A 10-year comparative analysis of the 2 most common reverse total shoulder arthroplasty implants (Delta Xtend and SMR) in the New Zealand Joint Registry.

HYPOTHESIS AND BACKGROUND: As the incidence of reverse total shoulder arthroplasty (rTSA) continues to rise, better understanding of the long-term risks and complications is necessary to determine the best choice of implant. The majority (75%) of rTSAs performed in New Zealand use either SMR (Systema Multiplana Randelli; Lima-LTO) or Delta Xtend (DePuy Synthes). The aim of this registry-based study was to compare implant survival, risk of revision, and reasons for revision between the 2 most frequently used rTSA prostheses: SMR and Delta Xtend. METHODS: Using data from the New Zealand Joint R

Comparison of the analgesic efficacy of intra-articular steroid injections and its combination with suprascapular nerve block for adhesive capsulitis of the shoulder joint: a randomized clinical trial.

BACKGROUND: Adhesive capsulitis is a distressing ailment that progressively limits the active and passive mobility of the shoulder joint. Physical therapy (PT) combined with intra-articular steroid (IAS) injection and suprascapular nerve block (SSNB) has shown improved functional outcomes. We aimed to assess whether providing both IAS injection and SSNB improved outcomes compared with IAS injection alone. METHODS: Ninety-six patients with adhesive capsulitis were randomized into three groups consisting of group IAS, group IAS+SSNB and group PT (control). All patients were assessed on day 0, w…

Editorial Commentary: Why We Have To Respect The Anatomy In Acromioclavicular Joint Surgery And Why Clinical Shoulder Scores Might Not Give Us The Information We Need!

Over the past decade, the interest in acromioclavicular (AC) joint research has experienced a revolutionary increase. Biomechanical and anatomic studies have been carried out to investigate and obtain a better understanding of the function of this joint. The reason for this huge investigational effort is the fact that we do not have any gold standard for the treatment, diagnosis, or follow-up of patients with acute or chronic AC joint injuries. This is reflected by the huge number of over 160 different surgical techniques described in the literature. So far, we have a variety of anatomic proce

Effects of Adding Muscle Energy Technique to Joint Mobilization on Pain, Range of Motion, and Shoulder Function After Arthroscopic Rotator Cuff Repair: An Observational Cohort Study.

BACKGROUND Although muscle energy technique (MET) and joint mobilization (JM) are individually used in shoulder rehabilitation, evidence for their combined application following arthroscopic rotator cuff repair (ARCR) remains limited. This study evaluated whether adding MET to JM yields superior outcomes compared with JM in patients after ARCR. MATERIAL AND METHODS Forty patients who underwent ARCR were enrolled in this observational cohort study and assigned to either a control group (n=20) receiving JM or an experimental group (n=20) receiving MET in addition to the same JM. Shoulder functio

Perioperative testosterone therapy linked to higher 5-year risk of periprosthetic joint infection without increased 90-day major complications or revision rates in total shoulder arthroplasty.

INTRODUCTION: Preoperative testosterone exposure has been associated with various health outcomes, but its impact on postoperative complications in patients undergoing total shoulder arthroplasty (TSA) has not been fully explored. This study aims to evaluate the association between preoperative testosterone exposure and postoperative medical and surgical complications in patients undergoing TSA. METHODS: A retrospective cohort study was conducted using the PearlDiver database. Patients who underwent TSA and used preoperative testosterone within 3 months of the index procedure were identified

Conservative treatment of acute traumatic posterior shoulder dislocations (Type A) is a viable option especially in patients with centred joint, low gamma angle, and middle or old age.

