyperkeratosis (EH) has been reported in a variety of lesions. We describe here incidental EH in a patient with progressive systemic sclerosis (PSS). We reviewed 108 skin biopsies from our cases of PSS and localized scleroderma, but this finding was seen only in the present case. Focal acantholytic dyskeratosis is analogous to EH, but we believe that this case is most consonant with EH. Since there has been no other report of EH in PSS, this association may be coincidental.

Solitary morphea profunda following trauma sustained in an automobile accident.

Solitary morphea profunda (SMP) is a variant of localized scleroderma (LS). We report the case of a 50-year-old white woman with a history of trauma sustained in an automobile accident who presented with SMP on the right upper arm. We also provide a review of the classification, epidemiology, etiology, diagnostic studies, pathogenesis, physical findings, histopathology, treatment, and prognosis of SMP, along with other important details pertaining to the disease. We also provide a brief overview of LS and morphea profunda (MP).

Unilateral linear lichen sclerosus et atrophicus.

We report on a 23-year-old woman with unilateral lichen sclerosus et atrophicus (LSA) who exhibited extensive eruptions on the right extremities and right aspect of the abdomen. The lesions on the right extremities appeared to exhibit a linear pattern along Blaschko's lines, while the right abdominal skin lesion was a whitish, parchment-like plaque. Histological findings for both lesions showed characteristic features of LSA. LSA should be differentiated from the linear type and the macular type of localized scleroderma.

Linear scleroderma after contusion and injection of mepivacaine hydrochloride.

A 36-year-old woman initially was treated for a contusion by local injection of mepivacaine hydrochloride into the left dorsum of the foot. Approximately 3 months after the injury and injection, linear sclerotic plaques originating from the site of contusion and injection were recognized. These progressed in extent and severity over a period of 3 years, when she presented to our clinic. By biopsy, swelling of collagen fibers in the lower dermis was revealed and the condition was diagnosed as linear scleroderma. Our present case had multiple linear sclerotic plaques of the left lower extremity,

Deep morphea.

Deep morphea encompasses a variety of clinical entities in which inflammation and sclerosis are found in the deep dermis, panniculus, fascia, or superficial muscle. Morphea profunda, eosinophilic fasciitis, and disabling pansclerotic morphea of children are included in this group, but overlapping of the extent and depth of cutaneous involvement in these various conditions precludes their distinction on the sole basis of clinical or even histologic examination. Furthermore, the limits between morphea profunda and generalized morphea, which usually are classified outside this group, are not clea

Improving outcomes in scleroderma: recent progress of cell-based therapies.

Scleroderma is a rare, potentially fatal, clinically heterogeneous, systemic autoimmune connective tissue disorder that is characterized by progressive fibrosis of the skin and visceral organs, vasculopathy and immune dysregulation. The more severe form of the disease, diffuse cutaneous scleroderma (dcSSc), has no cure and limited treatment options. Haematopoietic stem cell transplantation has emerged as a potentially disease-modifying treatment but faces challenges such as toxicity associated with fully myeloablative conditioning and recurrence of autoimmunity. Novel cell therapies-such as me
