A pull function attachment to manual wheelchairs: a case report on usability and shoulder pain in people with spinal cord injury.

INTRODUCTION: Shoulder pain mostly due to high biomechanical load of the anterior-shoulder musculature is prevalent among manual wheelchair users with spinal cord injury or disorder (SCI/D). This case study showcases a novel pull-function attachment to the wheelchair that reverses the propulsion motion by pulling rather than pushing the wheelchair. Additionally, the study reports its preliminary usability and impact on shoulder pain. CASE PRESENTATION: Four individuals with SCI/D (median age: 33; 1 female) used the novel pull-function attachment to the manual wheelchair for six weeks. Usabili

Effect of arm lengthening on functional outcomes and nerve injury after reverse shoulder arthroplasty: A prospective electrodiagnostic study.

BACKGROUND: The original Grammont Reverse Total Shoulder Arthroplasty (RTSA) increases the deltoid moment arm by medializing the centre of rotation, which is biomechanically advantageous. However, excessive tension can also produce complications, being one of the main risk factors for nerve injury. The purpose of the present study was to correlate arm lengthening after a medialized RTSA implant with functional outcomes and neurological injuries in the setting of rotator cuff tear arthropathy (CTA). METHODS: Prospective cohort study enrolled 40 consecutive patients who underwent RSA for CTA. E

Clinical outcomes of reverse shoulder arthroplasty in obstetric brachial plexus palsy with suprascapular nerve injury and glenohumeral osteoarthritis.

BACKGROUND: Untreated obstetric brachial plexus palsies (OBPP) present with various upper limb impairments, including muscle weakness, soft tissue contractures, and osteoarticular deformities. Reverse shoulder arthroplasty (RSA) medializes the center of rotation, optimizing deltoid function to compensate for deficits caused by OBPP. This study evaluates the effectiveness of RSA in patients with untreated OBPP involving the suprascapular nerve and end-stage glenohumeral osteoarthritis (OA), focusing on functional and quality-of-life improvements. METHODS: A retrospective analysis was performed

Age Influence Upon Glenohumeral Remodeling After Shoulder Axial Rebalancing Surgery in Brachial Plexus Birth Injury.

BACKGROUND: Shoulder rebalancing procedures have been proven to provide satisfactory functional improvements in patients with shoulder external rotation (ER) deficit due to brachial plexus birth injury (BPBI). However, the influence of age at the time of surgery on osteoarticular remodeling remains uncertain. The purposes of this retrospective case series were (1) to assess the age impact on glenohumeral remodeling and (2) to determine an age limit after which significant changes can no longer be expected. METHODS: We reviewed preoperative and postoperative magnetic resonance imaging data of

Subacromial-subdeltoid bursitis following COVID-19 vaccination: a case of shoulder injury related to vaccine administration (SIRVA).

Vaccination injection site adverse reactions are usually mild and transient, and post-vaccination musculoskeletal symptoms, such as myalgia and arthralgia, are very common. Shoulder injury related to vaccine administration (SIRVA), defined as shoulder pain and limited range of motion occurring after the administration of a vaccine intended for intramuscular administration in the upper arm, is a well-established condition in the medical literature, yet underreported. In such cases, subacromial-subdeltoid bursitis may occur, leading to shoulder dysfunction and ongoing pain. Millions of doses of

A protocol-based treatment plan to improve shoulder function in children with brachial plexus birth injury: a comparative study.

Children with permanent brachial plexus birth injury have a high risk of developing posterior shoulder subluxation. In 2010, we implemented a protocol to reduce the incidence of this deformity, including early passive exercises, ultrasound screening, botulinum toxin-A injections, shoulder splinting and targeted surgeries. Two-hundred and thirty-seven consecutive children treated at our institution, with a mean follow-up of 11 years (range 1 to 17) were compared in three groups: children born from 1995 to 1999 (n = 53), 2000-2009 (n = 109) and 2010-2019 (n = 75). Posterior shoulder subluxation

Anterior humeral circumflex artery avulsion with brachial plexus injury following an isolated traumatic anterior shoulder dislocation.

A 70-year-old man presented to accident and emergency with an isolated anteriorly dislocated shoulder, in the absence of a concomitant fracture. There was no neurovascular deficit at presentation, and the shoulder was reduced under sedation, using the Kocher's technique. Following this, the patient developed signs of hypovolaemic shock. Clinical examination revealed an expanding fullness in the deltopectoral area, with compromise of the limb neurovascular status. CT imaging confirmed an expanding haematoma from the axillary vessels, restricting left lung expansion. Once resuscitated, the patie

Structural characteristics of the subscapularis muscle in children with medial rotation contracture of the shoulder after obstetric brachial plexus injury.

The aim of this study was to obtain a better understanding of the cause of the medial rotation contracture of the shoulder after obstetric brachial plexus lesions by studying the morphology of the shortened subscapularis muscle. Muscle biopsy specimens were harvested from 13 children with obstetric brachial plexus palsy who underwent corrective surgery for the rotation contracture. The majority of the subscapularis muscle biopsy samples had an essentially normal morphology and showed a predominance of type I myosin heavy chain isoform, while one biopsy showed signs of marked fibrosis and a pre
