Incidence, risk factors, characteristics and shoulder joint function status of heterotopic ossification following surgical treatment of clavicle fracture or acromioclavicular joint dislocation.

BACKGROUND: Heterotopic ossification (HO) around the shoulder is common in patients with clavicle fracture or acromioclavicular joint (ACJ) dislocation. Periarticular HO sometimes leads to poor joint function. HO of the elbow and hip requires active prevention. However, studies on the incidence, risk factors, characteristics, and shoulder joint function of HO around the shoulder after surgical treatment of clavicle fracture or ACJ dislocation are still lacking. STUDY HYPOTHESIS: ACJ dislocation, clavicular hook plate fixation and excessive screw length are independent high-risk factors for po

Patient characteristics are more important than preoperative radiographic factors in predicting the risk of acromial stress fracture following reverse total shoulder arthroplasty.

BACKGROUND: Acromial stress fractures (ASFs) after reverse total shoulder arthroplasty (rTSA) can have a devastating effect on a patient's overall function, causing decreased range of motion and patient satisfaction. The purpose of this study was to evaluate preoperative patient demographics and radiographic findings associated with risk for ASF after rTSA. METHODS: A retrospective review was performed on patients who underwent primary rTSA by a single surgeon between December 2011 and December 2022. Inclusion criteria were primary rTSA with minimum 6-month postoperative radiographs. Preopera

Zone-specific bone density evaluation of the acromion may predict postoperative acromion stress fracture in patients undergoing a reverse total shoulder arthroplasty.

BACKGROUND: The goal of this study was to use preoperative computed tomography (CT) scans to identify differences in the Hounsfield units (HU) of the acromion in patients who did and did not develop a postoperative acromial and scapular spine fracture (ASF) after primary reverse total shoulder arthroplasty (rTSA). METHODS: A retrospective analysis was performed at a single institution. All patients undergoing an rTSA with either a 135° neck-shaft angle (NSA) humeral inlay design combined with a lateralized center-of-rotation (COR) glenosphere or a 145° NSA onlay combined with a medialized COR

Diagnostic accuracy of Fresno-Quebec Rules and risk factors for an associated fracture in patients presenting to the emergency department with anterior shoulder dislocation: A retrospective study.

BACKGROUND: Anterior shoulder dislocation is the most common shoulder injury in patients presenting to the emergency department (ED). Up to 25% of these injuries are fracture-dislocations. In general, the standard approach is to obtain plain radiographs before and after reduction. Fresno-Quebec Rules (FQR) are described to identify the patients who require an x-ray before reduction to reduce radiation exposure and delays in treatment. We aimed to evaluate the efficacy of clinical predictors used in the Fresno-Quebec algorithm for detecting a shoulder fracture-dislocation. METHODS: Records of

Short-Term Projected Use of Reverse Total Shoulder Arthroplasty in Proximal Humerus Fracture Cases Recorded in Humana's National Private-Payer Database.

Indications for reverse total shoulder arthroplasty (RTSA) have been expanding, and there has been a recent trend in using RTSA to manage proximal humerus fractures. In this study, we used a large private-payer database to analyze this trend over the period 2010 to 2014 and project RTSA use through 2020. Results showed modest quarterly growth in use of RTSA (4.9% compound quarterly growth rate). The number of RTSAs was projected to more than triple by 2020. RTSA is becoming an increasingly popular treatment option for proximal humerus fractures.

Axillary artery injury after an anterior shoulder fracture dislocation and "periosteal sleeve avulsion of the rotator cuff" (SARC). Case report and review of the literature.

We present the rare complication of an axillary artery injury associated with an anterior dislocation of the humerus and what we believe to be the first reported periosteal sleeve avulsion of the entire rotator cuff (SARC). We review the literature and discuss the cause of this unusual injury pattern.

Reverse shoulder arthroplasty for proximal humerus fracture: does tuberosity reinsertion improve the functional outcomes more than 5 years after surgery?

INTRODUCTION: While tuberosity reinsertion significantly improves the short-term functional outcomes of reverse shoulder arthroplasty done for proximal humerus fracture, we do not know how well these results hold over the long term. The objective of this study was to analyze the effect of tuberosity reinsertion on the quality of life of patients and the functional outcomes of the operated limb after a minimum follow-up of 5 years. METHODS: Sixty-two patients were included. Their mean age at the final review was 79 ± 10 years. The Katz and Lawton scales, Constant score, DASH and SSV were colle

Reverse shoulder arthroplasty versus locking plate fixation for proximal humeral fracture dislocations in elderly patients: study protocol for a randomized controlled trial.