PURPOSE: Purpose of this study was to evaluate the mid- to long-term outcome after conservatively treated first-time posterior shoulder dislocations and to determine structural defects associated with failure. METHODS: In this multi-centric retrospective study, 29 shoulders in 28 patients with first-time acute posterior shoulder dislocation (Type A1 or A2 according to the ABC classification) and available cross-sectional imaging were included. Outcome scores as well as radiological and magnetic resonance imaging were obtained at a mean follow-up of 8.3 ± 2.7 years (minimum: 5 years). The asso

Revision risk of cross-linked versus non-crosslinked polyethylene in reverse shoulder arthroplasty: an analysis of 63,502 cases from the Australian Orthopaedic Association National Joint Replacement Registry.

BACKGROUND: The effect of increased cross linking of modular polyethylene components in primary total stemmed reverse shoulder replacement (rTSA) upon revision rates is unknown outside biomechanical testing. We compared the rate of revision of primary stemmed reverse shoulder replacement with either a highly crosslinked (XLPE) or non-highly crosslinked polyethylene (non-XLPE) glenosphere or humeral cup. MATERIALS AND METHODS: Using data from the Australian Orthopaedic Association National Joint Replacement Registry, all primary rTSA with either polyethylene humeral cups or glenospheres were i

Magnetic Resonance Arthrography Findings of the Glenohumeral Joint in the Abduction External Rotation Position in Symptomatic Anterior Instabilities: Comparison Between Overhead Throwing Disorders and Traumatic Shoulder Dislocation.

BACKGROUND: The utility of indeterminate magnetic resonance (MR) imaging findings in identifying shoulder pathology in overhead throwing athletes is yet to be determined. The assessment of capsular redundancy on MR arthrography, specifically in the abduction external rotation (ABER) position, could help accurately distinguish between throwing disorders involving internal impingement and those involving anterior instability. PURPOSE: To compare MR arthrography findings in the ABER position between patients with internal impingement with anterior instability and those with traumatic shoulder di

Clinical Outcome and Quality of Life after Modular Reverse Total Shoulder Arthroplasty in Comparison with Joint-Preserving Locking Plate Osteosynthesis in Aged Patients: A Retrospective Comparison Study.

OBJECTIVES: Proximal humeral fractures (PHFs) show a high incidence in aged patients. While nondisplaced fractures achieve good results by conservative treatment, surgical procedures are discussed controversially. Next to open reduction and internal fixation (ORIF), the primary use of reverse shoulder arthroplasty (RSA) has become increasingly important. The aim of this study was to investigate the clinical function, activities of daily living (ADL), as well as pain assessment in patients with PHFs, treated by ORIF or RSA. METHODS: A retrospective comparison study was assessed. From November

Extra-Articular Abscess Expansion as a Specific Complication of Septic Arthritis of the Shoulder Joint: Report of Three Cases with Extension of the Infection through the Biceps Groove Producing a Large Abscess.

BACKGROUND: Septic arthritis of shoulder is a rare clinical entity as the metaphysis is extracapsular and there is no communication between epiphyseal and metaphyseal vessels. Septic arthritis of the shoulder joint is a diagnostic and surgical emergency because joint destruction develops rapidly and can cause significant morbidity and mortality. Unusual complications of septic arthritis of the shoulder joint may include extra-articular abscess extension to the upper arm through the biceps groove and osteomyelitis of the greater tuberosity. CASE PRESENTATION: Septic arthritis of the shoulder, …

Latarjet operation carries three times the risk of failure in seizure versus non-seizure recurrent anterior dislocation of the shoulder joint: outcome of a systematic review with meta-analysis.

BACKGROUND: Recurrent anterior shoulder dislocation (RASD) in cases of seizure disorders (SDs) total 50%-80% of all SD-associated shoulder instabilities. Based on the extent of bone loss, treatment options include bony and soft-tissue reconstructions, arthroplasty, and arthrodesis. The primary objective of this paper was to review the treatment options for RASD in SDs. METHODS: Several bibliographic databases were searched for RASD treatment options in SD patients. The demographic outcome measures, the failure rate (defined as the relative risk of recurrence of dislocation postoperation), and