BACKGROUND: Proximal humeral fractures are common injuries in the elderly population, with their incidence expected to rise due to increased life expectancy. A small subset of these fractures may be associated with dislocating forces that can result in shoulder dislocation concurrent with the fracture. For the elderly patients, proximal humeral fracture dislocation can be treated by open reduction and internal locking plate (LP) fixation or reverse shoulder arthroplasty (RSA). By now, no high-quality trials have compared the two. METHODS: This study is a prospective, single-center, superiorit

A Retrospective Study of 1076 Cases of Shoulder Dislocation at a Single Center in Türkiye to Evaluate the Role of Post-Reduction Radiography in the Detection of Clinically Significant Fractures.

BACKGROUND The role of post-reduction radiography in patients with shoulder dislocation remains controversial. Therefore, this retrospective study of 1076 cases of shoulder dislocation at a single center in Türkiye aimed to evaluate the role of post-reduction radiography in the detection of clinically significant fractures. MATERIAL AND METHODS Patients with radiographically confirmed anterior shoulder dislocation were included in the study, and their demographic data, mechanism of injury, pre- and post-reduction radiograph readings, reduction method, and patient outcome were recorded. A cost-effectiveness analysis of reverse total shoulder arthroplasty compared with locking plates in the management of displaced proximal humerus fractures in the elderly: the DelPhi trial.

AIM: To evaluate the cost-effectiveness of surgical treatment with reverse total shoulder arthroplasty (RTSA) compared with open reduction and internal fixation (ORIF) with a locking plate for patients 65-85 years old with a displaced proximal humerus fracture. METHODS: A cost-utility analysis was conducted alongside a multicenter randomized controlled trial, taking a health care perspective. A total of 124 patients with displaced proximal humerus fractures were randomized to treatment with RTSA (n = 64) or ORIF (n = 60) during a 2-year period. The outcome measure was quality-adjusted life ye

A Case of Proximal Humerus Fracture Dislocation Presenting With Failed Primary Open Reduction and Internal Fixation Followed by Salvage Reverse Total Shoulder Arthroplasty.

The treatment of proximal humerus fractures is complex. Factors like fracture pattern, patient age, pre-injury activity level, soft tissue status, and comorbidities play a role in decision-making for non-operative versus operative management. Complications of non-operative vs operative management of proximal humerus fractures include but are not limited to arthrofibrosis, fracture nonunion, and avascular necrosis. We report an unusual case of a 64-year-old female presenting with a three-part proximal humerus fracture dislocation. The patient underwent primary open reduction internal fixation.

Complications after surgical treatment of proximal humerus fractures in the elderly-an analysis of complication patterns and risk factors for reverse shoulder arthroplasty and angular-stable plating.

BACKGROUND: To date, there is a lack of consensus regarding surgical treatment recommendations for complex proximal humerus fracture (PHF) patterns, especially between joint preservation and joint replacement techniques. METHODS: Between 2012 and 2017, 146 patients (aged 74.1 ± 8.0 years) with complex PHF were treated with locking plates (open reduction-internal fixation [ORIF]) or reverse total shoulder arthroplasty (RTSA). Complications and unplanned revision surgery were recorded in a mid-term follow-up. Potential patient and surgical risk factors for complications were extracted. Univaria

Is the Posterior Approach With Posterior locking compression plate and Anterior Allograft Useful and Safe in the Treatment of Periprosthetic Humeral Fractures Following Reverse Total Shoulder Arthroplasty?

INTRODUCTION: As the reverse total shoulder arthroplasty (RTSA) surgery has dramatically increased in the last few decades, many complications have followed through. The periprosthetic fracture, at the moment, is still a subject of debate in the orthopedic world. In this monocentric study, along with a literature review of periprosthetic humeral fractures, we would present our institutional experience with the treatment of periprosthetic humeral fractures with a posterior humeral approach, posterior cortex plate fixation, anterior strut allograft, screws, and cerclage wires. Does delay matter? The restoration of objectively measured shoulder strength and patient-oriented outcome after immediate fixation versus delayed reconstruction of displaced midshaft fractures of the clavicle.

Outcome after surgical treatment for nonunion and malunion of midshaft displaced clavicle fractures has generally been described as favorable and equal to results of acute repair. This assumption has been based on subjective criteria, however, and no direct comparison is available in the literature. This study used objective measurements of limb function to compare outcome in patients who underwent delayed operative intervention for nonunion and malunion with the outcome of patients who underwent immediate open reduction and internal fixation after displaced clavicle fracture. All patients had
